TITLE: TRANSITIONAL GUIDELINES TO NEW TRAINING MANAGEMENT PROCEDURES i ISSUANCE - ( w f 'e. f.160's f PORC EFFECTIVE REVIEW PORC 6 8 8 SEP11JS85 DATE 9 SS The Nuclear Production Division Training Section is currently developing a new training development system to satisfy the training program accreditation requirements of the Institute of Nuclear Power Operations (INPO). Training Management Procedures have been prepared to establish the specific requirements and administrative controls necessary to achieve and maintain INPO accreditation of the Fort St. ; Vrain training program. Accreditation of the training program requires the entire development process to be completed for specific training units prior to training (- implementation. In order to- fulfill current training-commi-tments - ( until the accredited program is fully implemented, exceptions must be made to the administrative controls contained in the Training Management Procedures. Full implementation of the Operations training program is required by 1 January 1986. Implementation of the remainder of the training programs is required by 1 January 1987. As a result, the following guidelines shall apply until the Nuclear Production Division Training Section Programs are accredited.
- 1. The new administrative requirements regarding Training ,
Department Administration, Training Program Administration, + Training Curriculum Planning and Development, and Evaluation , shall apply to all, such activities unless specified below. ! 1
- 2. The Training Department Training Program, TPAM-T, shall be used for qualification requirements specified in TMP 1.4,

INSTRUCTOR ASSIGNMENT AND QUALIFICATIONS, until TP-TI, Training Department Training Program is implemented. ,
- 3. Existing procedures issued in the Training' Programs Administrative Manual, TPAM, shall be used for the training procedure implementation requirements specified in TMP 2.1, TRAINING PROGRAM IMPLEMENTATION.

3 7 GENERAC-All training supervisors are responsible for the activities of the instructors assigned to them. They are responsible for instructor workload reviews, employee discipline and corrective ! actions, new employee orientation, employee work assignments, establishment of work goals, subordinate motivation, instructor candidate interviews, employee hiring, employee promotion recommendations, discharge recommendations, time card approvals, , schedule change approvals, meal ticket and mileage approvals, material request approvals and other personnel programs and procedures in accordance with established company policies. 4.0 PROCEDURE 4 .1- ORGANIZATION The organizational structure for the Nuclear Production Division Training Section is depicted by Attachment TMP 1.la. 4.2.1 Superintendent of Training The Superintendent of Training reports to the Support Services Manager and is responsible for all training activities conducted by. or through the i Training Section 4.2.2 Training Support Supervisor The Training Support Supervisor reports to the Superintendent of Training and is responsible for the accreditation of all Nuclear Production Division Training Programs in the areas of Operations, Shift ; Technical Advisors, Instruments and Controls, j Electrical Maintenance, Mechanical Maintenance, l Chemistry, Radiation Protection, and Technical Staff and Management. The Training Support Supervisor is also responsible for the administration and coordination, with Nuclear Production Division supervisory and management personnel, of the following General Training Programs:

() ' General Employee Training and Retraining Program designed to familiarize new employees. with descriptions of plant design, plant policies, emergency response, safety, Health Physics and Security and to provide annual retraining as required. Fire Fighting Program designed to initially train

