Enclosed are.two Reports of 011, Salt,or Polluting Material Losses, Form R4616, applicable to the recent overfilling of the neutralizer tank at our (

Palisades Plant near Covert, Michigan. These overflows were discovered and reported to the Plainwell District MDNR on May 13 and 15, 1986.

This notification shall be made promptly by telephone or telegraph, giving briefly tt'a parv,culars, and b'/ mail, giving a detailed account of events and conditions.

As the result of a previously leakinz valve which allowed an unknown a-cunt of vster *n enter the neutralize &" tank (diminishing its holding capacity) the tank overfleved to the ground during an extended regeneration cycle.

_The overflow of desineralizer regenerants soaked into sand under terk. There was no significant environmental harm.

Tank level instrument was out-of-service for calibration. The level cauce and a hich level alarm were reinsts11ed on 5-16-86. The leaking valve was scheduled for maintenance repair.

Rinse valve was inadvertent 1'f left open overfilling the tank. There is a leak in the overflow line resulting in the 16ss of material to the ground.

The overflow water was absorbed in the soil under the tank.

The soil under the tank vas testLMr pH on 5/15/86. One sample point ceasured a pH of L.S.

All other samples ranged in W Ce from 5.0 to 7.0. Based on these results, no further clean up is planned at this e h;eratorsresponsibleforvalveoperationvillbeinstructed on the importance of valve closure to prevent similar spills in the future.
