Description:

Based onthe initial findings from the root cause report following the unexpected increase in reactor building airborne levels on 11/21/09, the TMI Radiation Protection Manager has requested that TMI station RP Airborne Engineering Controls be validated by RP personnel following a job site review of all plant installed portable HEPA ventilation systems and HEPA vacuums to ensure that procedure controls and requirements are being met. This validation will include a detailed review to ensure that all portable HEPA ventilation systems are installed and operating correctly, that there are no uncontrolled non-HEPA vacuums in plant'radiological controlled areas, that periodic inspections meet procedure guidelines, and that each HEPA and vacuum is properly logged out to the appropriate location.

Immediate actions taken:

Initiated a detailed inspection via a checklist of portable HEPA ventilation units and HEPA vacuums in use at TMI within the RCAs. The inspection will also ensure all non-HEPA wet vacuums are taken out of service.

Recommended Actions:

Results of the detailed inspections of portable HEPA ventilation units and HEPA vacuums and non-HEPA wet vacuums will be documented and corrective actions will be initiated as necessary based on these results.

All non-HEPA wet vacuums found in a RCA will be taken out of service by cutting the electrical cord.

Description:

While the RB Construction Opening Curtain (COC) was open in preparation for moving the new "B" OTSG into the RB, ribbons on the outside of the opening intermittently indicated flow out of the RB.

Immediate actions taken:

TMI-1 Standing Order for breach of the RB Liner was implemented.

Specifically, the actions required when outflow is observed were taken, including closing the COC. Radiation monitors at the construction opening indicated no airborne contamination was present. Based on this, no EAL thresholds are affected. RB ventilation and Aux Building ventilation parameters were reviewed and no anomolies were noted. Note that when the COC was closed, it billowed inward, indicating airflow into the RB.

Recommended Actions:

Continue to monitor conditions at the construction opening. Open the COC when required to transport the OTSG into the RB, and if outflow is still present take periodic flow measurements and perform bounding calculations for inclusion in ODCM reports for non-routine release.

Unknown. Suspect outside weather/wind conditions.

What are the consequences?

None other than observations need to be considered for ODCM non-routine release.

Were there any adverse physical conditions?

The COC was first opened at 18:30. Flow measurements taken at 19:30 indicated an average outflow of 145 fpm. The COC was closed at 20:00. Once closed, the curtain billowed in, and flow measurements at the cracks around the curtain indicated an average inflow of 148 fpm. The COC was reopened at 21:00 for final transport of the OTSG. Tell-tail ribbons intermittently indicated inflow and outflow. Flow readings will be taken every 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> while the COC is open for OTSG transport.

There are no operability or reportability concerns. Monitoring of air flo W and activity levels is in progress to support required reporting.

Sampling shows no signs of any airborne contamination. Required standing order actions are completed or in progress. This does not meet any thresholds for the E-Plan. Opening will be closed with the tarp once teh EOTSG is in the RB.
