QA/QC Qualifications OBSERVATIONS: ,

! A surveillance was conducted at Hatfield on all on-site QA/QC inspectors. Each inspector surveyed was done in accordance with ANSI N45s2.6 1978. Four deficiencies were found.

, Tom Wells was certified as a Level II to Hatfield's Welding i

Procedures. As part of his certification, his experience while i working at Continental Drilling was used to comply with ANSI N45.2.6.

This is unacceptable. Hatfield was notified and his certification was i voided. His inspections have been reinspected for the first 3 months l'

he was qualified.
