RCCAs at Wolf Creek Preliminary g has recently completed results show (1)eddy current testing Unexpected wear of all 53 which includes at least two RCCAs showing through clad wear and (2)(thinntrg) ,

Non uniform swelling or expansion of almost all RCCAs of at least 5 mills (The instrumentation used to conduct the testing was only calibrated to measure a I maximum swell of 5 mills). The thinning was primarily located at the 228 '

l position and attributed to guide tube wear. However, the swelling is I currently unexplained and may relate to the hafnium control material.  !

In order to minimize the impact to the current refueling outage, the licensee '

has chosen to replace all 53 RCCAs with identical, new assembites. Further justification for this action includes: i (1) New RCCAs are imediately available from Comanche Peak. Unit 28 t (2) Replacing the RCCAs will provide additional time for H to evaluate the findings and will not impact the corrent outaget and f (3) The old RCCAs will remain on-site thus being available for future l usependingtheyevaluation.

i Fuel Cycle Safety DOE Notification Of UF6 Contamination i The NRC was. informed by DOE that the Paducah Diffusion Plant has recently encountered problems with an arsenic contaminant in UF feed received for "

e enrichment from the Canadian Eldorado Plant. Theprobiemwasrevealed through the examination of a cylinder valve, removed because it was found

to be plugged during a cold pressure check prior to feeding the cylinder ,

contents to the cascade. The material plugging the valve contained some arsenic compounds, and the valve showed some corrosion on the inside of '

,~

the threaded section that screws into the cylinder. The cylinder had been

- filled by Eldorado and shipped to Paducah in 1985 where h was placed in '

storage until the recent processing.

In the UF, feed, the contaminant is probably in the form of arsenic i pentafluoride (AsF the scurce of the arsenic is believed to originate '

l from hydrofluoric $c);d i (HF) that Eldorado purchases for its processing of yellowcake to the UF g form. Because of its characteristics, AsFg remains in the gas phase in cy1Tnders after the UF, has solidified. Ir, this form, the j AsF, apparently removes the protective Y1uoride film that fcrms from the UF6 on the inner surface of the cylinder and valve. Repeated action of the AsF ,

! on this protective film allows tis UF 6 to slowly corrode the metal. Theva5ve .

l- DOE is now replacing cylinder valves on Eldorado cylinders before feeding i them to the cascade. DOE also is studying whether the arsente contaminant is  ;

transported through the cascade to the product. Because of the nature of the material and the process, DOE dces not believe this would occur. However, the notification was provided to the NRC in order that it might alert licensees j i

receiving enriched UF, from the DOE enrichment plants although the risk to

licenstes from the prBblem is considered to be small. The staff is notifying  !

these licensees and will keep them informed as it receives additional (

- information from DOE. The staff also is alerting licensees for the two UF6

' production facilities in the U.S. of the possible source of the arsenic l contamination. [

9 Standardization of Shipping and Storage Casks 01 November 2,1988, NMSS and DOE staff attended a U.S. Council on Energy

/wareness Nuclear Transportation Task Force Meeting on Standardization of

! hipping and Storage Casks for Spent Nuclear Fuel. The need for compatibility

.ith the back end of the fuel storage cycle was recognized. In the absence of 00E transportation fleet cask designs and a repository canister design, industry consensus was that the process of standardization and compatibility achievement could not and should not be forced beyond the evolution that natural forces were already causing.

Dual Purpose Shipping / Storage Casks On Noven.ber 4,1988, NHSS management met with the Director General and other "

officialsofEmpresaNacionaldeResiduosRadiactives,S.A.,(ENRESA).

ENRESA occupies a position analogous to the U.S. DOE for the back end of the nuclear fuel cycle in Spain. Spain is developing plans for a central storage facility end additional onsite storage capacity for reactors because of spent fuel storage shortfalls anticipated in the early 1990's. ENRESA is presently considering four bids for dual purpose transportation / storage casks. Two

designs submitted are from U.S. companies; Westinghouse and Nuclear Assurance Corporation (NAC).

It is expected that these designs for Westinghouse's 11C-10 and NAC's NAC S/T casks (for which there 4re previously reviewed topical reports under 10 CFR Part 72) will be submitted with modifications to be certificated by the NRC under 10 CFR Part 71. hMSS management saw no barrier to review these nodified designs under Part 72, as necessary, and Part 71. Dual purpose casks fit NRC s interests in compatibility for the back end of the fuel storage cycle with direct offsite shipment to !!RS or repository.

