JAMES A. FITZPATRICK NUCLEAR POWER PLANT NOTIFICATION OF UNUSUAL EVENT DUE TO FIRE IN MAIN TRANSFORMER AND BUS DUCTING On November 11, 2012, at 3:55 a.m. EST, the James A. FitzPatrick Nuclear Power Plant reactor automatically tripped due to a main turbine trip signal. All systems responded as expected and operators stabilized plant conditions. Operators determined that the turbine trip was in response to a fire in one of two main transformers located outside and adjacent to the turbine building used to raise main generator output voltage to the grid voltage.

On-site fire brigade personnel with assistance from a local fire department extinguished the transformer fire and determined there was still an active fire in the transformer bus ductwork located exterior to the turbine building. At 5:45 a.m. EST operators declared an Unusual Event (UE) due to inability to extinguish the fire in the ductwork. As of 6:39 a.m. EST the fire in the transformer bus ductwork was extinguished. The licensee exited the UE at 8:01 a.m. EST.

The NRC Senior Resident Inspector responded to the site, inspected the location of the fire, evaluated Licensee response to the fire and the plant trip, and determined the plant was in a safe, stable condition. NRC inspectors continue to independently assess licensee actions in responding to the fire. Additionally, the inspectors are monitoring FitzPatrick staff actions to determine and correct the causes of the fire and activities associated with plant restart. The NRC Region I Public Affairs Office has responded to media inquiries.

The NRC notified New York state officials by telephone.

Region I will update this PN after plant restart.

Region I received initial notification of this occurrence from FitzPatrick at 6:08 a.m. The NRC remained in the normal mode of operation throughout this event. The information presented herein was discussed with Entergy and is current as of November 13 at 12:30 p.m.
