' WEEK ENDING NOVEMBER 4, 1988 Salem During the Salem, Unit 2 refueling outage, defective tubes were found in the row 1 tubes in two of the four steam generators. Public Service Electric and Gas opted to plug all the row 1 tubes in all four of the Salem 2 steam generators, in addition, some unrecovered loose parts were left in the reactor coolant system. Because Salem 2 used the 1978 Westinghouse ECCS evaluation model, GL 86-16 requires a reevaluation using an up-to-date, corrected model. The reanalysis by Westinghouse will take about five months.

A temporary exemption from the requirements of 50.46 was issued to PSE&G on November 1,1988, to allow Salem 2 to operate during the tirie the reanalysis is being performed.

The NRC consent incorporated Sequoyah Holding Corporation comitments related to decorrissioning funding and noted the requirement fur a license anendment reflecting changes in corporate oversight of plant operations. Concurrent with the letter from Sequoyah Holding Corporation by application dated October 18, 1988, Sequoyah Fuels Corporation, requested a license amendment to reflect organizational and administrative changes resulting from the proposed transfer of ownership. Farley Emeroency Planning Exercise Robert Trojanowksi Director of Region !! State and Government Affairs Staff, participated in an emergency planning exercise at the Farley nuclear power plant on November 1-3, 1988. The States of Alabama and Georgia and the applicable local governments participated in this full-scale exercise. During the exercise, the alert notification system failed to function properly which will most likely result in a FEMA deficiency finding. The licensee and the State of Alabar.a have initiated action to identify and resolve the problem.

ADMINISTRATIVE MATTERS Rocky Mountain Trading Company (RMTC) Bid Protest on Award for Microcomputer Software NRC has suspended placing any additional orders for specific microcomputer software items listed in the contract awarded to General Technology, Inc.,

pending the outcome of the protest. The contractor will be allowea to complete delivery of software NRC ordered before the protest was filed by RMTC.

The "Rule 4" protest file was submitted to the GSA Board of Contract Appeals and to RMTC on October 26, 1988. Affidavits responding to the protestor's interrogatories were filed October 28, 1988. Depositions were taken November 1, 1988.

L OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending November 4, 1988 General, Electric On October 28, 1988, the BWR Owner's Group and General Electric contacted NRR and Region !!! concerning the results of analyses performed by General Electric for the Owner's Group. The analyses simulated thermal-hydraulic instabilities using the TRAC model with three-dimensional kinetics and thermal-hydraulics. A set of analyses designed to determine the magnitude of possible localized oscillations for readily detectable average power range monitor (APRM) indications showed that, for operation at the extreme limits of the Maximum Extended Operating Domain (ME00), critical power limits could be exceeded. The analyses showed a loss of critical power margin for peak-to-peak oscillations of 10% to 20% indicated on the APRM (local  ;

escillations of approximately 2001 peak-to-peak). The loss r:f margin only occurs for conditions above the 100% rod control line.

General Electric is developing interim guidance for use by the Owners.

Comonwealth Edison has elected to limit operation to the region below the 100% rod control line and to trip the reactor for a two-pump coastdown and <

flow less than 45%. General Electric plans to issue a letter to each BWR Owner describing the analysis results and the Comonwealth Edison actions by ,

the close of business on October 28, 1988. Comonwealth Edison also plans to enter the information into the Note-pad system.

While GE indicates the concern applies to BWR 55, 6s, and later 45, the only plants which currently operate at the ntreme of the MEOD region are Grand  ;

Gulf and Perry. Perry was notified by Region !!! of this issus and we L intend to verify Grand Gulf on 10/31. GE is planning to meet with all BWR Owners on 11/2 and both GE and the Owners will meet with the staff on 11/9.

