. SUBJECT: NOTICE OF VIOLATION AND NOTICE OF DEVIATION

(INSPECTION REPORT NOS. 50-413/89-16.50-414/89-16)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

W. T. Orders and M. S. Lesser on May 28,'1989 to July 1, 1989.- The inspection

included a review of ectivities authorized for' your Cataw'oa facility. At the-

conclusion of the inspection, the findings were discussed with those members of

your staff ider,tified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. ..Within- j

these areas, the inspection consisted of selective _ examinations of procedures.

and representative records, interviews with personnel, arsi observation = of

activities in progress.

The inspection findings indicate that certain activities appeared to violate.

NRC requirements and other activities appeared to deviate from applicable. codes i

approved by the NRC. The violations and deviation, with pertinent references l

and elements to be included in your fesponse, are presented in the enclosed 4

Notices.

Your attention is invited to the unresolved item identified in the inspection j

report. This matter will be pursued during future inspections. j

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,  !

Title 10, Code of Federal Regulations, a' copy of this letter- and its enclosures -

' will be placed 'in the NRC Public Document Room.  !

The responses directed by this. letter and the enclosures are not subject to the  ;

L clearance procedures of the Office of Management and Budget as required by the l