Instructor Training Program . designed to establish and maintain individual instructor technical I competence, including theoretical and practical knowledge, work experience, oral and written communication abilities, interpersonal skills and instructional capabilities. 4.2.3 Operator Training Supervisor V The Operator Training Supervisor reports to the Superintendent of Training and is responsible for all activities associated with the training of Licensed and Non-Licensed Operators. ihe Operator Training Supervisor provides coordination with Nuclear Production Division supervisory and , management personnel for program effectiveness and technical content of the following programs: Licensed Candidate Program which prepares operating and engineering personnel for successful completion of Nuclear Regulatory Commission examinations to become licensed Equipment Operators and Licensed . Reactor' Operators. Water Chemistry Training Program designed to ensure that a qualified Water Chemistry staff is maintained to evaluate and control plant water quality to prevent degradation of plant systems due to corrosion and to maintain overall plant operating " efficiency. VRAIN NUCLEAR GENERATING STATION Page 3 of 4 O SOTVICO' PUBLIC SERVICE COMPANY OF COLORADO O 1.0 PURPOSE The purpose of this procedure is to assure that personnel l assigned instructional duty possess the appropriate technical i qualifications and instructional skills needed to maintain the highest possible quality of training. 2.0 APPLICABILITY This procedure applies to all Training Section Instructors and Public Service Company of Colorado personnel utilized by the Nuclear Production Division Training Section. 3.0 CENERAL 3.1 DEFINITIONS 3.1.1 Appropriate Supervisor-A supervisor who has been given the responsibility, as defined in TMP 1.1 TRAINING SECTION ORGANIZATION AND RESPONSIBILITIES, to insure a course or unit of instruction is taught. 3.1.2 Primary Instructor-An instructor who has been given the primary responsibility to instruct a course or unit of instruction. 3.1.3 Designated Alternate Instructor-An instructor who is qualified to instruct a given unit or course of instruction in the event the primary instructor is not available. 4.0 PROCEDURE 4.1 INSTRUCTOR ASSIGNMENT RESPONSIBILITIES It is the responsibility of the appropriate Supervisor to assign qualified instructors as the primary instructor for a given course of instruction. In the assignment of instructors, the Supervisor should consider the workload of the instructor, qualifications of the instructor and designation of the instructor as to the type of training to be accomplished (i.e. In plant versus classroom). Instructor assignment schedules are norrally developed on a quarterly basis in accordance with TMP 1.5, TRAINING SECTION EVALUATION. In the event an instructo who has not attained the required instructional qualifications for a course is assigned to a course or unit of instruction, the instructor i shall work under the supervision of an instructor qualified in that course or unit of instruction. The qualified l instructo.r will periodically observe the performance of the instructor during the course or unit of instruction to O ensure that the objectives of the course are mot. l In the event the primary instructor for a given unit of i l instruction is not available, an alternate instructor may be s elected by the appropriate Supervisor for the course. If the alternate instructor is not under the supervision of the appropriate Supervisor, the Supervisor should submit a written request for the . instructor's. assignment to the Supervisor of the instructor. If conflicts in instructor assignments occur, the Superintendent of Training will make the final. determination as to the instructor assignment. 4.2 INSTRUCTOR QUALIFICATIONS It is the responsibility of the appropriate Supervisor to ensure the primary instructor has the technical qualifications appropriate for the course .or unit of instruction based on the instructor's qualification status l in accordance with the appropriate Training Procedure. All instructor qualifications shall be maintained in accordance with TP-TI. It is the responsibility of every instructor to maintain his/her individual instructor qualification information by providing the Training Section Nuclear Training Documents Specialist with the appropriate information to update the record.

This procedure also describes the method for maintaining INPO accreditation of the Nuclear Production Division training programs. 2.0~ APPLICABILITY This procedure applies only to the Nuclear Production Division

Minutes for the meetings shall be kept on file for documentation of the evaluation of the ! facilities in accordance with TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. 4.3 On a bi-ennial basis, the Training Section shall conduct an Accreditation review of the training program based on the established Institue of Nuclear Power Operations (INPO) accreditation criteria contaired in 'INP0 85-002, The Accreditation of Training in the Nuclear Power Industry. l The ASER is developed by members of the Training staff and reviewed by the unit Supervisors. The final ASER shall be approved by the Superintendent of Training. The final ASER shall be submitted to the INPO for reaccreditation of the l training programs. Action items developed in the ASER review may be addressed in the Annual Planning Guidelines Meeting for resolution. l H ' review ~of'the plant-specific task lists used 'as the basis-for the training program. The review shall be conducted for the job positions listed in TMF 3.1, JOB AND TASK ANALYSIS. The current ravision of the Job Surveys reviewed in TMP 3.1 are reviewed by the appropriate Subject Matter Experts, in- i plant Supervisors, and Training Instructors to verify the adequacy of the task list. Any modification of the task list identified in the review shall -be documented and evaluated using a Training Development Request (TDR), TMP 4.1, TRAINING PROGRAM EVALUATION. Tasks added to the task list shall be analyzed in accordance with TMP 3.1, JOB AND -TASK ANALYSIS, if appropriate. Performance of the task list review is the responsibility of the Training Support Supervisor. 4.5 The Superintendent of Training shall submit a monthly operations summary to the Support Services Manager. Each , Training Supervisor shall submit a monthly unit operation summary to the Superintendent of Training. These summaries shall be submitted in the form of a PPC. 3.0 GENERAL l l Implementation of training activities shall be in accordance with written approved procedures derived in Section 3.0, TRAINING CURRICULUM PLANNING AND DEVELOPMENT. 4.0 PROCEDURE 4.1 TRAINING PROCEDURE IMPLEMENTATION A Training Procedure, TP, for each program listed in Procedure TMP 1.1, TRAINING SECTION ORGANIZATION AND RESPONSIBILITIES, which requires accreditation is developed ' in the design phase of curriculum planning and development. j t gi The Training Procedure shall be used as the primary < d management tool for implementing training to ensure that instructional activities are conducted reliably and , consistently. It is the responsibility of the appropriate Supervisor to ensure compliance with the Training Procedure, i Waivers of training requirements may be granted based on the waiver criteria established in individual Training Procedures. Waivers of training requirements shall be ' documented and evaluated on Attachment TMP 2.la, Training . Waiver Request. The training shall be conducted ir, accordance with the appropriate Training Procedure using the lesson plans and training materials provided by the curriculum development process, TMP 3.3. Prior to instruction, the Instructor copies the current revision of the Lesson Plan Master Copy maintained in the Training Reference Library. The Instructor shall ensure that all Training Development Requests (TDR), are completed and personalize the lesson plan to become familiar with the lesson content and learning activities. The Instructor is expected to use instructional techniques appropriate to the lesson content and learning objectives. When individualized instruction is used, training materials- and referenced texts. will be readily available. At the conclusion of a course or area of instruction, the Instructor will provide the trainees with a Trainee Course Evaluation Sheet in accordance with TMP 4.1, TRAINING PROGRAM EVALUATION. l 4.2.2 In-plant and Laboratory Training