Arkansas 1 On October 26, 1988, at about 2:35(EST), with the unit in cold shutdown and mid-loop operation, decay heat removal (DHR) system flow to the pressure vessel was lost for a total of about 23 minutes. This occurred when the A DHR heat exchanger (HX) outlet control valve closed on loss of control power when the wrong fuse was removed by an instrumentation and control technician during a maintenance activity. Although the event did not have imediate safety consequences (the decay heat was low, coolant temperature was initially about 68'F and the temperature rise was estimated to be about 7'F the loss of DHR was of short duration and level control of the reactor coolant system was not a factor), several other general concerns were identified: (1) control power to the cutlet control valves for both the A and the B DHR HXs was routed through a single fuse (the low pressure injection function, to go full open, of these valves was not affected because of separate circuits witi independent trains of power); (2) there was loss of control of a maintenance activity; (3)

the DHR low flow alarm was ineffective (the DH flow had been reduced to just above the set-point to avoid cavitation in the DH pump; the alarm had become a nuisance and was ignored); and (4) there was no instrumentation to monitor the temperature rise in the pressure vessel in the no flow condition. (The temperature rise has to be estimated through calculation.) Warren Lyon and Harry Balukjian of the Reactor Systems Branch were onsite from October 26-28, for a Generic Letter 88-17 related review of the ANO-1 DHR system mid-loop operation. They provided assistance tu the resident inspectors in the followup of this event. The Unit is currently in the last third of its eighth refueling outage. This event will be discussed at the November 1,1988 events briefing.

Vemont Yankee Early or. Saturday, October 29, 1988, the licensee entered a maintenance program that required the two recirculation pumps to be taken out of service.

Sequentially, the flow rate in both pumps was reduced to the pump's minimum flow poiht, corresponding core themal power of about 55% of rated power.

This resulted in a core power oscillation of about Pump (A was then tripped.Nomally, this procedure would result in a 5% peak-to-peak 20% peak-to-peak).

power oscillation. The operators responded by increasing flow on pum) 8 to 70% of rated flow. This action damped the oscillations and stopped tiem within 30 seconds. The plant is Luthorized for single loop operations and procedures did not require a plant trip. Thus, the licensee continued with its maintenance procedures. After three hours the procedure was repeated but with pump B being tripped. The power oscillation in this case were observed to be about 5% (peak-to-peak) as expected.

The plant procedures require a reactor trip only on dual pump trip or if power oscillations of greater than 10% were to occur for more than two minutes.

While the "pump A" oscillations exceeded 10%, the 2 minute criterion was not exceeded; thus, the plant was not tripped. On November 1, 1988, the plant is operating at full power.

The NRR and Region I staff's had a conference call on this event at 11:00 a.m.

on Tuesday, November 1, 1988. The licensee and GE will complete their analysis of the event in a few days. The Region has submitted some data on the event and some questions which were transmitted to the Reactor Systems Branch. Both the Region and NRR are closely coordinated and following the event.

SAFEGUARDS Dorestic Headquarters / Region 1 Joint PC&A Inspection During the week of Ortober 31, 1988 a menber of Dhiston of Safeguards and Transportation participated in an unannounced Material Control and Accounting inspection at Cintichem Tuxedo NY (a Category 11 fuel facility). The inspection was a Headquarters /Pegion ! joint effort.

,tnternattenal Safeguards for Large Scale Reprocessinc plants (LASCAR)

A Japanese initiative to support the International Atonic Energy Agency (IAEA) in tee develorrer.t of effective and efficient safeguards for large scale retro-cessing plants has been formalized under LASCAR. The first Plenary Meeting under LASCAR which was held in hoto and Tokyo, Japan froc October 17-24, 1988, was attended by representatives from FN, France, UK, IAEA, EURATOM, USA and Japan. The products of the reeting were the develeprent of definiti t ebjectives for LASCAR, the finalization of a set of working papers, and the formation of the first working i,roups to address the technical issues that will impact on the application of safeguards. TRANSPORTATION Hecting on TRUPACT-11 Shipping Cask On October 26, 1988, the Transportation Branch met with DOE, Nuclear Packaging (NUPAC), Westinghouse, and Sandia to discuss TRU waste forms to be shipped in the TRUPACT-II shipping package. Specific items discussed include TRU waste characterization, compatibility of waste forms, and gas generaticn. NUPAC has proposed categorizing the waste based on physical and chemical form, gas generation rates. anti radiation source strength. The procedure for characterizing 1s lividual waste drums and for loading drums into the TRUPACT package will be addressed in a later meeting in December or January. An application for the TRUPACT-II package is expected in February 1989.

Meeting on GA-4 Truck Cask On November 1, 1988, the Transportation Branch met with General Atomic concerning the proposed Model No. GA-4 truck cask for shipment of spent fuel under NWPA. The cask has a capacity of four PWR elements and weighs 55,000 pounds. The cask cavity is surrounded by a 2.5-inch thick depleted uranium gamma shield, 1.5-inch thick stainless steel containuent vessel, and a 5.5-inch thick boro-silicone neutron shield. The applicant discussed methods of analysis which will be used to evaluate the cask for normal and accident conditions. The applicant is planning 1/4 and 1/2 scale tests of the impact limiters and the cask. An application is expected to be submitted in about two years.