Materials Licensig Meeting with the University _ of Missouri (UM) Officials to Discuss their License Application for the Distribution of Neutron-Irradiated Topaz to Persons Exempt from Licensing Representatives from NHSS and Region III mc? with officials from the University of Missouri (UM), Columbia to discuss a letter dated October 28, 1988 sent to UM President C. Peter Magrath that outlined and explained the options available to the University of Missouri Research Reactor (HURR) regarding their neutron-irradiated topaz operations. On the basis of discussions at this meeting, the MURR is expected to submit 2 full and complete application by November 21, 19P8. The stoff is scheduled to make a licensing decision within 45 days af ter receipt of the application.

SAFEGUARDS  :

International NRC/D0E physical Protection In-Country Visits

~

Staff members from the Division of Safeguards and Transportation and from International Programs participated in joint NRC/ DOE /D00 physical protection visits to Switzerland, Belgium, and Greece from October 18 through November 2, 1988. For the first time a representetive from the Defense Nuclear Agency accompanied the team, which was led by the Department of Energy. The purpose of the visit was to exchange information with each country on the physical pro-o tection programs implemented at their nuclear facilities processing Category I nuclear materials. ,

Domestic I NRC/ DOE Comparability Rule f The Federal Register Notice for the rule was signed on November 4, 1988 and is expected to be published within the week.

NRC/ DOE Clearance Comparability During a joint NRC/ DOE briefing on their respective personnel clearance programs on November 8, 1988, DOE revealed p'ans for a major restructuring of its clearance levels. Among the features of interest to NRC are proposals to

place access to special nuclear material in the same category as access to nuclear weapons design information. For this category, DOE would require an initial background investigation covering 15 years of an applicant's history

4 rather than five years as is currently required and a Personnel Assurance

Program including psychological testing, random drug testing and annual financial records.

l TRANSPORTATION Heeting on West Valley Casks f

! On Novereber 3,1988, the Trarsportation Branch met with Transnuclear, Inc. (TN) and Sandia National Laboratories to discuss the TN-BRP and TN-REG casks for ,

shipment of spent fuel from West Valley, New York to Idaho. Sandia discussed the rescits of various scale model tests and analyses which were performed for l the 30-foot drop test. TN discussed the information they plan to submit in response to the previous staff request. TN expects to submit a revised application to the NRC in January 1989.

4 HIGH LEVEL WASTE MANAGEMENT DOE / Yucca Mountain Project Office (YMP0) Observation Audit From October 31 through November 4,1988, staff from the DHLWM participated as r observers on a DOE / Yucca Mountain Office audit of Holmes and Narver (H&N).

H8H is responsible for design of the underground support systems and the above ground facilities, involving field surveillance and inspection of facilities construction. Based on the audit, the DOE /YMPO team proposed that 13 Standard Deficiency Reports and 13 observations be issued against H&tl's Quality Assurance Program.

The major issues identified by the DOE /Yl1P0 team were: (1) arnual management assessments not performed; (2) H&N quality assurance personnel not involved in reviewing design inputs for quality standards; (3) audit reports did not contain statements as to the effectiveness of the QA program; (4) no procedures to cover quality assurance level I and 11 activities; and (5) several design drawings lacked evidence of not being checked by an independent reviewer.

Based on its observations, the DHLWM staff concluded an acceptable audit was performed by the DOE /YMF0 team. The DHLWM staff did identify several areas where DOE /YMPO needs improvements; however, these did not detract from the overall acceptability of the audit.

4 t OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending November 11, 1988 PREVENTING DAMAGE TO REACTOR CORES ICAP The International Code Assessment and Application Program (ICAP) was initiated in 1985 to measure the performance of NRC-developed themal hydraulic computer codes (TRAC-PWR, TRAC-BWR, RELAP, COBRA-TF) under loss-of-coolant accident and other transient conditions. This code assessment information is mostly generated by foreign participants in the ICAP. Based on the assessment infomation obtained from the ICAP as well as other programs such as 2D/3D, MIST, and ROSA IV, RES determined that there was a need to improve the TRAC-PWR (PF1/ MOD 1) and RELAP5/ MOD 2 codes. Therefore, RES modified the ICAP to establish an international consortium to latach code improvement activities which are jointly carried out by LANL, INEL, UK, Japan, FRG, and others.

