ENCLOSURE 1 NOTICE OF VIOLATION Duke Power Company Docket No. 50-413 Catawba Unit 1 License No. NPF-35 During the Nuclear Regulatory Commission (NRC) inspection conducted on September 26 - October 25, 1986, a violation of NRC requirements was identified.
The violation involved an inadequate procedure which resulted in an uncontrolled draining of the Refueling Water Storage Tank and a spill of contaminated water.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Technical Specification 6.8.1 requires that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A to Regulatory Guide 1.33, Revision 2.
Contrary to the above, the licensee failed to maintain an adequate procedure for conducting the Residual Heat Removal Pump 1B Performance Test while in mode 6.
PT/1/A/4200/10B was inadequate in that its prerequisite steps did not require isolating the residual heat removal pump suction from ' the reactor prior to lining up the Refueling Water Storage Tank to the pump's suction.
As a result of this, on October 6,
1986 while performing PT/1/A/4200/10B prerequisites, approximately 12,000 gallons of water uncontrollably drained from the Refueling Water Storage Tank into the reactor vessel. Contaminated water eventually spilled through the vessel flange / head gap, out the open Upper Head Injection ports and into the containment.
This is a Security IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Duke Power Company is hereby required to submit to this Office within thirty (30) days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further viola-tions, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
