1. PURPOSE The purpose of this procedure is to outline the system by which the Millstone Licensed Operator Requalification Programs will be implemented. This also provides general methods of obtaining performance - based data for determining a particular implementation scheme.
2. APPLICABILITY This procedure applies to all licensed personnel with " active license status" (Ref. 3.1), and to those persons responsible for the training of these licensed personnel.

4. OEFINITIONS

4.1 Evaluation

A measurement of a person's performance ability in specific conditions or areas of knowledge.

4.2' Examination: An evaluation of one of the following forms:

4.2.1 Written Exam: Both the questions / problems and the examinee's response are performed by writing; 4.2.2 Oral Exam: Questions / problems are either of written or verbal form; with the examinee's response given verbally in the presence of the examiner.

4.2.3 Demonstration Exam: Actions or tasks performed by the examinee under actual or simulated conditions, under the observation of the examiner.

4.3 Quiz: A questioning technique of similar form to a written (or oral) examination; however it is developed and administered as ,

a training aid, not as an evaluation.

4.4 Lecture

A verbal training session which may consist of any of the following:

5.5 Assistant Training Supervisor Responsible for recommending topics, methods, and schedule of training for this implementation of this procedure; for the review and approval of all lesson plans for licensed operator training; for the preparation, administration and grading of all examinations given to license holders; and for providing an annual simulator training program for all license holders.

d-k 6. INSTRUCTIONS 6.1 Discussion.

This program is to be a continuous, on going program in terms -

of training and evaluation. It-is not intended that there be an obvious beginning.or end. Frequent evaluations' results

% will be the basis for determining immediate and long-term trainee needs, which in turn will determine inclusion / exclusion of subject matter in the programs' existing schedule.

3 However, training will be scheduled such that all license i holders receive the required training outlined in Ref. 3.1 and 3.2, at a frequency not to exceed 2 years.

6.2 Determination of Program content

The following methods and sources of input will be used, at a minimum, to assist in the determination of program content:

k 6.2.1.3 Simulator performance evaluations, 6.2.1.4 Periodic examinations administered during training sessions, 6.2.1.5 Operations and Shift Supervisor's evaluations.

2. Each license holder reviews the contents of all abnormal and emergency procedures on a regularly scheduled basis. (Ref. 3.1).

General weakness or insufficiencies noted by the above methods will be factored into existing program w schedules such that these insufficiencies are corrected as soon as practical.

Individual weakness noted, which are not common to a significent number of persons, will be corrected on a case basis by the Performance Review process.

Each of the methods listed in 6.2.1 through 6.2.3 will be described in further detail in subsequent sections of this procedure.

6.3 Implementation of Training Program NOTE: It is recognized that training may be achieved by a number of methods, such as self-study, videotapes, etc; however, a requalification program based solely

( upon the use of these methods is not an acceptable substitute for a lecture series. (Ref. 3.1).

6.3.1 Based upon the needs, requirements, and inputs established by the methods listed in 6.2, the Assistant Training Supervisor will maintain an organized program schedule, which shall include:

6.3.1.1 Normal lecture series.

6.3.1.2 Simulator training sessions.

6.3.1.3 Regular reviews of all abnormal and emergency procedures.

6.3.1.4 Frequent points of evaluation.

6.3.2 This training will be scheduled to accommodate plant outages, vacation periods, and shift staffing requirements.

6.3.3 Document Acknowledgement 6.3.3.1 A copy of changes or revisions to plant

  • design, procedures, or facility license, k which are determined by the Operations

/3 U Supervisor to be significant, will be placed in a notebook for review. This copy will be accompanied by a Document Acknowlegdement Sheet.

6.3.3.2 Each license holder will review the contents of this notebook, and upon completion of this review will acknowledge, by signature and date, his understanding of the change.

6.3.3.2.1 A file of completed Document Acknowledgement Sheets will be maintained as a record of program compliance.

6.3.4 Changes that require immediate operator notification, as in the case of significant interim procedure changes, will be covered as 6.3.4.1 Night orders to the Shift Supervisor, or r7 Q 6.3'.4.2 During Operations Department Meetings followed by a document acknowledgement under this program.

6.3.5 In addition to these methods of change review, the program lecture series will include discussions of changes of significance, as determined by the Operations Supervisor and Assistant Training Supervisor.

6.3.6 All licensed personnel will attend and participate in the simulator program at least annually.

6.3.6.1 Programs developed for simulator training shall consist of control manipulations and discussions to emphasize those evolutions, transients, and casualties not normally experienced durin'g routine operation.

6.3.6.2 This program should also include discussions of nuclear plant operating transients that have occurred at this, and k

( facilities, and actions that should be taken to limit the severity of damage to equipment under accident conditions.

6.3.6.3 The program content shall also include, at a minimum, those manipulations and evolutions delineated in Ref. 3.2.

6.3.7 On-the-job Training 6.3.7.1 Watchstanding 6.3.7.1.1 All licensed PE0's should stand at least one Control Operator (RO) watch per shift rotation. (This requirement does not apply to Relief and Training shifts).

6.3.7.2 Control manipulations during plant evolutions and transients shall be documented on SF-807 or SF-808 and forwarded immediately to the training

.( department for retention.