These code improvement activities and other assessment activities were discussed on October 19-21, 1988 by representatives from U.S., UK, Japan, France, FRG, Belgium. Italy, Spain, Korea, Finland, Taiwin, and Switzerland.

Improved models for post-CHF heat transfer and interfacial shear are being i developed by the UK. Improved break-flow models are being developed by the '

European Joint Research Center at Ispra, and conduction heat transfer models are being developed by Japan. The FRG has provided funding for improved reflood models for RELAP3 at INEL.

Nuclear Safety Research Review Comittee Subcommittee Review of the Aging Program On November 3 and 4, 1988, the Aging Program Subcommittee of the NSRRC (Nuclear Safety Research Review Comittee) met with RES/DE staff members and contractors to review the Materials, Aging, and Containment Research Programs. The subcomittee was chaired by S. Bush with J. Hendrie and N. Todreas. NSRRC Chaiman, present.

The Pressure Vessel Integrity research program was introduced by C. Z. Serpan, Jr., Chief of the Materials Engineering Branch. Presentations were headed by a review of Non-Castructive Examination research being done at Br.ttelle-PNL, aimed at primary system integrity, followed by presentations from the Oak Ridge Heavy Secticn Steel Technology Program, including low upper shelf energy materials, pressurized thermal shock, irradiation effects, low temperature embrittlement and its impact on vessel supports and advanced inelastic fracture mechanic!;. Objectives, progress and regulatory application of the research were discussed.

The Nuclear Plant Aging Research Program was introduced by M. Vagins, Chief of the Electrical and Mechanical Engineering Branch. The introduction dealt with the need for and the objectives of the NPAR Program, as well as a detailed NOVEMBER 10, 1988 ENCLOSURE D

2 description of its functioning and the interrelationship of its various sub-sections. The introduction was followed by contractor presentations from INEL, SNL, ORNL and PNL. These presentations were detailed synopses of the work underway in the aging program dealinD with the development and use of risk ,

based algorithms for prioritization of research projects, cable aging and '

diagnostic development, and the aging assessment of pumps, valves, battery chargers, inverters, and emergency diesel generators. Also briefly covered was the description of the work in the development of the methodology to be used for residual life determination for LWR components.

The Containment Program was introduced by A. Murphy, Chief of Structural and ,

Seismic Engineering Branch, followed by detailed presentations led by W. von Riesemann, SNL, and discussion of the results to-date and of the plans l for the completion of the program, which is planned for FY 1991. Tne topics covered by the SNL presentations included: testing of the steel and reinforced concrete containment models, NRC participation in the CEGB test of a prestressed concrete model, testing of the electrical and mechanical penetrations, and testing of seal and gasket materials.

REGULATORY ANALYSIS Emergency Planning and Preparedness Requirements for Nuclear Power Plant Fuel Loading and Initial Low-Power Operations (10 CFR 50)

NRC has amended its regulations to establish more clearly what emergency pisnning and preparedness requirements are needed for fuel loading and low power operation of nuclear power plants. i The final rule was published in the Federal Register on September 23, 1988 (53 FR 36955).

Acceptance Criteria for Emergency Core Cooling Systems (ECCS) for Light Water Nuclear Power Reactors (10 CFR 50, App. K)

The NRC has amerded its regulations concerning acceptance criteria for '.

emergency core cooling systems (ECCS) to allow the use of realistic methuds to demonstrate that an ECCS would protect the nuclear reactor core during a loss-of-coolant accident. This action was proposed because research has shown ,

that calculations performed under the previous requirements greatly underestimated the ability of the ECCS to protect the core. This resulted in unnecessarily restricting the operation of some nuclear reactors and increased the cost of electricity generation. This rule will allow use of the best information currently available to demonstrate that the ECCS world protect the reactor core during a loss-of-coolant accident.

The final rule was published in the Federal Register on September 16, 1988 (53 FR 35996) and was effective as of October 17, 1988.

Items of Interest Office for Analysis and Evaluation of Operational Data Week Ending November 10, 1988 Diagnostic Evaluation and Incident Investigation On November 7,1988, the Femi Diagnostic Evaluation Team report was transmitted to the EDO for infomation and appropriate action. The team's report discusses the major licensee performance strengths and deficiencies and includes the team's findings, conclusions, and root causes. Recomendations for NRC staff actions resulting from the diagnostic evaluatior at Fermi were also forwarded to the EDO for appropriate action.

Incident Response On November 8, the IRB Branch Chief and staff members met with a NUMARC representative to discuss licensee effort required for participation in EROS.

