HIGHLIGHTS OF MEEKLY INFORMATION REPORT WEEK ENDINS MAY 6, 1988 a

Waterford Steam Electric Station, Unit 3 i

On May 1, 1988 during an Inservice Inspection of the Waterford 3 reactor vessel nozzles, Westinghouse discovered a midwall, subsurface indication approximately 4 inches long by 1 inch deep in an 8 inch thick nozzle section.

The nozzle is on the hot leg which leads to steam generator number 1.

The licensee believes the indication to be weld slag. An indication of this size will require a fracture mechanics analysis by the licensee and approval by the NRC before restart, currently scheduled for early June. NRR and Region IV l

are continuing discussions with the licensee regarding an accurate determination of the indication size.

The reactor vessel and nozzles had been examined with acceptable codes and methods during manufacture, however, es the testing equipment and methods have improved, indications of this type have been found and successfully resolved at a number of other plants.

Veterans Administration Training Presentation On May 3, 1988, NRC representatives from NMSS, OE and Regions I and !!! held a meeting with Veterans Administration Regional Industrial Hygienists at Fort Root VAMC, North Little Rock, Arkansas. The purpose of the day-long presentation was to inform the VA about NRC's medical use program, policy, and regulations. A portion of the presentation was dedicated to developing an internal audit program for medical use of byproduct material. The presentation was requested by the VA, in an effort to improve the quality of Radiation Safety Program, and was attended by approximately 15 individuals representing the Veterans Administration's Regional Industrial Hygiene Program, j

Interacency Briefing on US-Soviet Safety Cooperation On Tuesday GPA participated with the Departments of State and Energy in a i

briefing of the Interagency Coordinating Comittee for US-Soviet Affairs (ICCUSA) on the recent meetings with the visiting Soviet nuclear delegation and l

the Memorandum of Cooperation signed on April 26. The ICCUSA forum, of which GPA is a regul:r participant, facilitates interagency discussion and review of

US government cooperative programs with the Soviet Union.

i Meeting with Governor Kunin Thomas Murley, Director, NRR; William Russell, Regional Administrator; Marie Miller, Regional State Liaison Officer; and John MacDonald, Vermont Yankee Resident Inspector, met on May 3 with Governor Madeleine Kunin of Vennont to l

discuss NRC activities regarding Yemont Yankee. The discussion centered on Mark I containment issues and NRC's draft schedule for recomending improvements to these containments.

MILLSTONE UNIT 2 On May 6, 1988, the licensee is expected to shutdown Millstone Unit 2 to investigate the build-up of boric acid on the Control Element Drive Mechanism (CEDM) Coolers. The CEDM coolers are located above the reactor vessel head.

At the present time, the unidentified RCS leakage rate is approximately.7 gpm compared to a Technical Specification limit of 1.0 gpm.

It is not known if there is a connection between the unidentified leakage and the boric acid on the CEDM coolers.

Geological Survey From April 26-28, 1988, members of the Division of High-Level Waste Management (HLWM) staff perfomed an "observation audit" of Department of Energy / Waste Management Project Office (DOE /WMP0) audit 88-03 of the U. S.

The purpose of the observation audit was to determine if DOE is meeting the NRC's quality assurance (QA) requirements. The HLWM staff will issue a sumary report which describes its observatior.s.

In short, the HLWM staff believes that the effectiveness and scope did not meet the standards necessary to allow them to gain the confidence needed to ensure that the QA program is meeting the Examples of concerns raised by the staff include:

Major issues pertaining)to the USGS programexpand identified by the staff were the need for the USGS to (1 and indoctrination program; (2) perform more internal audits; and (3) increase the number of QA staff devoted to its Menlo Park Office. At present, these NMSS staff concerns are preliminary, and will be further invectigated at the DOE /WMP0 USGS audit in Denver, Colorado, scheduled for the week of May 23, 1988.

SAFEGUARDS AND TRANSPORTATION Domestic Licensee Safeguards Event Log Data Following SGTR's presentation of preliminary observations regarding the analysis of the first quarter log inputs at the Region I Physical Protection Seminar on April 21, 1988, staff has received several inquiries from licensoes and regional staff. Positive feedback has been received from several licensees regarding preliminary analysis of log data and its pertinence to their facilities. Additionally, Regions II and III requested copies of briefing

materials and handouts used at the Region I seminar for presentation to their respective licensees. The staff is now receiving the first quarter 1988 logs (Jan. - March). To date 32 logs have been received and event data is being i

entered into the database system for analysis.

Transportation l

1 Transuranic Waste Package DOE is developing, under contract, a package designated TRUPACT II for transporting contact-handled transuranic waste from DOE facilities to the Waste Isolation Pilot Plant (WIPP). TRUPACT II is to be reviewed by the NRC.