The training shall be conducted in accordance with the appropriate Training Procedure using lesson plans provided by the curriculum development process, TMP 3.3, CURRICULUM DEVELOPMENT. Prior to instruction, the Instructor shall review the course Lesson Plan to ensure he/she is familiar with the lesson content and learning activities. During the training, the lesson plan is used by the instructor to outline the proper actions in sequence to accomplish the required task. When the actual task can not be performed but is simulated or walked-through, the conditions of task performance, references, tools and equipment shall reflect the actual task to the extent possible. These evaluations will be written or oral examinations developed during the curriculum design phase based on-the learning objectives for the course. Quizzes may be .used; in addition to tests, to supplement the evaluation process. Security of tests and answer keys shall be maintained by Training personnel during storage, reproduction and testing of trainees to
. prevent compromise of training material. ,
Written tests shall be corrected and graded within one(1) week after the completion of a course, l rotation or unit of instruction. The tests are made j () available for review by the trainees to enhance i V benefits derived from the test. The criteria for l successful completion of a course shall be 80% i unless specified by specific Training Procedures, i The test scores and the Trainee Course Evaluation Sheets (TMP 4.la) should be forwarded to the appropriate Training Supervisor for evaluation of ! program modification in accordance with TMP 4.1, TRAINING PROGRAM EVALUATION. l 4.3.2 In plant and Laboratory Trainee Evaluations i In plant and laboratory evaluations shall be administered by an Instructor or his. qualified designee during the training in the form of the Job l Performance Measure (JPM). The trainee shall perform, simulate or walk-through the task specified in the JPM given initial task conditions. The Instructor or qualified designee will document the trainee's ] performance in the JPM. Successful completion of the ! JPM requires all actions to be completed as stated in ( the JPM. Deficiencies in performance of the task i will be identified, a pass / fail grade given and the I trainee advised of the results promptly. ./ 3 - s %f 1.0 PURPOSE ..Thi s procedure provides the method for performing job and task analysis.
2.0 APPLICABILITY This procedure applies to all Nuclear Production Division training programs that require performance based training either as a result of an Administrative decision or pursuant to any program accreditation commitments. 3.0 GENERAL 3.1' Job and Task Analysis may be divided into two separate processes, Job Analysis and Task Analysis, with the ultimate output of the procedure being a tabulation of skills and knowledges that relate to each task that selected personnel perform. 3.2 Definitions which fpply to this procedure are listed in Attachment TMP 3.la. 3.3 It is the responsibility of the Training Support Supervisor

