Your current license authorizes the use of the Arco Nuclear Model Nu-5, the Coratomic Model C-101 and the Cordis Nuclear Omni-Stanicor Model 184A.
Please clarify if you have ever used or plan to use
- j either the Coratomic or the Cordis nuclear pacemakers and whether you wish j
these nuclear pacemakers to be included in your license.
