HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING APRIL 25, 1986 Rancho Seco On April 18, 1986, the Sacramento Municipal Utility District (SMUD) met with the NRC staff in Bethesda to review SMUD's proposed plant performance and management improvement program for the Rancho Seco plant. This presentation was a follow-up to the meeting held between SRJD and senior NRC management on March 24-25, 1986 where SMUD was requested to take a broader perspective regarding the issues affecting l

the safe and reliable operation of the Rancho Seco plant beyond the narrow focus of responding to the December 26, 1985 transient.

i Accordingly, SMUD presented an outline of its program to' improve its management organization, systems and procedures, connunications, and plant performance. The organization is being restructured to include six departments vs the present three, with new management personnel being brought in from outside SMUD. The plan has been reviewed and approved by the General Manager and the Board of Directors.

l The plant performance improvement program will involve several l

multi-disciplined tea:ns assigned to make reconnendations for improvements that will minimize reactor trips, challenges to safety systems, or that are needed to permit the plant to stay in a subcooled i

j post-trip " window" for forced and natural circulation. The program will incorporate short-term elements requiring resolution prior to I

restart, and longer ters elements. SMUD expects the analysis portion of the program to be completed in June in order to support issuance of its startup report for NRC review in early July 1986.

SEQUOYAH FUELS CORPORATION, GORE, OKLAHOMA On April 23, 1986, the NRC staff authorized Sequoyah Fuels Corporation (SFC) to resume the receipt, sampling, and storage of uranium ore concentrates I

(yellowcake) at the Sequoyah Facility in Gore, Oklahoma. The staff also authorized the shipment of uranium hexafluoride cylinders which were in inventory at the facility at the time of the January 4, 1986 incident. These authorizations were issued following the preparation of safety evaluation reports for each activity, and contain special conditions related to assurance of appropriate operation and the condition of uranium hexafluoride cylinders to be offered for transport.

The activities approved by the staff constitute Phases I and II of SFC's three phase plan to renew operations at the Sequoyah Facility. The activities authorized do not include any chemical processing of yellowcake or production of uranium hexafluoride.

In taking this action, the staff noted that the NRC has not determined the eventual acceptability of resumption of the production of uranium hexafluoride. Decisions regarding uranium hexafluoride production are yet to be made and will be dependent upon matters such as SFC's plans for safety improvements at the facility, the results of studies and investigations undertaken by SFC and the NRC, and NRC's continuing analysis of SFC's commitment and ability to operate the plant in a manner which protects the health and safety of the employees, members of the public, and the environment.

Weekly Information Report Division of Contracts ADMINISTRATIVE MATTER On April 21, 1986, the Contracting Officer forwarded a letter to the Small Business Administration (SBA) protesting the small business representation made by Advanced Computer Environmental Systems Support, Inc., (ACESS) under Solicitation No.

RS-ADM-86-218. The solicitation was a total small business set-aside. ACESS, the apparent low bidder, certified that it is a small business and that all supplies to be furnished would be from another small domestic firm as required by the solicitation. However, a separate certification included in the bid package indicates that several of the required items will be produced in Israel. The matter has been referred to the SBA for a determination as to whether ACESS qualifies as a small business and is eligible for award.

0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending April 25, 1986 Fermi-2 Detroit Edison (DECO) announced on Friday, April 18, 1986 that it will replace the depleted neutron sources in the Fermi-2 facility. This announcement stated that it will complete this effort and the other issues which remain to be completed (e.g., the design issues and the embedments) by about mid-July 1986. They also plan to conduct some other work such as the local leak rate tests d ying this outage. This will allow Deco to conduct the power ascension test program starting in 1 ate July /early August without shutting down in October 1986, to conduct the LLRT's then.

Limerick On April 11, 1986, the Appeal Board denied Mr. R. L. Anthony's Motion for a stay of the effectiveness of License Amendment Nos. I and 2.

Those license amendments extended the surveillance testing interval in the Technical Specifications for certain containment isolation valves by about three months, on a one-time-only basis.