- a. Job descriptions / Position information questionnaires
- b. Technical Specifications
- c. Administrative Procedures
- d. System Operating Proceduras
- e. Specific Departmental Procedures
- f. INPO Job Specific Task Lists
- g. Training Development Request (TDR)
The initial task list is divided into duty areas which relate to the tasks functional performance. Plant procedures used to perform the task may be p includsd in the list to assist Subject Matter Experts g in review of the list. Additional tasks after the completion of the initial task list shall be developed using the Training Development Request (TDR), TMP 4.1, TRAINING PROGRAM EVALUATION. Upon completion of the initial list, the list is distributed to Subject Matter Experts from the respective departments to be verified. 4.1.2 Task Verification The initial task list or TDR is reviewed by the department Subject Matter Experts to ensure that the tasks are accurate and appropriate for the department. During the initial review, tasks may be added or deleted to improve the accuracy of the list. Upon completion of the review, the original task list and comments from the review is returned to the Training Support Unit for incorporation of the comments into the list. A modified task list, based on the comments, is produced to develop a Job Survey. The initial task list and comments are normally maintained on file for a period of no longer than one (1) year by the Training Section at which time they are sent to Records Storage or disposed of at the discretion of the Superintendent of Training. The
- Task lists and comments may be retained for longer than one year if desired.

The purpose of the survey is to determine those tasks which should be included in the training program based on the frequency, difficulty, and the importance of task performance. The Job Survey is then distributed to each job incumbent for completion. Job Surveys sent for review shall include instructions outlined on Attachment TMP 3.1b, Job Task Survey. Upon completion of the review, the Job Survey is returned to the Training Support Supervisor.
! 4.1.4 Task Deselection Tasks are deselected by a Task Deselection Committee.
The committee shall be composed of a minimum of two appropriate Subject Matter Experts and one member of the Training staff. This committee reviews the task list to provide a consensus of the appropriateness of the tasks listed and to document the reason for deselection of tasks. Documentation of selection /deselection should include the numbering (W) system of Attachment TMP 3.lc, TASK ANALYSIS DESELECTION, Tasks may be selected at this time based on a case by case review of deselected tasks. Results of this committee are filed in accordance with TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. Selected tasks from the Task Deselection Committee are used in the Task Analysis process. l l 4.1.5 Task Training Selection l Tasks are selected by a Task Selection Committee ! utilizing criteria in Attachment TMP 3.le. G 4.2 TASK ANALYSIS Task analysis is performed manually using the format of Attachment TMP 3.1d, TASK ANALYSIS DATA COLLECTION FORM, or by using a computer format. Task analysis consists of five distinct phases. The.five phases are listed below and are discussed in the subsequent sections:

The selected task list is assigned to appropriate task analysis personnel, which may consist of an instructor or a Subject Matter Expert (SME) who will . analyze each task and identify the task's elements. [.'3 The task elements include action steps, their V sequence, and other performance task data to include, appropriate conditions, references, equipment needed and standards of proper performance. 4.2.2 Technical Review i Following the completion of element development, a second instructor or SME shall review the element . I development process for each task analyzed. 4.2.3 Skills and Knowledges Subsequent to the technical review, the elements are j j further analyzed for the appropriate skills and knowledges necessary for the proper performance of l that task. The codes are defined as:

- b. Component: Information pertaining. to characteristics, function, construction or i operation of a specific piece of equipment.
- c. Academic: Information pertaining to facts,

definitions, principles, rules or theories. 4 The skills and knowledges are assembled into single listings according to their code. The lists are evaluated by the Subject Matter Experts to separate or eliminate entry level skills and knowledges. The Taxonomy coded elements are normally taught in the classroom as opposed to task performance. 4.2.5 Data Input The final phase of task analysis is that of data entry into the appropriate computer system. This phase is the responsibility of the Nuclear Training Documents Specialist. A hard copy of the initial TASK ANAL.YSIS, TMP 3.1d, or the computer format shall be retained in accordance with TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT.

Job Analysis:The basic method used to obtain a listing of tasks necessary to perform a clearly defined, specific job. Job analysis is the first step to obtain data required for a task analysis. Knowledge: Understanding of facts, principles or concepts. Output:The result of co.rieting a task. Paradigm:A graphic representation of task actions and cues using stimulus-response formats. Performance Based Training:A systematic program of instruction designed around the tasks and their related knowledges and skills required for competent job performance. The derived knowledges and skills for each task are organized into enabling and terminal learning objectives that are composed of related conditions, actions and job standards.

