NUCLEAR TRAINING MANUAL I
NTM-1.11 EXAMINATION CONTROLS 1.0 PURPOSE The purpose of this procedure is to provide requirements for the security, administration and storage of examinations.
2.0 APPLICABILITY This procedure applies to the Nuclear Engineering and Operations (NEO) Group, including the Northeast Nuclear Energy Company (NNECo) and the Connecticut Yankee Atomic Power Company (CYAPCo), and other Northeast Utilities (NU) organizations performing safety related functions for NU nuclear generating stations.

5.2 NU Managers / Supervisors 5.2.1 Responsible for assigning personnel to function as examiners and proctors.
5.2.2 Responsible for investigating instances of-examination compromise or student cheating and notifying appropriate Directors / Station Superintendents.
5.2.3 Responsible for ensuring security of examination materials.
5.3 NU Personnel Responsible for acting as examiners / proctors.
6.0 INSTRUCTIONS.
6.1 Categories of Examinations Every examination shall be placed into one of the five categories listed below.
The cognizant supervisor shall assign the examination category.
The division into categories allows for different levels of security and administrative control.
j 6.1.1 Category I - Written examinations administered for NRC required annual requalification or NRC mock examinations.
This category applies to nuclear operator training only.
6.1.2 Category II - Written examinations whose grade forms all or part of the basis for certifying the satisfactory completion of required training.
6.1.3 Category III - Ouizzes, the results of which are not generally used in certifying the satisfactory completion of required training.
6.1.4 Category IV - Oral examinations conducted by an examiner or a board consisting of two or more examiners.
6.1.5 Category V - Skills / operational examinations used to evaluate the students ability to perform actual or simulated tasks.

6.2 Security of Category I and II. Examination Materials

The cognizant supervisor shall ensure access to Category I and II. examination materials is restricted to individuals with the need'to know.
The following practices shall be utilized to provide the proper level of security.
6.2.1 Examination materials shall be maintained in lockable storage when not under the direct control.

i 6.2.2 Security of examination data.in word processing or data processing systems shall include lockable storage of discs, tapes, and printouts.
Examination material in computer memories shall have secure password access.
6.2.3 Access of personnel to areas where examination materials are used or stored shall be restricted to individuals with the need to know.
6.2.4 Techniques shall be utilized to produce varied examinations.
6.2.4.1 For cyclic programs, such as rotating shift training, a different examination shall be used each week.
6.2.4.2 For non-cyclic, repetitive programs, such as General Employee Training, two or more different examinations shall be administered to each session.
6.2.5 Examination materials shall be hand carried between locations. If it becomes necessary to send examination materials, the recipient shall be specified by name, and the mailing container sealed and marked " Personal and Confidential".
Surplus examination materials shall be shredded or 6.2.6 destroyed such that the materials are unreadable.
Security of Category III Examination Materials 6.3 6.3.1 Examiners and proctors shall follow the requirements in 6.2.1 to 6.2.3 to safeguard Category III examination material from compromise.

6.3.2 Category III examinations may be reused for subsequent classes.
6.4 Security of Category IV Examination Materials 6.4.1 Category IV examinations are considered to be secure if the questions to be asked are not revealed to the student prior to the scheduled examination. In addition, requirements in 6.2.1 through 6.2.3, 6.2.5 and 6.2.6 apply.
6.4.2 If oral examinations are given to a group of students, the examiner (s) should vary the questions asked.
6.5 Security of Category V Examination Materials Category V examinations may evaluate a student's ability to troubleshoot a piece of equipment or diagnose certain events.
To ensure that the students are not pre-alerted or allowed to view the equipment prior to the examination, access to the materials used for the examination shall be restricted by employing the following practices:
6.5.1 Examination materials / equipment shall be maintained in lockable storage or locked rooms when not under the direct control of an examiner.
6.5.2 For written examination materials, requirements in 6.2.1 through 6.2.6 shall apply.
6.5.3 For simulator operational examinations, adequate safeguards shall be taken to ensure successive groups of students do not view the examination (e.g., secure the TV monitoring system, close the curtains, etc.).
6.6 Conduct of Written Examinations 6.6.1 The supervisor responsible for the training program shall assign an examiner to coordinate examination activities.
The examiner shall be responsible for:

