SUPPLEMENT - SHUTDOWN TO REPAIR SMALL CRACK IN THE TORUS PORTION OF PRIMARY CONTAINMENT On July 7, a Special Inspection Team was dispatched to inspect and assess corrective actions for cracks in the torus shell and a shutdown cooling (SDC) pipe discovered by the licensee on June 27 and July 4, respectively. The inspection team completed its initial review of the licensee's repair work, preliminary root cause evaluations, extent-of-condition evaluations, and corrective actions prior to restart of the reactor. The special inspection will continue after the plant restart for further assessment, including a review of equipment issues that may constitute potential generic concerns.

Entergy completed a permanent repair of the torus shell in accordance with ASME code requirements and subsequently pressure tested the primary containment. The inspection team reviewed the licensee's repair plans, observed work in the plant, and reviewed the results of post-repair non-destructive examinations.

On July 4, during the plant shutdown for repair of the torus, Entergy identified a crack on the residual heat removal system's shutdown cooling (SDC) suction line. The SDC subsystems were declared inoperable based on the presence of a through-wall defect on an ASME Class 2 pipe. Notwithstanding the TS inoperability, the SDC suction line remained in service and fully capable of providing decay heat removal. Entergy repaired the crack and tested the SDC line in accordance with ASME code requirements, prior to restart of the reactor. While the SDC function is not credited in the FitzPatrick Safety Analyses, the system is designed to remove decay heat from the reactor coolant system. Alternate methods of decay heat removal were available.

The special inspection team's review is projected to be complete in August, and the inspection results will be documented in an inspection report within 45 days of the exit meeting. An NRC press release was issued on July 7 to inform the public that a Special Inspection was initiated for this event. In addition, the State of New York has been updated on current status.