(.' 1 1.0 PURPOSE l This procedure describes the. methodology to design training l curriculum-from material obtained from Job and Task Analysis. l 2.0 APPLICABILITY This procedure applies to Nuclear Production Division training j programs that require performance based training either as a result of 'an Administrative decision or pursuant to any program accreditation commitments. 3.0 GENERAL . j I 3.1 The Training Support Supervisor is responsible for proper curriculum design in accordance with this procedure. 3.2 Modifications to training curriculum due to audits, l evaluations or program changes shall be made in accordance J with this procedure. 1 3.3 Documentation of the Curricium Design process will be managed and maintained in accordance with Procedure TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. 4.0 PROCEDURE 4.1 JOB PERFORMANCE MEASURES l k During job and task analysis, tasks are selected for j training which define the supporting elements, conditions ' and standards necessary for adequate on-the-job performance. 1 A Job Performance Measure (JPM) is developed for each task to document a plant employee's performance of a task by actual performance, simulation, or walkthroughs of the task. ] Job Performance Measures shall be developed in the format of Attachment TMP 3.2a, JPM FORMAT. The con'ditions of the task j performance, references, tools and equipment shall reflect ') the actual job to.the maximum extent possible in the JPM. Taxonomy 1 coded skills and knowledges, TMP 3.1, JOB AND TASK ANALYSIS, are normally taught as classroom or self-study. Classroom and Self-study evaluations are detailed in TMP 2.1, TRAINING l l PROGRAM IMPLEMENTATION. These Training .attings are l l established and the lesson plan numbers (if .pplicable) are l placed on the Training Setting Matrix during the design
! phase of the program. The Training Setting Matrix used l shall be in the form of Attachment TMP 3.2d.
4.3 LEARNING OBJECTIV2S DEVELOPMENT > Ouring the skills and knowledge analysis, all skills and knowledges required for each task element analyzed during i task analysis are listed and categorized. The Subject Matter ' Experts assigned to the curriculum design process develop a . learning objective statement from similar skills and knowledges. Guidelines for development of the Learning Objactives are included in Attachment TMP 3.2b, LEARNING OBJECTIVE DEVELOPMENT. Learning objectives developed shall state the actions trainees must demonstrate, the conditions

( performance upon completion of the training activity.The ' learning objective statements are grouped into areas based on systems or duty areas. Common learning objective statements are sequenced based on their relationship to each . other. L . Learning objectives for the Classroom instruction are . developed from the Taxonomy coded skills and knowledges produced in TMP 3.1, JOB AND TASK ANALYSIS. The learning objectives are listed in the format of the lesson plan, , TMP 3.3, CURRICULUM DEVELOPMENT. Upon completion of the I learning objective listing in the lesson plan, the objective l listing and the lesson plan cover sheet are sent to the i appropriate in plant Supervisor for review of the l l objectives. The in plant Supervisor's signature is not i l . required for subsequent revisions that do not affect the l l eubling learning objectives. 4.4 EXAMINATION CONSTRUCTION l l Examinations are developed from the learning objectives ; produced in the learning objectives development process. ' There should be at least one test item for each learning objective identified in the process and more if it is considered justified. plan outlines produced during the design phase are used to form a Training Procedure for each area of training. The I Training Procedures shall be developed using the procedure format of Administrative Procedure G-2, FSV PROCEDURE SYSTEMS. The procedure should establish the requirements for training as it relates to qualification of trainees, requaliff' cation of trainees, continuing training, sequence i of training, lesson plans, job performance measures, entry level criteria, waiver criteria and specific documentation of the training. Trainees may be pre-tested, prior to instruction, to ensure that they are prepared adequately for the training. The pre-test may be developed as an entry ) level examination or experience evaluation within the bounds i of the specific training procedure. Entry level examinations ) or experience evaluations shall not be used for hiring l evaluations. Each job position identified in Job and Task Analysis shall be identified in a Training Procedure. ,. 0$9tVlCG* PUBUC SERVICE COMPANY OF COLORADO i l 1.0 PURPOSE l *#*7.1*#* This procedure provides the method for development of the training curriculum from material obtained from the Curriculum Design process. 2.0 APPLICABILITY This procedure applies to Nuclear Production Division training programs that require performance based training either as a result of an Administrative decision or pursuant to any program accreditation commitments. 3.0 GENERAL . 3.1 During development, the training setting, the learning objectives, and the training plan are used to determine instructor and trainee activities and to develop supporting materials for the training program. Personnel qualified in the position for which training is being conducted should help determine training content and confirm its completeness. 3.2 Modi fications to training curriculum due to audits,