HIGH LEVEL WASTE MANAGEMENT Resignation Of Charles E. Kay, Director. OCRWM, DOE Mr. Charles E. Kay, Acting Director of the Office of Civilian Radioactive Waste Management at the U.S. Department of Energy infortred fir. Hugh Thompson on October 31, 1988, that b will be resigning as of November 4, 1988 to take a position with the U.S. Dr.partinent of Interior. His replacement as Acting Director will be Mr. Sam Russo. Mr. Frank Peters will be assigned as permanent Deputy Office Director.

Status Of HRC Staff Observation Audit Of DOE Nevada Waste Management Project Office Audit eriod of October 24-28, 1988, the Division of High-Level Waste During the Management p(HLWM) staff performed an observation audit of the DOE Nevada Yucca liountain Project Office (YMPO) audit of the Lawrence Livermore National Laboratory (LLL). LLL is responsible for developing the waste package for the tuff environment found at the Yucca Mountain Site. The purpose of the staff observation audit was to assess whether DOE was conducting its audit so th6t the staff could gain confidence that the LLL QA program was being properly implemented.

4 As a result of its observatlons and reviews, the HLWM staff believes that the DOE /YMP0 team conducted a complete and effective audit. A summary report describing the staff observations will be issued.

With respect to the LLL program, the staff has several concerns as a result of findings made by the YMP0 team. These include: (1) the lack of an effective training program; (2) the fact that corrective actions are not being performed in a timely and effective manner; (3) the lack of an effective records manage-sent system; (4) no procedure to control interfaces between LLL and other participants; and (5) the need for LLL to reevaluate the QA level assignmeats of its ongoing work. In addition to these major findings, the team also iden-tified many other issues representing procedural violations. Overall, the audit resulted in 38 findings or, the LLL program. ,

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Ir.tcrc:t Week Ending November 4, 1988 PREVENTING DAMAGE TO REACTOR CORES Evaluation of Instability in BWRs TheReactorandPlantSystemsBranchsponsoredameeting(duringtheWater Reactor Safety Information meeting) to exchange information on the analysis of instabilities in BWRs. About 50 persons attended, including 18 persons from foreign organizatfons. Presentations were made by representatives of ORNL, BNL, EPRI, EPRI/NSAC (Nuclear Safety Analysis Center), JAERI (Japan), Toshiba (Japan), Hitechi (Japan), Spain, Sweden, and Switzerland. As a result of the presentations, it was observed that calculations perfonned to date by BNL under RES sponsorship are consistent with calculations performed in Japan.

O D Items of Interest Office for Analysis and Evaluation of Operational Data Week Ending November 4, 1988 On October 30, 1988, the Director, AE0D met with members of the National Emergency Management Association (NEMA) in Kansas City, Missouri to participate as a panel member on a program with FEMA and utility representatives during their annual fall conference. The NEMA members expressed concern over the implementation of new FEMA Guidance Memoranda as new requirements, causing previously acceptable programs to have significant deficiencies.

Diagnostic Evaluation and Incident Investigation On November 1,1988, the Director, AEOD. Femi Diagnostic Evaluation Team (DET)

Manager and DET Deputy Manager, together with the Deputy Executive Director for Regional Operations, Region III Administrator, Acting Assistant Director of Projects, NRR, and other NRC personnel met with Detroit Edison management officials at the Fermi site to present and discuss the results of the diagnostic evaluation. The briefing, which consisted of the team's preliminary findings and conclusion was led by the DET manager. On November 4, the final DET report for Fermi will be forwarded to the Director, AE0D for review and approval.

Incident Response On November 2, the IRB Branch Chief and staff met with representatives of the Department of Energy (00E) and their contractor, and conducted a tour of the NRC Operations Center as part of DOE's efforts to upgrade their emergency response facilities.

On November 3, the IRB Branch Chief conducted a briefing and tour of the NRC Operations Center for a group of Soviet visitors.

