Gentlemen:

Please renew U.S. Nuclear Regulatory Commission special nuclear materials license SNM-1531, which authorizes Shadyside Ilospital to implant plutonium powered cardiac pacemakers in humans for clinical evaluation purposes.

The Chief Physician responsibic is Emir A. Zikria, M.D.

We request the following possession limits for the following listed manufacturer and model all for human implantation for clinical evaluation.

Plutonium (principal radionuclide Pu-238) as scaled sources.

A.

Arco nuclear Model Nu-5:

10 grams (20 individual sources not to exceed 500 milligrams each).

The hospital agrees to follow the protocol associated with the various manufacturer as approved by and on file with the Commission.

The Ilospital further agrees to continue the following, reporting and recovery procedures during the life of the patient until the pacemaker is recovered and returned l

to the manufacturer even in the event that the physicians involved are no longer associated with the llospital.
