SUBJECT: NOTICE OF VIOLATION

] (INSPECTION REPORT NOS. 50-269/86-33, 50-270/86-33 AND 50-287/86-33)

! This refers to the Nuclear Regulatory Commission (NRC) inspection conducted on. L

! October 15 - November 10, 1986. The inspection included a review of activities- '

( authorized for your Oconee facility. At the conclusion of the inspection, the

-

findings were discussed with those members of your staff identified in'the.

previously forwarded inspection report. The report documenting-this inspection '

was sent to you with a letter dated December 1, 1986. NRC concerns relative to

i the inspection findings were discussed by Dr. J. N. Grace Regional Administrator,

i NRC, Region II, with you and members of your staff in an Enforcement Conference '

held on December 22, 1986. -

! The inspection findings indicate that certain. activities appeared to violate NRC

1 requirements. The violation, references to pertinent requirements, and elements

to be included in your response are described in the enclosed Notice of Violation.

The violation described in the enclosed Notice of Violation involved a failure

to satisfy a Technical Specification Limiting Condition for_0peration which .

'

l required the Emergency Condenser Cooling Water _(ECCW) system to be maintained in ,

an operable condition. The violation resulted from inadequate . surveillance .

j testing to detect degradation of the ECCW system. As discovered during a test 1

i conducted on October 1,1986, because of air inleakage, the ECCW system was not .

j able to perform its required function of providing sustained siphon flow through

the main condenser during a station blackout with the level of Lake Keowee nine 'i

feet below full pond. Although the system had been tested _according to the

procedure and at the required frequency, the test procedure:was inadequate to

reveal the system deficiencies.

.The ECCW system would be inoperable under the' conditions of the loss of all ~

off-site and on-site power (station blackout) coincident with a low level in '

Lake Keowee. Because under other conditions, suchlas a loss of just off-site

power, the ECCW system remains operable, and the fact that sufficient water i

supplies exist to provide decay heat removal even without the operation of the '

j ECCW system, this violation is considered to be of low safety significance.

In accordance with the " General Statement of. Policy and Procedure for NRC

Enforcement Action," 10 CFR- Part 2 Appendix C (1986),' the violation described

[ in the enclosed Notice has been categorized as a Severity Level IV violation.~.

Although the violation was identified by the licensee with the resident

inspectors present, the discovery of the degradation of.the ECCW system was  ;

considered not to have been a part of the test as originally planned and 1

previously performed and was discovered fortuitously because the test being

conducted lasted longer than anticipated. Previous testing had not identified

the ECCW system degradation. Therefore, this violation is being cited.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosure are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980 PL 96-511.