Trends and Patterns Analysis The second quarter draft A0 report has been sent to the EDO for forwarding to the Conunission for approval. The report contains three proposed A0s (none at the nuclear power plants, two at the other NRC licensees, and one at an Agreement State licensee). The report also contains eight updating items and l six "Other Events of Interest."

CRGR CRGR visited the Grand Gulf Nuclear Station on November 2-3, 1988. Foreign Visitors On Tuesday, October 25, Prof. Adolf Birkhofer of the Federal Republic of Germany's Gesellschaft fuer Reaktorsicherheit (GRS) met with Chairman Zech and members of the RES staff to discuss NUREG-1150, activities of the senior regulators and the Nuclear Energy Agency, and other current safety issues. The Chaiman also hested a lunch for Prof. Birkhofer, attended by other Comissioners and senior NRC staff.

On Monday Messrs. Rafael Caro and Luis Echavarri, Comissioners at the Consejo de Seguridad Nuclear (CSN) in Spain, met with Comissioners Roberts and Carr to discuss bilateral activities between NRC and CSN in general. They also discussed organization and activities at the CSN and areas in which NRC-CSN safety cooperation could be expanded, suggesting that these be considered during the upcoming discussions on renewing the NRC-CSN nuclear safety exchange arrangement. Similar discussions were held with Chairman Zech, Comissioner Curtiss E00 Victor Stello, and GPA Director Harold Denton during meetings on Tuesday.

On Monday and Tuesday, GPA/IP and RES staff met with Mr. Jose C vo, Head of the CSN Working Group on Probabilistic Safety Analysis, to discuss a number of topics in the area of reliability and risk.

On Tuesday Mr. Toshiyasu Sasaki, the new Executive Director of Japan's Power Reactor and Nuclear Fuel Development Corporation (PNC), met with Victor Stello, EDO, and representatives from GPA/IP and NHSS for a discussion of the Furkowski Amendment on the air shipment of plutonium.

On Tuesday Mr. Remy Carle, Vice President of Electricite de France, in Washington to attend the American Nuclear Society meeting, paid a courtesy call on Chaiman Zech and Comissioner Curtiss.

On Friday Messrs. Alberto Loper Garcia, Director General, and Aurelio M.

Ulibarri, Engineering and Projects Director, Spanish National Waste Management Company (ENRESA), met with staff from NMSS and GPA/IP to discuss the possible Spanish purchase and NRC certification of American dual purpose cask designs for the transportation and storage of waste.

Nuclear Export Control Meeting On Monday, Tuesday, and Wednesday, October 24-26, there was a meeting of the Subgroup on Nuclear Export Coordination (SNEC) at the Los Alamos National Comerce Laboratory for the Nuclear Referral Listpurp(ose NRL). - 2 nonproliferation reasons. There has been a need to update the NRL to reflect the results of SNEC's technical reviews and licensing experience of recent years, and to be responsive and Supportive of U.S. efforts to obtain improved nultilateral cooperation in dual-use export controls.

The extensive review included the deletion, addition, and modification of many items on the NRL.

Robin Delobarre, OES/NEC, Department of State, headed the meeting which included representatives from DOE, ACDA, DOS, DOC, CIA, DIA, D00, NRC, Sandia Laboratory, Oak Ridge National Laboratory, Los Alamos National Laboratory, and Meridian Corporation (DOE contractor). Marvin Peterson and Elaine Hemby were the NRC attendees. NRC coordinated the update exercise and, along with DOE and Meridian Corporation, will follow up on the review to ensure that the proposed NRL revisions are reflected in the DOC regulations.

00E/NRC/D0D Physical Protection Infomation Exchange Trip A team of physical protection experts from the Departments of Energy and Defense and the Nuclear Regulatory Comission visited Switzerland, Belgium, and Greece from October 18-November 2 as part of an ongoing worldwide program to exchange information on national physical protection practices. In addition to formal discussions with host government nuclear physical protection officials, the team visited the Swiss Saphir and Proteus research reactors and Hot Laboratory for waste recovery at the Paul Scherrer Institute at Wuerenlingen; the Belgonucleaire M0X Fuel l Fabrication Facility, Plutonium Laboratory, the Societe Franco-Belge de Fabrication de Combustibles (FBFC) plant, and the BR-2 research reactor at the Centre d' Etude de l'Energie Nucleaire/Studiecentrum voor Kernenergie (CEN/SCK) in Mol, Belgium; and the GRR-1 research reactor at the Demokritos energy research facility in Athens, Greece.

