In Reference 1, the Nuclear Regulatory Commission (NRC) issued one white finding in the Mitigating Systems Cornerstone to NextEra Energy Duane Arnold, LLC (hereafter NextEra Energy Duane Arnold), for failure to prescribe a work instruction of a type appropriate to the circumstances for the re-assembly of the "A" standby diesel generator lube oil heat exchanger.

In Reference 2, the NRC issued one white finding in the Mitigating Systems Cornerstone to NextEra Energy Duane Arnold, for failure to perform an immediate operability determination in in accordance with licensee procedures when a reactor core isolation cooling (RCIC) system turbine speed indicator was found degraded.

Document Control Desk NG-14-0126 Page 2 of 2 References 2 and 3 indicate the NRC will conduct an inspection using Inspection Procedure 95002, "Supplemental Inspection for One Degraded Cornerstone or Any Three White Inputs in a Strategic Performance Area," upon being notified of NextEra Energy Duane Arnold's readiness for inspection.

NextEra Energy Duane Arnold has completed investigations to determine the root and contributing causes of the individual and collective performance issues associated with the degraded cornerstone. The extent of condition and extent of cause of the individual and collective performance issues have also been evaluated, and a determination has been made whether safety culture components were causes of, or contributors to, the issues. Corrective actions have been developed to address the root and contributing causes and to prevent recurrence of the significant performance issues. NextEra Energy Duane Arnold believes the corrective actions taken and planned will address the issues and provide the basis for sustained, improved performance at Duane Arnold Energy Center.

NextEra Energy Duane Arnold is notifying the NRC of readiness for the 95002 Supplemental Inspection and is requesting the inspection begin the week of May 26, 2014.