Development activities shall be followed in the sequence indicated but training materials and training methods development may be performed in parallel. The five major development areas are listed below and discussed in subsequent sections: ,

- e. Training Program Tryout 4.2 LEARNING ACTIVITY DEVELOPMENT
.q if Learning activities are the unique activities the Instructor and trainee participate in to accomplish a level of '~'
learning. The learning objectives developed in the design phase are compared to any training programs which exist to determine if the existing program addresses the learning objectives. If the existing program satisfies the needs of the objectives, the existing program shall be modified to the format of Attachment TMP 3.3c, Lesson Plan. If existing programs are inadequate or no program exists, learning activities shall be developed. The Instructor developing the activities shall use the format of Attachment TMP 3.3c, lesson Plan, to develop the lesson plan for the area of interest. The Instructor uses the objectives listed on the 1e.sson plan to classify the learning objectives as skill or knowledge related. Skill s relate to physical or manipulative activities and knowledges relate to recall, comprehension or application of information. The Instructor uses the classification to determine skill and knowledge development guidelines from l Attachment TMP 3.3a, Skill and Knowledge Development Guidelines. From the guidelines, the Instructor develops the learning activities for all the objectives listed in the lesson plan. 4.3 TRAINING' MATERIALS ; The Instructor assigned to the development of the lesson plan uses the activities and the lesson plan developed to select training materials and methods. When selecting training materials, the materials should support the learning objectives and should emphasize job-related information and situations. Sources of information may include current plant procedures, current training programs, technical journals, system abstracts and materials used in other utilities. Existing material snould be collected and reviewed with the goal of using materials with minimum 1 modification. The material should be consistent with the expected entry-level skills and knowledges of the trainees. When self study is used, the training materials should contain the information to be learned or referenced texts > should be readily available. 4.4 TRAINING METHODS Training methods are techniques or strategies employed to , enhance the learning process. The lesson plan and the learning activities are to be used in the development of the training methods. Training methods include le.ture, . r' - demonstrations, discussion, oral questioning, role playing ( and walk-throughs. Training methods shall be selected for the learning activities that are appropriate for the training setting. Lecture, discussion and oral questioning are considered appropriate for the classroom setting while role playing and walk-throughs are considered appropriate for On-The-Job training and Job Performance Measures. j l 4.5 INSTRUCTOR GUIDES l l 4.5.1 Academic Instructor Guides I l The Instructor Guide shall be used by the Instructor l l as the primary training tool to guide the learning i l process. The training material should be presented to a small group of trainees and the material evaluated using a trainee course evaluation sheet, Procedure TMP 4.1, TRAINING PROGRAM EVALUATION. The small group should consist of more than one trainee with entry-level skills and knowledges expected of future trainees. The first performance of a course or area of instruction may be , considered the program tryout. VRAIN NUCLEAR GENERATING STATION ' 3 g 34 SCTVICO* Pusuc SERVICE COMPANY OF COLORADO ' h I . - 'd -1.0 PURPOSE' l *#*7.1*#* This procedure provides the method for the evaluation of. training programs at the Fort St. Vrain Nuclear Generating Station, i 2.0 APPLICABILITY This procedure applies to FSV training programs and will be administered in accordance with applicable Training Procedures (TP). 3.0 GENERAL Evaluation is the determination of a training program's effectiveness in producing competent employees. Evaluation is used to monitor, verify and maintain program performance. The evaluation process identifies performance problems and new or changing training needs. The output of the evaluation process serves as feedback for the analysis, design, development and implementation processes so the training program can be modified and improved. Training program evaluation consists of feedback from employees and Supervisors, Training internal evaluations, review of Plant (% -) Operation and Maintenance Information, and employee requests. Evaluation monitoring is controlled by the Training Support Unit under the direction of the Training Support Supervisor. All documentation of the Training Evaluation process will be managed and maintained in accordance with Procedure TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. I l i C, )

