The purpose of this letter is to apply for renewal of the SNM-ll56 license granted September 30, 1969.
The license expires September 30, 1974.
The license and amendments accurately describe conditions of use of scaled sources of plutonium in our facilities except as noted below:
Amendment 1:
We no longer have any. radiation source or combination of sources such as to cause a surface radiation level of 500 mrem /hr.
A maximum surface radiation level of 100 mrem /hr is adequate.
Amendment 6 is no longer applicable.
Amendment 9 contained a list of the company officers and a description of the Radiation Safety Committee.
The list of officers should be amended to read:

Mr. James R. Mason September 5, 1974 In order to further improve the physical control, accountability and traceability of plutonium sources during the manufacturing cycle, Medtronic wishes to add another amendment to SNM-ll56 that would allow the receipt and testing of the GIPSIE batteries and the Finished-Goods storage of the completed Model 9000 pulse generators in our Rice Creek facility at 6970 Old Central in Fridley, Minnesota.
The entire facility has controlled entry and is under tuenty-four hour surveillance by security guards.
The Receiving Area and Finished Gcods Area are controlled access areas within the controlled facility.
Storage areas where GIPSIE batteries and/or Model 9000-pulse generators would be left over night would be locked and properly labeled.
A floor plan of the Finished Goods and Receiving Areas is enclosed with this letter.
