SUBJECT:
QA/QC Qualifications OBSERVATIONS:
A surveillance was conducted at Hatfield on all on-site QA/QC inspectors.
Each inspector surveyed was done in accordance with ANSI N45s2.6 1978.
Four deficiencies were found.
Hatfield'QA/QC inspectors had no record of a practical exam, as per All s Procedure 17, Appendix C Table II.
All QA/QC inspectors j
have taken or will take a practical, exam and be included in their 1

. training record.
I Al Koca, certified Level III Inspector, var, certified on 12/5/81 for a period of one year.
During this year Mr. Koca was used
- i specifically for auditing, a QA function.
A question atose as to why Mr. Koca was certified a Level III in QC but was used primarily in j
QA.
Since QA must be a separate entity from QC Mr. Koca's ire.
This was Identified and he was certification was c.1. lowed to exp/83.

immediately recertified on 2/24 Mr. Koca will not do any QC i
i inspections and his primaty function is to audit.
John Malunda, who was qualified as a Level II in Procedure #10, has no documentation of OJT training or classroom lecture as per Hatfield's Procedure 17 Appendix B Revision 5.

certification was removed and will no longer inspect to Procedure #10.

1 Tom Wells was certified as a Level II to Hatfield's Welding i

Procedures.
As part of his certification, his experience while i
working at Continental Drilling was used to comply with ANSI N45.2.6.
This is unacceptable.
Hatfield was notified and his certification was i
voided.
His inspections have been reinspected for the first 3 months l
he was qualified.