The Appeal Board also addressed Mr. Anothony's compliance with the Rules of Practice as follows:

We note that throughout his involvement with the Limerick proceedings Mr. Anthony has often failed to prepare his filings in accordance with the Commission's Rules of Practice, 10 CFR Part 2.

He has been warned about this on numerous uccasions....

In the future should his filings with us related to these license amendments fail to conform to the regulations, they either will not be docketed (and no leave to refile will be granted), or will be summarily denied for failure to conform to the rules.

Palo Verde Unit 2 Initial criticality on Palo Verde Unit 2 was achieved at 2:45 p.m. MST on April 18, 1986. The Commission meeting regarding full power is scheduled for April 23, 1986.

ANO-2 AN0-2 remains in hot standby following an unplanned reactor trip caused by an RCP trip from full power on April 21, 1986. A short-circuit from the installation of an apparently defective light bulb for an RCP status indicating light on the remote shutdown panel is believed to have caused the RCP trip. The licensee is investigating the root cause and related corrective actions associated with this event.

It should

be noted that there was no abnormal indication associated with the tripped RCP during this event to suggest an RCP shaft fault.

Comanche Peak' Units I and 2

'On April 18, 1986, Texas Utilities announced that its schedule to support comercial operation cf Unit 1 in mid 1987 and Unit 2 six months later is no longer achievable. The reinspection / rework effort by the Comanche Peak Response Team to address and resolve issues concerning safety of the plant has been expanded. Modifications are expected on about 30 percent of Unit 1 pipe supports and containment electrical penetrations are being replaced.

This construction effort on Unit 1 is expected to be completed before the end of 1986. However, because of uncertainties regarding further reinspection and rework and the nature and extent of the hearing process before the ASLB, Texas Utilities cannot predict exactly when the units will be in full power operation.

Texas Utilities continues to expect that Unit 2 will be ready for operation about six months after the operation of Unit 1.

Shoreham Emergency Planning On April 18, 1986 FEMA issued its report on the Shoreham Emergency Plan exercise conducted on February 13, 1986. The report was generally favorable but FEMA did not make a finding of " reasonable assurance" regarding the plan, deferring that decision to the NRC.

Perry 1 On several occasions during fuel loading operations (through April 20,1986), there were inadvertent Reactor Protection System (RPS) actuations due to upscale trips of the neutron flux Intermediate Range Monitors (IRMs).

For each occasion, the licensee attributed RPS actuations to " noise" in the IRMs caused by the incorrect grounding of the IRM cabinets to the reactor building. During one such occurrence, a control rod position indicator reed switch was believed to have been momentarily stuck, causing a false position indication for that control rod. The " stuck control rod" was subsequently stroked " full-out" and

" fill-in", with the position indicator showing that the rod operated as designed.

Modifications have been made by the licensee to improve the ground connection from the IRM cabinets to the reactor building. However, the licensee will be continuing to investigate the matter to assure that the IRM problems have been fully resolved.

On April 22, 1986, another spurious signal event had occurred involving the Average Power Range Monitors. The cause of this most recent event l

is under investigation by the licensee.

In addition, difficulty was experienced in loading one of the fuel assemblies into the reactor i

vessel, and some scoring was observed on the surface of the channel box l

when the problem assembly was removed. The cause for this recent difficulty is also under investigation.

Fuel loading has been halted, l

and as of 8:00 a.m. on April 23, 1986, 731 of the required 748 fuel l

assemblies had been loaded into the reactor vessel.

l In view of these difficulties and the need to fully investigate the l

causes of the problems related above, the need to complete certain pre-criticality surveillances interrupted by those problems, and the need to close-out certain regional open items before going critical, the plant's initial criticality has been rescheduled for May 13, 1986, by the licensee.

r NRC TMI-2 PROJECT DIRECTORATE WEEKLY STATUS REPORT WEEK ENDING MARCH 28, 2986 i

1.

DEFUELING No transfers of fuel debris canisters have taken place this period.

