SUMMARY

INTRODUCTION The ASME Boiler and Pressure Vessel Code,Section XI, 1989 Edition (Code), is the applicable code for conducting inservice inspection activities during the third ten-year inspection interval at Farley Unit 1. Examinations and tests required by the Code are scheduled in accordance with "Inspection Program B" as defined in Code paragraphs IWB-2412 and IWC-2412. This "Owner's Report for Inservice Inspection" is for those third inspection period examinations and tests which were performed between May 22, 2006 (date of last examination performed during the twentieth maintenance/refueling outage, 1R20 / 3-3-2) and November 10, 2007 (date of last examination performed during the twenty first maintenance/refueling outage, 1 R21 / 3-3-3).

The examinations were performed in accordance with the approved Examination Program Plan.

The primary areas of examination included Reactor Pressure Vessel (RPV) welds (10-year ISI),

Pressurizer (PZR) and Steam Generator (SG) dissimilar metal welds and various Class 1 and Class 2 piping welds and supports. The tables documenting the Class 1 and 2 vessel and piping examinations as well as component supports examined are under Tab C of this report.

Farley Nuclear Plant (FNP) has implemented ASME Section XI, Appendix VIII, "Performance Demonstration for Ultrasonic Examination Systems," as required by 10CFR 50.55a, amended by the Federal Register Notices 64 FR 51370 dated September 22, 1999, as later modified by 69 FR58804, dated October 1, 2004. The initial Rule specified the 1995 Edition with Addenda through 1996 as the applicable edition of ASME Section Xl, Appendix VIII. The 2004 Rule incorporated the 2001 Edition with NRC caveats. FNP has, except where requests for relief, technical alternatives or exemptions have been approved, complied with the requirements for expedited implementation of the applicable Appendix VIII supplements using the EPRI Performance Demonstration Initiative (PDI).

In addition to the summary of inservice inspection activities, this report addresses Code-applicable repairs and replacements documented at FNP Unit 1 since outage 1 R20 through the end of outage 1 R21. Class 1 and Class 2 Owner's Reports for Repairs or Replacements (Form NIS-2) are provided herein.

It should be noted that Authorized Nuclear Inservice Inspector (ANII) inspection services were provided for those examinations and tests required by the Code. ASME Section XI examinations and tests are itemized in the applicable sections by reference to Examination Category.

Examinations which do not meet the Code-required coverage either reference request(s) for relief or indicate that additional relief is required.

Steam Generator Tube Inspections are required to be reported per FNP Technical Specification 5.6.10. No eddy current inspections were performed during the 1R21 refueling outage.

Also included in the 1 R21 outage were six PZR nozzle dissimilar metal weld overlay application and examinations. The summary and results of these examinations were submitted under a separate cover letter (NL-07-2116) to the NRC on 11/12/2007.

RESULTS Certain examinations resulted in recordable indication areas being noted on the basis of procedure recording criteria, which generally are more conservative than specified in the ASME Section XI Acceptance Standards. Indications were evaluated and dispositioned by Indication Evaluation Reports (IER's). A summary of the recorded indications and their associated IER's is listed immediately following this text section. Data sheets are available at FNP for review. The results are summarized below.

SUMMARY

OF INDICATIONS CLASS 1 (A)

VOLUMETRIC EXAMINATIONS Automated ultrasonic examinations of the Farley Unit 1 RPV yielded a total of twenty-one recordable indications. Flaw bounding measurements were performed in accordance with Appendix VIII qualified procedures and examiners. All of the indications were assessed in terms of the criteria in ASME Section XI, 1989 Edition and were found to be within acceptable limits. Specific details are contained in the IER section of this report.

During 1 R21 volumetric inspections, manual inspections yielded no recordable indications.

In addition to components already identified in current requests for relief, ten Class 1 welds had limited volumetric coverage during UT examinations because of physical limitations due to the geometric configuration of the welded areas. More than ninety percent of the required volume must be examined as addressed in ASME Section XA Code Case N-460 for adequate ASME Section Xl Code-required examination coverage to be attained. The subject code case has been approved by the NRC for use as documented in NRC Regulatory Guide 1.147. It is impractical to achieve the ASME Section XI Code-required coverage due to the geometric configuration of the welded areas. These limited examinations will be submitted to the NRC through the relief request process as allowed by 10 CFR 50.55a.

SURFACE EXAMINATIONS 0

There were no reportable Class 1 surface indications.