Another draft of licensee required efforts will be forthcoming.

On November 8, IRB's Source Evaluation Panel for the new Task Order Agreement (T0A) for the Operations Center held its initial meeting. The T0A will be used for supporting Operations Center activities, including unanticipated response for emergencies.

On November 8, IRB staff attended a present6 tion by NUMARC on Severe Accident Management.

On November 9, the Deputy Director and IRB staff attended the quarterly reeting of the FRPCC. The meeting covered a variety of emergency planning and incident response subjects of interest to the twelve agencies represented. The major concern of the agencies related to extent of modifications which FEMA believes should be made to the FRERP as a result of the President's approval of the NSEC.

The agencies appeared to be quite unified in their position that FEMA's interpretation of the NSEC is incorrect and little change is needed to the FRERP.

Technical Training George Schneider and Bobby Eaton of the TTC PWR Technology Branch presented a Westinghouse Technology Course (R-104P) for the Illinois Department of Nuclear Safety (IDNS) in Springfield Il from November 7-10, 1988. The course was presented at Sangamon State University and was attended by approximately 25 engineers and scientists employed by the State of Illinois.

Two representatives of the Engineering Division of Brookhaven National Laboratory met with TTC management on November 9,1988 to explore the feasibility of conducting limited scope human factors research on control room annunciators with the assistance of the TTC staff. The meeting included demonstrations of the annunciation systems present on all three NRC controlled simulators (GE, Westinghou"., and B&W vendor designs) as well as informal discussions of generic annunciator system upgrades previously identified in NUREG/CR-3217.

ITEMS OF INTEREST OFFICE OF GOVERNMENTAL AND PUBLIC AFFAIRS WEEK ENDING NOVEMBER 11, 1988 INTERNATIONAL PROGRAMS Foreign Visitors On Monday Mr. Jacques Furet, Head of the Electric and Nuclear Instrumentation Division at the Nuclear Center in Saclay, France, met with NRR representatives to discuss control rooms, technical support centers, human factors and emergency preparedness. Mr. Furet is a contractor for the International Atomic Energy Agency, preparing a review report on "The Design of Control Rooms and Man-Machine Interface for Operation and Surveillance of Nuclear Power Plants." He discussed with GPA/IP staff his plans to visit the La Salle, South Texas and Palo Verde Nuclear Power Plants.

On Wednesday and Thursday, Mr. Angel Coello, Environmental Qualifications Branch, Division of Electrical Engineering and Auxiliary C of Analysis and Assessment, Consejo de Seguridad Council Nuclear (ystems, Office of Nuclear Safety), Spain, met with staff from Region II and NRR to discuss environmental and equipment qualification.

Italy The NRC was infonned this week that amounts of radioactive contamination of Cobalt-60 were fouM in shipments of steel flanges supplied by Venete, a steel mill company in Italy, and imported into the U.S. for use in oil pipeline work. The broker, Ferrostall Metals, Houston, Texas, has notified the exporter of a shipment to National Flange, Houston, of the contaminated flanges. The NRC issued a Preliminary Notification on November 8 and 9, 1988, and notified the Italian government through ENEA/ DISP (the National Comission for Nuclear and Alternative Energy Sources' Directorate for Nuclear Safety and Health Protection), the U.S. Embassies, and its administrative contacts.

National Flange seeks to return the contaminated flanges to Italy.

NEA Vacancy Announcement The Organization for Economic Cooperation and Development's Nuclear Energy Agency has announced a grade A-5 position as head of the Nuclear Safety Division. The deadline for receiving applications in Paris is December 9, 1988. The announcement is posted on NRC bulletin boards.

During the week ending September 23, 1988, licenses for export were issued to:

1) Edlow International for the export of 80.5 kgs of low-enriched l (19.95%) uranium to Denmark for use as fuel in the UR-3 Research l Reactor; and for the export of low-enriched (4%) reload fuel for l four nuclear power reactors in South Korea.
2) Department of Energy for the export of 200 grams of Plutonium-239 and 200 grams of Neptunium-237 to EURATOM for US/ EURATOM collaborative research.
3) General Electric Company to authorize the re-export of Canadian-origin material to Spain for use in the Cofrentes nuclear power reactor.
4) Martek Corporation for the export of an additional 750 kgs of heavy water to Canada for upgrading and return to the U.S.
5) Los Alamos National Laboratory for the export of 5.250 curies of tritium to Switzerland for use in muon-catalyzed fusion experiments.