The total number of canisters transferred to date is 42 versus 44 reported last period. About 16% of the damaged core has been i

removed.

Work continues on removal of the biological contamination of the l

i Reactor Coolant System (RCS). The Temporary Reactor Vessel Water Filtration Systems (TRVWFS) has been operating for about one week and has reduced the RCS water turbidity to about 40 NTU. This corresponds to a visibility of 6 - 8 inches in the reactor vessel.

The equipment fer high pressure pumping and hydro-lancing of the RCS has been installed in the reactor building and is expected to be placed in operation on April 28, 1986.

PLANT STATUS l

The reactor remains in long term cold shutdown, vented to atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 79'F.

The airborne radioactivity on the defueling platform is about 3.0 E-8 uCi/cc Tritium and 1.0 E-11 uCi/cc predominately Strontium-90.

The platform is mounted above the modified internals indexing fixture which is mounted on the reactor vessel flange. These provide water coverage of 156 feet over the core region. This water i

level is about 5 feet over the top of any debris canisters in the reactor vessel.

WASTE MANAGEMENT The Submerged Demineralizer System (SDS) is shutdown. SDS total processed to date is 3,885,859 gallons.

EPICOR II was in temporary shutdown this week. EPICOR II total processed to date is 2,854,371.

ENVIRONMENTAL MONITORING US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.

TMI water samples taken by' EPA at the plant discharge (includes TMI-1 and TMI-2) to the river consisted of seven daily composite samples taken from April 5 - 12, 1986. A gamma scan detected no reactor related activity above the lower limit detectability.

The Lancaster water sample taken at the water works intake and analyzed by EPA consisted of a seven day composited sample taken from April 5 - 12, 1986. A gamma scan detected no reactor related radioactivity.

The NRC outdoor airborne particulate sampler at the TMI site collected a sample between April 17 - 23, 1986. No reactor related radioactivity was detected. Analysis showed Iodine-131 and Cesium-137 concentrations to be less than the lower limits of detectabili ty.

The TMI outfall sample for March 29 through April 5,1986 was incorrectly reported as not quantifiable in the last status report.

The result was 3 pCi/l 1 pCi/1, just above the systematic lower limit of detectability of approximately 2 pCi/1.

AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES Scabbling of floor areas on the 281' elevation continued.

Vacuum cleaning of the 281' elevation continued.

The reactor building sump filters were changed out this week and vacuum cleaning was done in the filter room.

Staff briefed the ACRS on January 15, 1986, and NMSS Director on February 21, 1986, on proposed revisions to 10 CFR Part 60 for conformance to EPA HLW standards (40 CFR Part 191).

Action:

The proposed Federal Register notice of revisions to Part 60 (SECY-86-92) was submitted to the Commission in late March 1986 for approval and publication by late April 1986.

Project Decision Schedule Status:

DOE issued the draft Project Decision Schedule (PDS) on July 18, 1985 for comment.

NRC transmitted formal comments to DOE on October 24, 1985.

Action:

DOE published a Notice of Availability of the PDS in the Federal Register (51 F_R 11466) on Thursday, April 3,1986, and an official copy of the PDS was transmitted to the Chairman on April 11, 1986.

Staff is now reviewing the document for DOE response to NRC comments.

NRC has developed revisions to 10 CFR Part 72 to provide the licensing framework for the MRS, should it be authorized by Congress.

Action:

The proposed rule on 10 CFR Part 72 was submitted to the Commission (SECY-85-374) on November 15, 1985, and a supplement (SECY-85-374A) concerning State / Tribal involvement was submitted to the Commission l

on March 14, 1986.

Both papers have been approved by the Commission l

and the Staff Requirements Memorandum is expected soon.

i SEQUOYAH FUELS CORPORATION, GORE, OKLAHOMA On April 23, 1986, the NRC staff authorized Sequoyah Fuels Corporation (SFC) to resume the receipt, sampling, and storage of uranium ore concentrates (yellowcake) at the Sequoyah Facility in Gore, Oklahoma. The staff also i

authorized the shipment of uranium hexafluoride cylinders which were in inventory at the facility at the time of the January 4,1986 incident.