(C)

VISUAL EXAMINATIONS There were twenty seven bolted connections from the Class 1 pressure tests with various degrees of boron accumulation. In each case the boron was removed and either an evaluation or a re-examination found each one acceptable.

CLASS 2 (A)

VOLUMETRIC EXAMINATIONS There were no Class 2 volumetric indications.

In addition to components already identified in current requests for relief, one Class 2 weld had limited volumetric coverage during UT examination because of physical limitations due to the geometric configuration of the welded areas. As with the Class 2

1 limitations, these limited examinations will be submitted to the NRC through the relief request process as allowed by 10 CFR 50.55a.

SURFACE EXAMINATIONS 0

There were no reportable Class 2 Surface indications.

(C)

VISUAL EXAMINATIONS There were no reportable Class 2 Visual indications.

ADDITIONAL EXAMINATIONS Results from additional examinations which were performed during this outage are as follows:

Class 1 System Leakage Test In accordance with ASME Section XI 1989 Edition IWB-5210(a)(1), leak testing of the Class 1 Reactor Coolant Pressure Boundary was performed at the end of the 1 R21 refueling outage. The testing was completed on November 10, 2007. A copy of the completed test procedure, FNP-2-SOP-1.4, is retained by the FNP Document Control.

Dry boron was noted at twelve locations. In each case the boron was removed and necessary corrective action was taken. Results of this testing is documented in FNP STP-157, Indication Evaluation Report 004.

In addition, to meet the 1989 Section XI IWA-5242(a) requirement for removal of insulation from bolted connections in systems borated for the purpose of controlling reactivity, the alternative examination requirements of Relief Request RR-27 were utilized and visual testing of uninsulated Class 1 bolted connections at static pressure was performed. There were twenty-seven bolted connections with some degree of boron accumulation; one connection had affected bolting. The boron was removed and necessary corrective action was performed. Results of this testing are documented in FNP-1-STP-157 Indication Evaluation Report 004. The Class 1 leakage test was performed and the Code requirements were satisfied.

Class I and 2 Hydrostatic testing Using Code Case N-498-1, the hydrostatic testing requirements of ASME Section Xl, 1989 Edition, IWB-5210(a)(2) and IWC-5210(a)(3) were met by conducting a system leakage test which extended to all Class 1 and Class 2 piping within the system boundary. Results of this testing are documented in FNP-1-STP-156.2. One section of Class 1 piping (Pressurizer Auxiliary Spray Line), as described in Relief Request RR-60, is being deferred until the R22 Outage.

" Class 2 Functional/Inservice Testing Class 2 function testing performed during the 1R21 Outage included a portion of the Safety Injection systems. This exam is documented in FNP-1-STP-156.1. This completed all required testing for the third inspection interval.

IWE/IWL Examinations Inservice inspection examinations were completed on the Unit 1 Containment Building in accordance with the 1992 Edition with the 1992 Addenda of the ASME Code, Section XA, 3

Subsections IWE and IWL during the current inspection interval. The IWE VT-3 visual examination of the containment liner was completed in during the 1 R21 Outage and is documented by work order 1052498301. The Unit One 30-Year IWL tendon surveillance was completed in May 2006 which is documented in work order 1052742001. The results of these surveillances are available at FNP for review.

STATUS OF EXAMINATIONS REQUIRED FOR CURRENT INTERVAL This 1R21 Outage was the 3 rd Outage, 3rd Period of the 3 rd Interval and the examinations completed to date represent 100% of the total Class 1 and 2 scope for the current period and interval. 100% of the examinations required for the current interval have been completed. FNP Unit 1 is within ASME Code compliance, pending approval of the volumetric and/or surface examination limitations. Requests for relief will be submitted within the required time frame specified by 10 CFR 50.55a.

17. Description of Work A bare metal visual exam was performed on the bottom of Q B33IK(000I. A small white residue that resembled boron was observed on two penetrations (#1 and #2). As a part of the investigation, heaters I and 2 were cut out to allow access for NDE equipment. The NDE concluded that all welds were satisfactory. Heaters I and 2 were replaced with new heaters. Ref: Transaction # 124502

7. Description of Work Q1 NI IPV3371A was disassembled for inspection. The cage gasket landing was found to be eroded by steam. The landing was welded and machined back to manufacture's specifications tunder the technical guidance of a manufacturer's representative. The cage gasket landing is not a pressure retaining portion of the valve, however welding to the pressure retaining portion of the valve was required for this repair.
