Dear Mr. Keppler:

On May 8, 1986, Illinois Power Company (IP) notified Mr.

Jablonski, NRC Region III (Ref: IP Record of Coordination Y-200269 dated May 8, 1986) of a potentially reportable deficiency under the provisions of 10CFR50.55(e) concerning electrical lugs installed on the motor leads of certain Limitorque valve operators. Objective evidence was obtained which documents that the lugs installed on the motor leads of certain Limitorque valve operators are not the correct size. Our

investigation of this matter is continuing. This letter represents an interim report in accordance with the requirements of 10CFR50.55(e). Attachment A provides the details of our investigation to date.

We trust that this interim report provides you sufficient background information to perform a general assessment of this potentially reportable deficiency and adequately describes our overall approach to resolve this issue.

O ATTACHMENT A ILLINOIS POWER COMPANY CLINTON POWER STATION Docket No. 50-461 Potentially Reportable 10CFR50.55(e) Deficiency 55-86-02 Electrical Lugs Installed on Vendor Leads of Limitorque Operators Interim Report Statement of Potentially Reportable Deficiency / Background Illinois Power Company Quality Assurance Department initiated Condition Report-(CR) No. 1-86-04-042 which documented a potential problem with the lugs installed on the motor leads of Limitorque valve operators. Preliminary investigation of CR 1-86-04-042 identified motor leads for Limitorque valve operators which had incorrectly sized field lugs. Two (2) operators confirmed deficient (by dinassembly of the terminations) involved 18 gauge motor leads with 14-16 gauge lugs applied. Consultation with the lug manufacturer determined that this application was unacceptable.

Approximately 290 safety related Limitorque valve operators have had field lugs installed on the motor leads. Although lug sizes are generally available through site documentation, motor

' lead sizes were not furnished with vendor. documentation.

Additionally, the motor leads.are not physically marked with the wire size, and the terminations are covered with insulating tape or " heat shrink" which precludes direct observation of the lug / wire size.-

Investigation Results/ Corrective Action Illinois Power prepared and implemented an investigation plan to determine the extent of this problem at the Clinton Power Station (CPS). The results of our investigation to date are discussed below.

Preliminary investigation results indicated that valve operators with motors equal to or less than one-third (1/3) horsepower (which could have motor leads of 18 gauge wire) were most likely to have the wrong lug size installed.

Valve operators with motors equal to or less than one-third l (1/3) horsepower will be physically inspected to verify lug size and reworked as required. Twenty-eight (28) work documents

covering 189 valves have been initiated to date for accomplishment of this action.

Valve operators with motors greater than one-third (1/3)

horsepower will be verified by obtaining documentation of the motor lead sizes from the vendor and comparing this documentation i

ATTACHMENT A (continusd) with the on-site documentation which documents the sizes of the field installed lugs. Motor leads determined to have an unacceptable size lug installed will be reworked. To date, serial numbers for 83 motors have been obtained by walkdown and submitted to the vendor to obtain the necessary documentation for verification of the motor lead sizes.

In addition to the above action, applicable site procedures will be reviewed to determine the adequacy of these procedures with respecc to the installation of field lugs on conductors of unmarked sizes.

Root Cause A root cause investigation and evaluation is being aerformed. The root cause will be determined after further 3ackground information has been evaluated.

Safety Implication / Significance Illinois Power's investigation of this matter is continuing.

The safety implications and significance will be assessed after background information has been obtained and evaluated. It is anticipated that approximately thirty (30) days will be required to complete our investigation, determine reportability and submit a final report on the matter.
