Subject: Inspection Report Nos. 50-412/86-25, 50-412/86-27, and 50-412/86-28

This refers to three routine safety inspections conducted by Mr. Henri F.

vanKessel of this office on September 2-5, 1986, September 15-19, 1986, and

September 30 - October 3,1986 at the Beaver Valley Power Station, Unit No. 2,

Shippingport, Pennsylvania, of activities authorized by NRC License No.

CPPR-105 and to the discussions of our findings held by Mr. vanKessel with

senior members of your staff at the conclusion of the inspections. The

violations and the observations noted below were discussed with Mr. Swiderski

and Mr. Noonan of your staff on November 7,1986, by Messrs Bettenhausen and

Florek.

Areas examined during these inspections are described in the NRC Region I

Inspection Reports which are enclosed with this letter. Within these areas,

the inspection consisted of selective examinations of procedures and represen-

tative records, interviews with personnel, and observations by the inspector.

Based on the results of inspection 50-412/86-28, it appears that certain of

your activities were not conducted in full compliance with NRC requirements, as

set forth in the Notice of Violation, enclosed herewith as Appendix A. These

violations have been categorized by severity level in accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR

Part 2, Appendix C (Enforcement Policy). You are required to respond to this

letter and in preparing your response, you should follow the instructions in

Appendix A.

These three inspection reports, which include the observations and findings of

our inspector as well as your own QA organization, identify three areas where

we are concerned on the adequacy of the implementation of the preoperational

test program. These include adequacy of the implementation and knowledge of

both test and operations personnel in the drawing and document control system

including changes, adequacy of the implementation and knowledge of both test

and operations personnel in the administrative controls of the preoperational

test program and the fragmentation of system level preoperational tests into

subsystem or component level tests. Consequently, in your reply, you should

particularly describe those actions taken or planned to improve the implemen-

tation of the preoperational test program.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