STATE, LOCAL AND INDIAN TRIBE PROGRAMS LILC0 Shareholders Approve Settlement Agreement On November 4,1988, LILCO shareholders approved the proposed settlement agreement to sell Shoreham to New York State. LILCO CEO William Catacosinos stated that he would remain committed to the settlement agreement until December 1, 1988. The New York legislature currently plans to address the settlement agreement during a session scheduled to begin on November 28, 1988.

Referendum For Nebraska To Withdraw From LLW Compact Fails The Statewide referendum which would have required Nebraska to withdraw from the Central Interstate low-level Radioactive Waste Compact failed by l approximately a 2-1 margin (64% against to 36% for). The referendum would have also required State approval before any low-level radioactive waste disposal facility is built in Nebraska and approval of a proposed site by voters within 10 miles of the site. Nebraska is the host State in the Central LLW Compact.

3 Referendum To Close Nuclear Power Plants Fails in Massachusetts Massachusetts voters turned down a referendum which would have banned the generation of electric power by plants that produce nuclear waste effective July 1, 1989. This would have affected Pilgrim and Yankee Rowe. Approxi-mately 68% were against the proposal and 32% were for the proposal.

Office of Enforcement Items of Interest Week Ending November 11, 1988 The following Significant Enforcement Actions were taken the week of November 7, 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $75,000 was issued on November 7, 1988 to Consolidated Edison Company of New York, Inc. (Indian Point 2). The action was based on violations of NRC requirements for the environmental qualification of certain items of electrical equipment. (EN 88-93)

Civil Penalties Paid Carolina Power and Light Company (Brunswick Unit 2) paid the imposed amount of

$25,000. The licensee responded to the Notice in letters dated August 24 and Septer.be r 26, 1988 acknowledging the violation, paid the base civil penalty but contested the escalation of the civil penalty. After consideration of the licensee's responses, the staff concluded that the escalation was appropriate.

Combustion Engineering, Inc., Windsor, Connecticut paid the proposed civil penalty in the amount of $12,500. The action was based .,n failure to perform adequate surveys, failure of certain individuals to wear gloves while handling certain contaminated equipment, and the failure to supply certain visitors with badges containing criticality monitoring devices, shipping radioactive waste to a burial ground without meeting shipping requirements, and failure to pro erly measure the uranium-235 content of certain waste materials. OFFICE OF CONSOLIDATION Items of Interest Week Ending November 11, 1988 Temporary Parking tot The Director, CONS met with representatives of the Lerner Companies and the architect / project manager, Karl Kaufman, on Wednesday, November 9. At this point, Kaufman projects the parking lot will be ready for use on December 1 and will be operated by a private contractor, Diplomat Parking. Parking will be by permit only. There will be no provisions for hourly or daily parking and the fees are likely to be $50./ month for single-occupant drivers and

$30./ month for carpools. One item yet to be resolved is the Employee Recreation Association's possible role in selling the parking tickets and validating legitimate carpools.

Vrain. The amendments upgrade the safeguards requirements for ,

fuel facilities possessing formula quentities of strategic special nuclear material. Commissioner Carr, while approving the rule changes preferred to defer the requirement for two separate barriers until completion of a performance analysis of existing security systems to support the need for such a change.

Commissioner Curtiss did not participate in this act'on.

The Commission, with Chairman Zach and Commissioners Roberts, Carr, and Rogers agreeing, approved the staff's recommendation to initiate, pursuant to 10 CFR Sec. 73.5, an exemption from the new requirements for Fort St. Vrain. The basis for issuing the exemption is the fact that at the Fort St. Vrain site, the special nuclear material is only in the form of fabricatea fuel. This fuel would require extensive processing to yield weapons useable material. Furthermore, the weight and bulk of these fuel elements are such that they represent an undesirable source for the material. Hence, the Fort St. Vrain sito is not comparable with fuel facilities possessing formula quantities of strategic special nuclear material.

Commissioner Curtiss did not participate in this action.

  • Section 201 of the Energy Reorganization Act, 42 U.S.C. Sec. 5841, provides that action of ?.he Commission shall be determined by a "majority vote of the members present."

commissioners Carr and Curtiss were not present when this item was affirmed. Accordingly, the formal vote of the Commission was 3-0 in favor of the decision. Comminsioner Carr, however, had previously indicated that he would approve this paper and had he been present he would have affirmed his prior vote.

Commissioner Curtiss did not participate in this action.