- Delivery of the examination, answer key, examination grades, and the completed examinations to the supervisor responsible for the training program.
6.6.2 The examiner shall ensure each written examination has an examination cover sheet (Reference 3.1).
6.6.3 Examiners / proctors shall perform the following activities..1 is provided for reference purposes.
6.6.3.1 Schedule a room for conducting the examination.
6.6.3.2 Arrange the seating so that an individual table / desk is provided for each student.
Where class size or examination room size prohibits this seating arrangement, the requirements of 6.2.4.2 shall be applied.
6.6.3.3 Check the examination room and remove unauthorized information and material e.g.,
notes on blackboard, charts on wall, etc.
6.6.3.4 Distribute examinations after all students are seated.
6.6.3.5 Brief students on the amount of time allowed for the examination, point values, and other pertinent information.
6.6.3.6 Read the cover sheet instructions to the students prior to commencing the examination.
Ascertain that the students fully understand the intent of these rules.
6.6.3.7 Continuously monitor students taking the examination.
Proctors shall be present in the examination room at all times.
Proctors may perform concurrent activities that do not distract the test takers or interfere with the proctoring.

6.6.3.8 Provide student assistance when requested.
The proctor / examiner should go to the student when providing assistance.
The proctor / examiner shall obtain additional assistance if required.
6.6.3.9 Collect examinations and answers, including scratch paper, upon completion of the examination or when the allotted time has expired.
6.6.3.10 Ensure students have:
Signed the examination cover sheet.

Signed or initialed all examination pages.
Completed all questions.
Conduct of Oral and Skills / Operational Examinations c
6.7 6.7.1 Instructions for conducting oral and skills /
operational examinations shall be stated in writing, approved by the cognizant supervisor, and explained to the student (s) prior to the start of the examination.
6.7.2 Examiners shall administer oral examinations in accordance with Oral Examination Worksheets (Reference 3.1) and attached examination instructions.
6.7.3 Examiners shall administer skills / operational examinations as follows:
6.7.3.1 Simulator examinations in accordance with Reference 3.2.
6.7.3.2 On-The-Job Training Evaluations in accordance with Reference 3.3.
6.7.3.3 Other examinations in accordance with examination instructions contained in the skills / operational examination.

i 6.7.4 Examiners shall assign an overall grade of s
satisfactory or unsatisfactory to oral and skills /
operational examinations.
6.8 Grading and Retention of Examination Results 6.8.1 Examiners shall grade written examinations in accordance with the following practices:
6.8.1.1 Examination security during the grading process shall be maintained at all times.
6.8.1.2 Category I and II examinations should be graded within three (3) working days after the examination is taken.
Category III examinations should be graded within one (1) working day.
6.8.1.3 Examinations shall be graded using approved Answer Keys.
6.8.1.4 Examinations shall be graded in a manner that provides attention to detecting suspicious parallelisms among responses r
of different students.
Instances of suspicious parallelism shall be handled in accordance with 6.9.
6.8.1.5 Responses requiring examiner evaluation, e.g., essay questions, etc., shall be graded as follows:
Evaluate the responses to one question or section at a time for all of the examinations.
This will promote consistent evaluation.
If several examiners are evaluating the examinations, the examinations should be divided among the examiners, such that one examiner evaluates all of the students' responses to one section or question.
6.8.2 Students shall be notified of examination results.

