- 5. 1.0 Summary of Inservice Inspection

.This report describes the Inservice Inspection of Duke Power Company's McGuire . Nuclear Station Unit I during. the 1988; Refueling Outage (also referred to as Outage 5).
Included in this report are the final. inservice . inspection plan,. the inspection results for each item, a summary for each category' of exami-nations and corrective action taken when . unacceptable conditions were found. In addition, there is a section included for repairs and replace-ments required since November 11, 1987.
1.1 Class 1 Inspection The Class ,1 Inservice Inspection included examinations on the Reactor Vessel (CE), Pressurizer- (Westinghouse) Steam Generators 18 and IC (Westinghouse), piping welds of Reactor Coolant System (NC), Safety Injection System (NI) and pump bolting.
Visual examinations were performed on the Class 1 Pressure Boundary during inservice leakage tests. Also, visual examinations were performed on Class 1 Component Supports of the Safety Injection (NI) System.
The Inconel 600 Tubing in Steam Generators IA, IB, 1C 'and ID was inspected .by eddy current during Outage 5. The results of the inspec-tions are shown in Section 5 of this report.
. /O V Reportable indications were found on the Class 1 Inspections shown on the following pages. Inspection and evaluation data for each reportable indication found on Class 1 Items is included in Section 5 of this report.
A detailed description of each inspection is found in the final Inservice Inspection Plan in Section 3 of this report. Results of each examination are found in Section 4.
1.2 Class 2 Inspection The Class 2 Inservice Inspection included examinations on Steam Generator 10, Safety Injection Accumulator Tank 18, and circumferential butt welds of Auxiliary Feedwater (CA), Main Feedwater (CF), Main Steam Supply to Auxiliary Equipment (SA), Main Steam (SM), Containment Air Release and Addition (VQ) and Main Steam Vent to Atmosphere (SV) Systems.
A detailed description of each inspection is found in the final Inservice Inspection Plan in Section 3 of this report. Results of each examination are found in Section 4.
1.3 Augmented Inspection Augmented Inspections at McGuire 1 consisted of Reactor Ccolant Pump n Flywheel examination, Steam Generator Tubing examination, Steam Generator

- Steam Generator "A" and "C" were examined to the 2nd - TSP.'
The inspection ' results for Steam Generator "A" are as follows: There were twenty-five (25) tubes found showing degradation . 20% thru 39%
through-wall (TW). Twenty-eight (28) tubes were found showing degrada-tion 40% and greater through wall (TW). Twenty-eight (28) tubes were removed from service due to bobbin, Eddy-360, F* Calculation, and from good engineering practice.
The inspection results for Steam Generator "B" are as follows: There were four (4) tubes found showing degradation 20% thru 39% through-wall (TW). Twenty (20) tubes found showing degradation 40% and greater through-wall (TW). Twenty-seven (27) tubes were removed from service due
- to bobbin, Eddy-360, F* Calculation, and from good engineering practice.
The inspection results for Steam Generator "C" are as follows: There were four. (4) tubes found showing degradation 20% thru 39% through-wall (TW). Twenty (20) tubes found showing degradation 40% and greater through-wall (TW). Thirty-one (31) tubes were removed .from service due to bobbin, Eddy-360 and F* Calculation, and from good engineering practice.
The inspection results for Steam Generator "0" are as follows: There were four (4) tubes found showing degradation 20% thru 39% through-wall (TW). Sixty-seven (67) tubes found showing degradation 40% and greater through wall (TW). Thirty-nine (39) tubes were removed from service due to bobbin, Eddy-360, F* Calculation, and from good engineering practice.

. Inspections were performed on Steam Generator 10, Safety Injection Accumulator Tank 18, and piping welds of . Auxiliary Feedwater, Main Feedwater, Main- Steam Supply to Auxiliary Equipment, Main Steam, Containment Air and Release, and Main Steam Vent to Atmosphere' Systems.
6.1 Steam Generators

Steam Generator ID Transition Cone-to-Shell Weld received- an ultrasonic-examination, and Auxiliary Feedwater Nozzle-to-Shell Weld received
. ultrasonic and dye penetrant examinations. No reportable indications were found.
6.2' LSafety Injection Accumulator Tanks The Safety Injection Accumulator Tank 1D Shell-to-Lower Head Weld originally. scheduled for Outage 5 was inaccessible for preparation for the required volumetric examination. Safety Injection Accumulator Tank 1B Shell-to-Upper Head Weld was substituted and examined by ultrasonics.
No. reportable indications were found. See Request for Relief MNS-002 included in Section 10 of this report.
6.3 Pipina Nine (9) circumferential welds one-half inch and less nominal wall G

. thickness received a magnetic- particle examination. Two (2) circumferential welds one-half inch and less nominal wall thickness received a dye penetrant examination. No reportable indications were found.
Two (2) circumferential welds greater than one-half inch nominal wall thickness received an ultrasonic and magnetic particle examination. No reportable Indications were found.
Two (2) integrally-welded attachments on the Main Feedwater System and one (1) integrally-welded attachment on the Main Steam System received a magnetic particle examination. No reportable indications were found.
6.4 System or Component Functional Test A Class 2 Functional Test was performed as required by ASME Section XI, Article IWC-5000. No reportable conditions were found.
All of the required Class 2 Functional Tests for the Second Inspection l: Period have been completed.

%)j 1 Augmented Inspections were performed on the Reactor- Coolant Pump 1 Flywheels, Steam Generator Tube Preheater Sections, Steam Generator Feedwater Modification'and Thermal Stress Piping.
7.1 Reactor Coolant Pump Flywheels Reactor Coolant Pump 1A Flywheel was .remov'ed and was examined by ultrasonic and magnetic : particle- methods. Reactor Coolant Pump 10 received an. ultrasonic examination in place. No reportable indications i were found.
7.2 Steam Generator Tube Preheater Section l All four steam generators were examined by eddy current methods. Results are shown in Section 5 of this report.
- 7. 3 Steam Generator Feedwater Modification A , visual examination was performed on Steam Generator 1B Feedwater Modification. No reportable indications were found.
7.4 Thermal Stress Pipina Examinations e
Twelve (12) welds and three (3) bends were examined by ultrasonic volumetric examination on Reactor Coolant System (NC), as required by

Code Requirement that has been determined to be ASME Bo,ler and Pressure Vessel Code Section XI, 1980 Edition, including-1980. Winter Addenda, article IWC-2500-1, items C01.010.. The code requires the inspection of one Shell to Upper equivalent of one Head and one Shell to Lower Head to complete the vessel.
III. Basis for Requesting Relief The Safety Injection Accumulator Tanks 1A,1B,1C,1D are manufactured by Westinghouse. The tanks are designed in such a way that the welds (Shell to Lower Head) are inaccessible to prepare for ultrasonic examination, due to support-skirt interference.