4.1' INFORMATION MONITORING t 4.1.1 Feedback from Employee Trainees / Supervisors During or at the end of a course of instruction, the trainees should complete a Trainee Course Evaluation, Attachment TMP 4.la. This evaluation shall be sent to the Training Support Supervisor who will designate a person other than the course instructor to i summarize the evaluations. This summary will be forwarded to the course instructor who will make a recommendation to the Training Review Committee if 'a change / revision to the course is necessary. The Training Review Committee will review the instructor's recommendation and the Course Evaluation Summary and initiate a Training Development Request, if necessary, in accordance with paragraph 4.2 of this procedure.
- Approximately six (6) months after a course or unit of instruction is completed, a Post-Training Survey will be sent to trainees and their respective i supervisors. The Post-Training Survey is designed to I,) allow for specific opinions / comments relating to the C adequacy and relevance of course attended. The survey is not applicable for courses conducted as part of requalification or continuing training or courses less than five (5) days in length unless !
required by request. The Training Support Supervisor shall ensure that the appropriate Post-Training Survey Forms are prepared for a random sample of the tasks / trainees and their j respective supervisors. [ L' 4.1.2 Training Internal Evaluation l On.a semi-annual basis, the Supervisor shall evaluate the Instructor's performance while attending the course. The Instructor Observation Sheet, Attachment TMP 4.1d,-is used for_this evaluation. . The completed-l Instructor Observation Sheet should be discussed with the Instructor and deficiencies .noted on the sheet. The Supervisor's recommendations to correct the deficiencies also should be noted on the sheet. When the deficiencies have been corrected, the Supervisor I should . sign. that the deficiencies have been corrected. The Instructor Observation Sheet should be treated as a confidential record to be maintained i in the Instructor's training file in accordance with Procedure TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. 4.1.3 Operating Information Assessment Group (OIAG) Material selected for training .by the Operating Information Assessment Group is sent to the Superintendent of Training. The original selected items shall have a Training Development Request, Part 1,' completed for each item
.in the OIAG report by the Training Support Supervisor or his designee. Procedure changes may be grouped into areas, if appropriate, to prevent redundant evaluations. I I Information forwarded to the Training Support Supervisor ! shall be reviewed by a member of the Support Unit for j assignment to a training unit. A review is conducted using i the Training Development Request (TOR), Part 2, Attachment TMP 4.le. Upon completion of the review in the TOR, Part 2, , the reviewer shall determine the -appropriate disposition j action to be taken for the request. If no modifications to training programs are required, the ' Disapproved' block on the TOR should be checked and a brief explanation of disapproval reasons should. be entered in the ' Comments' section. The Feedback matrix, TOR, Part 5, should be updated and the TOR retained per TMP 1.2, TRAINING SECTION RECORDS MANAGEMENT. If a modification is required, the
' Approved' or ' Approved with modifications' block should be checked. An explanation of the modification is required to be entered in the Comment section of the TOR, Part 2. Upon approval of the request, the TOR should be assigned to an Instructor. Upon completion of Parts 1 and 2 of the TOR, a copy of the completed sections should be sent to the originator of the request to provide feedback to the employee, The Instructor or. an appropriate Subject Matter Expert (m') (SME), should analyze the information with respect to its incorporation in the training program. The SME may be either an instructor in the area of concern or a plant employee in the area of concern. When the Instructor has a proposed modification developed, he should have the modification reviewed by the appropriate Training j Supervisor. The training review and proposed modification i is documented on Part 3 of the TOR.
Upon approval of the modification, the TOR shall be placed in the file for the lesson plan (s) affected by the TOR in accordance with Procedure TMP 1.3, TRAINING SECTION REFERENCE LIBRARY. This allows groups of TOR's to be dispositioned, rather than on an individual basis, to prevent excessive modification time. 4.3 TRAINING PROGRAM MODIFICATION Before a course is taught, an Instructor shall make changes to the specific program incorporating the TOR's placed in the Lesson Plan file. Modifications which alter an identifed task or create a new task shall be completed in accordance with Procedure TMP 3.1, JOB AND TASK ANALYSIS. When the lesson plans are revised, the lesson plan number, the Instr,uctor making the revision, and the completed lesson A plan revision number shall be entered in the TOR, Part 4. Part 5, .by the Nuclear Training Documents Clerk. The feedback matrix format is shown in Attachment TMP 4.1g, l Feedback Matrix. 4.4 CONTRACTOR TRAINING EVALUATION Contractor non plant specific training materials shall be evaluated prior to their implementation using a Contractor Training Evaluation sheet, Attachment TMP 4.lf. The completed evaluation sheet is forwarded to the appropr'f ate Training Supervisor for review and approval. Development of plant specific training materials by contractors shall be in accordance with TMP 3.3, Curriculum Development.
