e File No P10.2.1 Incident No: PAL-86-05-NC125 NPDES NOTIFICATION Apparent Difference From Monthly Average Limitation Plant: Palisades Permit No: MI 0001457 Outfall
Description:
Turbine Sump Oil Outfall Code No: 800218 Separator (001/00F)
Parameter: Oil and Grease Parameter Code No: 00556 1.
Date of Incident: May 31, 1986 2.
Identification of specific effluent limitation or monitoring requirement affected:
Monthly average limit of 15 mg/L oil and grease 3.
Description of Incident:
The May monthly average oil and grease concentration for Tank T-41, turbine sump oil separator was 19.a ag/L.
4.
Apparent Cause:
There were two incidents earlier in May when excessive oil and grease were found in T-41.
On May 14 the oil and grease was 61.5 mg/L (Report dated May 21, 1986). On May 19 the oil and grease was 33.0 mg/L (Report dated May 28, 1986). As previously explained, the May 14 event resulted from a temporary bypass of the oil / water separator and the May 19 event resulted from an apparent failure of the separator.
5.
If not immediately corrected, the anticipated time the condition is expected to continue:
N/A 6.
Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
As previously explained in the May 28, 1986 report, the oil / water separator M-18 was cleaned by A&B Industrial Co. on 5/24/86. Accumulated oil and grease were removed from the separator components to improve its efficiency.
As previously explained in the May 21, 1986 report the oil / water separator was returned to service and arrangements made to better coordinate bypassing of the system for maintenance or repairs.
7.
Additional Comments:
A visual check of the mixing basin discharge during both events showed no visible oil in the discharge or lake.