These authorizations were issued following the preparation of safety evaluation reports for each activity, and contain special conditions related to assurance of appropriate operatian and the condition of uranium hexafluoride cylinders to be offered for transport.

The activities approved by the staff constitute Phases I and II of SFC's three phase plan to renew operations at the Sequoyah Facility. The activities 1

authorized do not include any chemical processing of yellowcake or production of uranium hexafluoride.

In taking this action, the staff noted that the NRC has not determined the eventual acceptability of resumption of the production of uranium hexafluoride.

Decisions regarding uranium hexafluoride production are yet to be made and will be dependent upon matters such as SFC's plans for safety improvements at the facility, the results of studies _and investigations undertaken by SFC and the NRC, and NRC's continuing analysis of SFC's i

commitment and ability to operate the plant in a manner which protects the health and safety of the employees, members of the public, and the environment.

Ongoina Transportation Information Management System (TIMS) Development On April 16, 2986, staff from the Transportation Integration Project I

(TIP)/FCMS met with staff from the DOT Office of Hazardous Materials Transportation to discuss NRC's use of their database, the Hazardous Materials Information System (HMIS).

The Incident Sub-System and the Radioactive Materials Routing Sub-System within HMIS were of special interest. Access privileges have been granted, and staff is now becoming familiar with its operation.

Earlier this month, staff also obtained access privileges to DOE /0CRWM's electronic bulletin board, INFOLINK, and applied for read privileges to Sandia National Laboratories Radioactive Materials Incident Report (RMIR) database and the Radioactive Materials Packaging (RAMPAC) database. These and other steps are being taken to augment TIMS capabilities in compiling statistics on RAM transportation shipments and incidents.

Safety Safety Inspections An IE Safety System Outage Modifications, Inspecticn (SS0MI) team is at the Dresden Unit 3 site to conduct an inspection of installation activities associated with plant modifications made during the present eight month outage. Previous IE inspections in the modification and design and procurement areas were completed in January 1986 for the outage. An associated preoperations readiness inspection is planned for June-1986.

Emergency Preparedness (1) An emergency preparedness exercise' report for the exercise of the LILC0 Offsite Emergency Response Plan for Shoreham was provided by FEMA. The report, which identified five deficiencies and several areas requiring corrective action, was forwarded to Region I for transmittal to the licensee.

(2) Emergency preparedness exercise reports for Braidwood and Oyster Creek were forwarded by IE to the regions for appropriate action. While there were no deficiencies in oftsite preparedness observed during the exercises, FEMA did identify areas requiring corrective action and areas recommended for improvement.

(3) A staff member from the Emergency Preparedness Branch, DEPER, participated as an observer in the partial participation exercise at St. Lucie.

(4) A contractor-prepared report on evacuation time estimates within the 10-mile EPZ for Braidwood was forwarded to Region III for completion of the emergency plan evaluation.

Week Ending April 25, 1986 Modelling of Two-Phase Flow A review of the program for modeling cf two-phase flow at Argonne National Laboratory (ANL) was conducted on April 9,1986. The purpose of the ANL program is to assess scaling compromises in experimental facilities and to develop two-phase flow models and correlations to support accurate light water reactor safety analyses. Data for the program comes from the hot leg U-bend experiments which consist of an air-water experimental loop and a Freon loop to stury two-phase flow regimes, flow separation mechanisms and natural circulatior termination for Babcock and Wilcox (B&W) type reactors. It is part af the Integral System Test (IST) program that will provide data on speci/ic issues / phenomena relevant to post small break loss-of-coolant acciJent transients for B&W reactors. Data gathering and analysis is ongoir.g in the air-water hot leg U-bend (4 in. in diameter) and emected to be completed in a couple of months. Construction of the Freon low; (2 in. in diameter) is near completion. The Freon loop will be used to sim64 ate the boilirg and condensation in the hot leg U-bend as well as to check fluid property dependency of flow regime transition.

