Dear Mr. Keppler:

On May 7, 1985, Illinois Power Company notified Mr. F.

Jablonski, NRC Region III (Ref: IP memorandum Y-32264, dated May 7,1985) of a potentially reportable deficiency per 10CFR50.55(e) concerning inadequate design change control by Zack. Company._ Our investigation of this issue is continuing, and this letter represents an interim report in accordance with the requirements of 10CFR50.55(e)(3). Attachment A provides the details of our investigation to date.

Potential 10CFR50.55(e) Deficiency 55-85-05 Inadequate Design Change Control by Zack Company

-Interim Report Statement of Potentially-Reportable Deficiency / Background

.A condition potentially adverse to quality was identified in the area of design change control. This matter is based on concerns identified by Zack. Company's Corrective Action Request (CAR) No. ZCS-003. The concerns involve design change-documents for which'the review and determination for retroactivity had not been performed.

.Approximately 840 change documents were received by the Zack Company since the lifting of Stop Work Order (SWO) Nos. 14, j

15, and 20. These change documents were generic in nature and could potentially affect the adequacy of previously completed work, in that the change documents were neither-reviewed nor implemented retroactive by.the Zack Company. ,

Investigation Results ,

Illinois Power has prepared and is implementing an investigation plan to determine the extent of this deficiency at CPS. The investigation plan includes:

1. .A review committee has been designated to review all generic change documents-identified by the Zack Company since lifting.of SWO Nos. 14, 15 and 20, to determine those documents which were retroactive.

2. 'Sargent and Lundy.(S&L) will perform an indepen-i dent review of the documents identified by item HNo . 1.to: confirm the findings ~and submit final results to-Illinois Power (IP).

Corrective Action Sargent & Lundy has issued procedural revisions stating that each typical, standard, or generic Field Change Request (FCR), Nonconformance Report (NCR), Engineering Change Notice (ECN), Field Problem Report (FPR), Field Engineering Change Notice (FECN), and Nonconforming Material Report (NCMR) issued or dispositioned by S&L and applicable to the Zack Company will include an effective date.

Root Cause The complete scope and root cause of this issue have not yet been identified, such that a determination of complete remedial and generic corrective action can be made.

This determination will be made after further background information has been obtained and evaluated.

Safety Implications / Significance Illinois Power's investigation of this potentially reportable deficiency is continuing. The safety implications and significance will be assessed after further background information has been evaluated. It is anticipated that approximately sixty (60) days will be required to complete our investigation, determine reportability and to file a final report on the issue.
