UNEXPECTED AIRBORNE RAD ACTIVITY IN THE RB Originator:
BRADLEY C SHUMAKER Supv Contacted:
T. Roberts Condition
==
Description:==
An unexpected increase in airborne radioactive activity occurred in the Reactor Building.
Local air monitors alarmed alerting Radiation Protection personnel who evacuated personnel from the RB. Many exiting personnel were unable to pass screening through the automated contamination monitors.Airflow from the RB passed through the Personnel Hatch as well, impacting the RP Control Point personnel in the Auxiliary Building.Operations secured RB Purge Ventilation supply flow to maximize purge ventilation exhaust and reduce air flow out of the RB hatches and OTSG construction opening.EP-AA-1009 was reviewed for impact on EALs and no criteria were exceeded.Assessment of the cause and the recovery plan are being developed.
Immediate actions taken: Evacuated personnel from the RB.Recommended Actions: Prompt investigation of cause.RB recovery plan to support resumption of work.Address personnel contamination issues.What activities, processes, or procedures were involved?To be developed during the prompt investigation Why did the condition happen?Uncertain at this time.What are the consequences?
Critical path and near critical path work has stopped inside the Reactor Building.Were there any adverse physical conditions?
See detail above.List of knowledgeable individuals:
T. Roberts, J. Goldman, J. Heishman 6-11q AR.- Assignment Report Page 2 0"f,29 , Operable Basis: Reportable Basis: Reviewed by operations for reportability.
The Exelon Reportability Reference Manual decision tree was reviewed, and no immediate reportability requirements were identified for the occurrence of the airborne radioactivity.
The event is not reportable under SAF 1.1, RAD 1.1, PAD 1.2, RAD 1.3, RAD 1.5, RAD 1.21, or RAD 1.22. The event needs to be evaluated for reportability under RAD 1.8, but there are no immediate reportability requirements under this section (30 day report). While the airborne activity event did not result in any immediate reportability requirements, media interest in the event did result in issuing a press release and an event of potential public interest (EPPI) per LS-MA-1254 TMI-02. As a result, it is reportable under SAF 1.9 (4 hours4.62963e-5 days
0.00111 hours
6.613757e-6 weeks
1.522e-6 months
). A report the NRC Operations Center via the ENS line was made at 01:13 on 11/22/09 (EN 455144) after issuing the EPPI at 22:45 on 11/21/09.SOC Reviewed by: CRYSTAL A WARLOW 11/25/2009 09:56:23 CST SOC Comments: SL3 as a result of meeting the following criteria contained in LS-AA-120, Att 2: "Contamination, airborne radioactivity, or radiation levels significantly above normal levels resulting from unplanned events.(HM Langley, TMI RP, 11/21/09)This event also meets SL3 due to > 1 day outage impact on critical path.Follow up to Prog Eng to document PINV (Hass 11/23/09)Per the Corp RP.Sr RP Technical Lead (G. This issue has no impact on the ability to safely operate the plant, to achieve or maintain the plant in a safe shutdown condition, or to mitigate the effect of any accident condition.
This issue does not identify an equipment deficiency that affects operability of any SSC. There are no Operability concerns.
See reportability basis for reportability determination.
Based on bounding calculations performed by Rad Protection, any airborne release from the Reactor Building was a small fraction of the criteria of EAL RU1. Nor did the event meet any of the criteria for entry into EAL RU2. No entry into the Emergency Plan was required.
This was reviewed and concurred by corporate and site Emergency Planning.1~tr*Ino-onhnanrrr,/I-

LLIP -McCarty: Develop Triage Procedure (LLIP) -Develop a corporate RP triage procedure.
This procedure will provide actions when dealing with a large number of personnel contamination events or workers with internal contamination.
Considerations within the procedure should include: creature comforts, alternate screening methods to release workers from the RCA without a WBC, corporate and site notifications, and the establishment of a log to track workers. This procedure should also include an evaluation for bringing in additional RPM's and RP Technical Support Managers to explain WBC printout sheets to workers prior to release from site while alarming a PM-7. Examples include: Ingestion versus inhalation potential, best estimate for elimination based on ingestion and inhalation flowpaths, biological and effective half-life.
The procedure should also contain an evaluation for additional RPM support following large scale refueling outage contamination events to support the site RPMs continued direct oversight and focus on refueling outage activities.