In addition, scaling criteria for a natural circulation loop under single and two-phase flow conditions will be published in a technical report entitled, " Reduced Pressure and Fluid to Fluid Scaling Laws for Two-Phase Flow Loop," NUREG/CR-4584 in April,1986.

Severe Accident Research Plan (SARP)

Over the pa'st five years a major research effort has been underway by the NRC to develop an improved understanding of severe accidents and to provide a l

technical basis to support regulatory decisions. A Severe Accident Research i

Plan (SARP) was issued in January 1983 as NUREG-0900. Revision 1 to this research plan has been issued (April 1986) to document our continuing plans for research. The plan discusses (for each research subarea) the scope of research, the research accomplishments to date, the outstanding issues, and the planned activities. The plan reviews the 18 major NRC/IDCOR technical issues t

and also covers the eight major areas of uncertainty in source term analysis identified in draft HUREG-0956.

This continued research is intended to quantify and where possible diminish the rather large uncertainties in severe accident phenomena which exist at this time.

. Earthquake Near Seabrook A small earthquake of magnitude 3.0 occurred at 11:21 p.m. local time on 16 April 1986 approximately 11 km southwest of the Seabrook Nuclear Power Station. The earthquake was not felt or recorded at the site; the plant's seismic monitoring instruments are not yet in operation.

Another small earthquake of magnitude 2.6 occurred at 2:00'a.m. local time on 22 April 1986 in eastern Westchester County, New York. This event occurred in the immediate vicinity of a series of small earthquakes that have been occur-ring in Westchester County since October 1985. The event was felt over an area of 12 to 15 km radius; no damage was reported. Both of these events were recorded by the NRC's Northeastern U.S. regional seismic monitoring network.

. RES RULEMAKING ACTIVITIES Severe Accident Risk Assessment The RES staff and supporting contractors are currently engaged in the reevalua-tion of the risk of six reference LWRs. This reevaluation is currently being used to support NRR's review of IDCOR submittals.

In addition, it will support the staff report NUREG-1150, which is intended to provide guidance to decision-makers as to the appropriate role of information on core melt frequency, containment performance, source terms, and risk in making severe accident decisions.

Meetings were held among RES, NRR, and appropriate contractors to discuss the uncertainties associated with the treatment of the various issues identified in NUREG-1150. These meetings will aid in developing NRC positions on uncertainty ranges and included discussions of hydrogen generation and loading (April 21-22) and direct heating (April 22-23).

An ACRS subcommittee meeting on the methods employed in the NUREG-1150 analyses, including uncertainty analyses, and presenting preliminary results will be held in Albuquerque, New Mexico, on May 1, 1986.

Procedures have been established which require the personal approval of the Director, Division of Risk Analysis and Operations, RES, of any project modifications or redirections that could adversely affect either schedules or i

Shipping Cask Response to Severe Transportation Accidents--Modal Study The purpose of this study is to determine the performance of licensed casks under various types of realistic accidents that might occur during shipments of spent fuel. This purpose is being accomplished by assessing the performance when subjected to severe transportation accident environments, as well as by assessing the likely frequency of such environments. A draft final report was received on April 23, 1986.

Presentations on the results of this study are being given to the Region II State Liaison Officers' Meeting on April 22 and to the National Conference of State Legislators Workshop on May 1.

The content of these presentations has been submitted to NMSS for their comments.

LLW Disposal in Massachusetts The Massachusetts Senate has petitioned the Massachusetts Supreme Court for a decision as to the constitutionality of provisions of a 1982 law adopted by voter initiative; the law in question requires voter approval of the establishment of a low-level waste site or the joining of a low-level waste compact.

Disposal Compact," California Assembly Bill 4269, as requested by the California Department of Health. Close adherence to the Low-Level Radioactive Waste Policy Amendments Act of 1985 was the major recommendation in the staff response. Such confonnance would assure that the existing scheme for Federal and State regulation of low-level radioactive waste would remain intact and help to avoid confusion over the definition of low-level radioactive waste.