6.8.3 Graded examinations should be reviewed with t he student (s).
Examinations shall be collected following the review.
6.8.4 Examination Materials shall be maintained in accordance with Reference 3.4.
6.9 Misconduct or Cheating Willful failure to comply with verbal or written 6.9.1 instructions on examination cover sheets or instructions of conduct for oral or Skills / Operational examinations shall constitute misconduct or cheating.
6.9.2 Written examinations, where instances of suspicious parallelism have been detected, shall be reported to the supervisor responsible for the training program and thoroughly investigated.
6.9.3 Proctors / examiners who compromise the validity of examinations by changing student answers or providing unethical technical assistance to students during examinations shall be subject to disciplinary action which could lead to suspension or termination.
6.9.4 Proctors / examiners shall take action to stop suspected or observed misconduct, up to and including collecting the examinations.
The proctor / examiner shall make an oral report to the supervisor responsible for the training immediately following the examination.
The proctor / examiner shall follow up with a written report.
6.9.5 The supervisor responsible for the training shall report by memo, via line management all cases of misconduct to the appropriate Director / Station Superintendent.
A copy of the report prepared by the proctor / examiner shall be attached.

1.0 PURPOSE The purpose of this procedure is to provide requirements for establishing, processing, maintaining and dispositioning Nuclear Training Records.
2.0 APPLICABILITY This procedure applies to the Nuclear Engineering and Operations (NEO) Group, including the Northeast Nuclear Energy Company (NNECO) and the Connecticut Yankee Atomic Power Company, and other Northeast Utilities (NU) organizations performing safety related functions for NU nuclear generating stations.

'The Supervisor, Records and Services shall transmit these working documents to GFR at intervals specified as follows:
a6.2.4.1 All documents listed in Attachment 8.1, Section A,'shall be transmitted to GFR within 6 months of completion of the Trainee Performance Guides.. Subsequent revisions-of these-working documents shall be likewise transmitted to GFR

6.2.4.2 'Upon_ notification by the Director, Nuclear Training, for programs-whose TPCC has been dissolved, training-development working documentation shall

6.'2.4.3 A record of receipt by GFR shall be permanently retained by the Supervisor, Records and Services.
6.3 Documents associated with Training Implementation Records are listed in Attachment 8.1, Section B.
t 6.4 Documentation of training program attendance shall include as a minimum:
6.4.1 Training Program or' Course title: (for example, Rad Worker Training).
6.4.2 Date of the training session (or inclusive dates).
4 6.4.3 Name of the-instructor (s) conducting the training.
6.4.4 Listing of all students by name.
6.5 Retention of training implementation working documents shall be such that retrieval is facilitated, and that personnel access is controlled in accordance with this procedure.
6.5.1 For those programs having definite beginning and endpoint (such as Replacement Operator, New Employee Training), the cognizant Nuclear Training Supervisor shall forward the completed working documentation package per 6.4 to the Supervisor, Records and Services within 30 days i
of program completion.

6.7 Student / Instructor training records, as listbdrin.2, shall be maintained in accordance with this procedure.
6.7.1 For those programs having a definite beginning and endpoint (such as Replacement Operator, New Employee Training), the cognizant Nuclear Training Supervisor shall forward all documentation listed in Attachment 8.2, Section A, of this procedure to the Supervisor, Records and Services within 30 days of program completion.
I The Supervisor, Records and Services shall transmit these working documents to GFR within 6 months of program completion.
6.7.2 For those programs which are continuous in nature (such as Licensed Operator Requalification), the cognizant Nuclear Training Supervisor shall forward all documentation listed in Attachment 8.2, Section A, of this procedure to the Supervisor, Records and Services within 30 days of document's completion.
The Supervisor, Records and Services shall transmit each working document to GFR within 6 months of its completion.
6.7.3 Certification, qualification, and indoctrination documents for $nstructers and SME's are listed in Attachment,#.2, Sect' ion B.
These working documents shall be forwarded to the Supervisor, Records and Services within 30 days of its completion.
The Supervisor, Records and Services shall transmit these working documents to GFR within 6 months of its completion, and retain a working copy.
6.8 Unless otherwise stated in this procedure, the Supervisor, Records and Services shall be the sole transmitter of all training documents to GPR, and shall permanently retain all records of transmittal to that facility. All transmittals shall be accomplished in l
accordance with Reference 3.5.
6.9 The Supervisor, Records and Services shall ensure retrievability is maintained for all working documents, and copies of working documents required by this procedure, while these documents are in the custody of the NTD.