6.4 Operator Evaluations and Feedback 6.4.1 Training needs that may be indicated during control manipulations, plant evolutions, or transients should l be noted by the operator, and relayed directly to the Assistant Training Supervisor, to be factored into the training program schedule.

6.4.2 Examinations will be administered to determine training effectiveness. Scope and content of these exams shall be, at a minimum

6.4.2.1 Comprehensive review of previous lectures, and 6.4.2.2. Review of presently completed lectures, and 6.4.2.3 Assigned abnormal and emergency procedures.

6.4.3 Evaluations of actions taken by operators during actual or simulated abnormal and emergency conditions shall be included in this program.

k These evaluations shall include:

6.4.3.1 Response to control room alarms /malfuncticci, 4.4.3.2 Use of normal, abnormal, and emergency procedures,

-s 6.4.3.3 Knowledge of equipment and control functions, 6.4.3.4 Ability to predict instrument response.

These evaluations shall be performed by the operator's supervisor, the Training Department staff, or an evaluator designated by the Training Department. Results shall be recorded on SF-804, which will be retained as determined by the cognizant Assistant Training Supervisor.

6.4.4 Annual Requalification Written Exam 6.4.4.1 This exam will be comprehensive in nature, and similar in both scope and content to an k NRC licensing examination.

6.4.4.2 This exam shall be administered on a'n annual basis, and should be scheduled with -

the same considerations given to all program scheduling (see 6.3.2). This exam may be given in its entirety or in two segments, each covering approximately one half of the. entire exam scope.

6.4.4.3 Examinees' responses in this examination will be evaluated to identify all areas of weakness, common to a significant numbers of. license holders. These areas, if any, will be included in the normal program schedule such that, they are sufficiently covered prior to the next annual written i

exam.

6.4.4.4 Individual weakness, which are not common k to a significant number- of examinees, shall a

u be corrected in the manner specified by the llf Performance Review process (discussed in subsequent sectie.1s of this procedure).

6.4.4.5 Since this exam will necessarily be administered in more than one exam session, different exams will be written for each session.

6.4.4.6 Newly licensed personnel will be exempted from the annual written exam if the NRC exam was within six months of the NNEco examination.

6.4.5 Annual Oral Examination 6.4.5.1 This exam will be comprehensive in nature and shall evaluate the operator's knowledge of the following:

(a) Theoretical aspects of nuclear power plant operations, ll[ (b) In plant systems and components, (c) Control room instrumentation and.

control functions, (d) Plant operations during abnormal and emergency conditions.

6.4.5.2 This exam will be administered by the training staff, and scheduled in such a manner that all license holders will be examined annually.

6.4.5.3 This exam need not coincide with the administration of the Annual Requalification Written Examination.

6.4.6 Examinations administered by the Training Department under this program shall be developed under the supervision of a licensed instructor, and they shall

  • be approved by the Assistant Training Supervisor prior to administration.

[s 6.4.7 - All written examinations shall be " closed-book," and

{ should be proctored continuously by a member of the training staff.

6.4.8 The author of a written exam (or exam sections) shall be exempted from the examination process for obvious reasons; however, this duty should be assumed by rotation of staff, such that any one person is not ccatinuously being excluded from this evaluation process.

6.4.9 Operator evaluations shall be used for the purpose of determining training effectiveness and training needs for future programs. In general, this should follow the guidelines previously outlined in 6.4.3.

6.4.10 For the benefit of each operator toward~

, self-improvement, all operator evaluations should be critiqued with the operator.

6.5 Performance Review 6.5.1 Discussion

( This process is designed to review the performance of the trainee in areas of indicated insufficient knowledge or skill, as determined by unsatisfactory performance on an operator evaluation.

This review will be conducted on a case basis and documented on SF-805. The purpose of the review is to designate a course of action for an individual, such that his knowledge in the specified area is returned to a satisfactory level.

6.5.2 A performance review will be conducted when any of the following conditions occur:

6.5.2.1 < 80% overall average on an annual written exam, or

- 6.5.2.2 < 70% in any section of an annual written exam, or

. 6.5.2.3 Less than " minimum passing score" on any written exam, other than the annual exam.

(This will normally be 70%; however, a higher standard may be assigned by the Assistant Training Supervisor) 6.5.2.4 " Unsatisfactory" result on a simulator evaluation, operational evaluation, or oral examination, or 6.5.2.5 Failure to attend required training sessions under this program, or 6.5.2.6 Upgrading license status from " inactive" to

" active" status (Ref. 3.1).

6.5.3 This review will be initiated by the Assistant Training Supervisor and noted on SF-805.

6.5.3.1 The Assistant Training Supervisor or Senior Instructor will meet with the individual,

- and by oral examination determine the extent of insufficient knowledge.

6.5.3.1.1 If this part of the review k - reveals that the individual's knowledge is satisfactory, and no further action is necessary, the Performance Review form (SF-805) will be routed for management review.

6.5.3.1.2 If this part of the review reveals that the individual's knowledge is unsatisfactory, the Assistant Training Supervisor will meet with the Unit Superintendent and the Operations Supervisor to determine further action.

6.5.4 The disposition of this second part of the review process will be to prescribe remedial training for that individual, such that his knowledge will be h upgraded to satisfactory standards.