( ANS Convention Briefed on U.S.-Soviet Cooperation Harold Denton, Director of GPA, and James Taylor, Deputy EDO, took advantage of the American Nuclear Society (ANS) convention in Washington l this week to hold a special session on "Current Status of U.S./ USSR l Civilian Nuclear Reactor Safety Cooperation." Dr. Nikolai Ponomarev-Stepnoi represented the USSR at the Session. Taylor outlined the ten technical areas agreed to during the Chaiman's trip to the Soviet Union in August. Ponomarev-Stepnoi went beyond the agreed upon items to suggest that perhaps an international research consortium on causes and consequences of Cherncbyl should be organized. He also reached out to the U.S. and western European nuclear industry to encourage further cooperation in safety with the Soviets, tenning such activity potentially beneficial to both sides. The meeting was chaired by President of the ANS, Dr. Gail LaPlanque, and was well attended.

Holt, recently appointed Region III State Agreements Officer, visited SLITP on Hovember 2-4, 1988 as part of her orientation to the State Agreements program. In addition to briefings by State Agreements staff, Ms. Holt participated in a meeting with Terry Lash, Director of the Illinois Department of Nuclear Safety, Harold Denton, Director of the Office of Governmental and Public Affairs and Carl 1:amerer, Director of State, local and Indian Tribe Programs. She also joined SLITP staff at a Government Liaison training session at the NRC Operations Center and met Comissioner Assistants.

Farley Emeroency Planning Exercise Robert Trojanowksi Director of Region II State and Government Affairs Staff, participated in an emergency planning exercise at the Farley nuclear p0wer plant on November 1-3, 1988. The States of Alabama and Georgia and the applicable local governments participated in this full-scale exercise. During the exercise, the alert notification system failed to function properly which will most likely result in a FEMA deficiency finding. The licensee and the State of Alabama have initiated action to identify and resolve the problem.

Office of Enforcement Items of Interest Week Ending November 4, 1988 The following Significant Enforcement Actions were taken the week of October 31, 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $2,500 was issued Oc.tober 28, 1988 to Shadyside Hospital, Pittsburgh, Pennsylvania. The action was based on failure to recover and return a nuclear pacemaker to the manufacturer upon the death of an individual. It was disposed of in the normal trash by the funeral home, a method of disposal not authorized by the licensee nor NRC regulations. (EN 88-92)

An Order Imposing Civil Penalty in the amount of $25,000 was issued to Florida Power Corporation (Crystal River, Unit 3). The action was based on a violation involving the failure to take appropriate corrective action to resolve a deficiency regarding the electrical loads placed on one of two plant emergency diesel generators. The licensee responded to the Notice in a letter dated June 2, 1988. After consideration of the licensee's response, the staff concluded that 50% mitigation of the civil penalty was appropriate because of the licensee's identification of the violation. (EN88-33A)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued November 1, 1988 to Boston Edison Company (Pilgrim).

The action was based on an access control violation, the failure to take prompt corrective action to mitigate the consequences of the violation, and a deliberate material false statement by a security lieutenant concerning his knowledge of the vulnerability. (EN88-94)

Civil Penalty Paid Power Authority of the State of New York (Indian Pt 3) paid the proposed civil penalty in the amount of $75,000. The action was based on a violation of the equipment qualification requirements of 10 CFR 50.49. (EA 88-148)

Commuter Guide Within the week, a 3ersonal computer for use as a Commuter Guide will be installed outside t1e Administrative Services Center on Floor 2. This Commuter Guide is an automated, menu-driven, instructional system. It will provide information on the following: transportation costs to and from OWFN; how, when and where to buy discounted metro fare and Marc rail media; parking options; and the status of fringe parking lots within Montgomery County. It also will accept applications for carpool matching with fellow NRC employees.

On November 1,1988, the Execative Director for Operations approved a final rule that amends 10 CFR Part 50, Appendix J, "Primary Reactor Containment Leakage Testing for Water-Cooled Power Reactors," to permit the use of a statistical data analysis techniques (Mass Point) that the NRC staff considers to be an acceptable method of calculating containment leakage rates.

This constitutes notice to the Commission that, in accordance with the rulemaking authority delegated to the EDO, the EDO has signed this final rule and proposes to forward it on November 9,1988 to the Office of the Secretary for FR publication, unless otherwise directed by the Commission.