On April 28, 1988, SGTB staff met with 00E, Westinghouse, Nuclear Packaging and Sandia Laboratory representatives to discuss pre-application efforts on design of the TRUPACT II.

Full scale physical testing of a prototype TRUPACT II, including thirty-foot free drop, puncture and fire tests, is tentatively scheduled to be conducted at Sandia Laboratories the week of May 20.

Depending i

on the outcome of the tests, an application for NRC review would be submitted by mid-June. DOE would like to have the NRC review prior to the opening of WIPP on October 1, 1988.

International Joint NRC/ DOE Physical Protection Team In-Country Visit To Spain, Portugal, and Italy From April 18-29, 1988, a physical protection specialist from the Division of Safeguards and Transportation, NMSS, and a policy analyst from International Programs, GPA, completed a joint NRC/ DOE Physical Protection Team visit at nuclear installations in Italy, Spain and Portugal. The purpose of the visits was to obtain information on physical protection programs for nuclear facilities and materials to support NRC export licensing actions pursuant to 10 CFR 110.43 j

and the Nuclear No-Proliferation Act of 1978. After meetings with foreign government representatives, the team visited the following nuclear facilities:

3 Casaccia Energy Research Center, and the IFEC fresh fuel fabrication pilot plant and the ERUEX pilot reprocessing plat at the Saluggia Nuclear Center; in Spain, the Centro Nacional de Energia Nuclear Juan Vigon research reactor; and in Portugal, the Laboratorio nacional de Engenharia e Tecnologia Industrial (LNETI) research reactor at Sacavem. During the meetings and site visits, physical protection policies, programs, implementation, and technical issues were discussed.

INDUSTRIAL AND MEDICAL NUCLEAR SAFETY FUEL CYCLE SAFETY Fuel Cycle Safety Kerr-McGee Rare Earths Facility, West Chicago, IL On May 3, 1988, IMNS and 0GC staff members met with representatives from Argonne National Laboratory who are preparing the final version of the waste disposal supplement to the FES for the Rare Earths Facility.

Progress on the supplement and problems ANL has encountered were discussed. The supplement is scheduled for issuance in August 1988.

Dry Spent Fuel Storage On May 4, 1988, NRC staff met with Duke Power Company and NUTECH, Inc., to discuss Duke's application, under 10 CFR Part 72, for dry concrete module and stainless steel canister system storage at its Oconee Nuclear Station site.

The Topical Report for the modular system design has been submitted separately by NUTECH, and referenced by Duke in its application.

Initial staff coments on the Topical Report will be issued by the end of May, with initial comments on the Duke environmental report to follow by the end of June. A site visit to Oconee is planned for late June or early July.

Reactor technical specification changes associated with removal of fuel to dry storage will be handled separately through an amendment application filed with NRR.

4 LOW-LEVEL WASTE MANAGEMENT Low-level Waste Meeting with EPA On Tuesday, May 3, 1988, members of the NMSS staff met with EPA at Waterside Mall, Washington, DC.

This was a regularly scheduled Interface Council Meeting.

Information was exchanged on a number of topics including NARM, earthquake planning, mixed waste, and the status of EPA standards on both HLW and LLW. The next meeting is scheduled for July 26.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending May 6, 1988 PRIMARY SYSTEM INTEGRITY / SEISMIC EFFECTS Tests on Seismic Vibration Table at Tadotsu Engineering Laboratory As a result of an agreement between the NRC and the Agency of Natural Resources and Energy (ANRE) of the Japanese Ministry for International Trade &

Industry (MITI), tests have been completed on the seismic vibration table at the Tadotsu Engineering Laboratory in Japan. The tests involved increasing the excitation within the limits of the vibration table in an attempt to induce inelastic response in a reactor coolant system piping model. The model was constructed by modifying the 1/2.5 scale model of one loop of a PWR primary coolant system used in a previous Japanese test.

The objective of the test was to provide additional data on the behavior near failure of piping systems subjected to earthquake-like loading. Testing began on March 31 and was completed on April 27, 1988. After a number of lower level tests, crack initiation was detected following the second application of the maximum test input excitation. Crack growth was carefully monitored during the next two additional test runs. Based on these test results, it was predicted that a failure could possibly occur during the next application of the test input load. This test resulted in a crack depth of 30m in an area where the original thickness of the hot-leg pipe is about 32m. At this point, testing was terminated since it was clear that a through-wall crack would occur during the next application of the test input load.. Further evaluation of the test results are continuing.

Items of Interest Office for Analysis and Evaluation of Operational Data Week Ending May 6, 1988 AE00 Support for IAEA Training Course On May 16, AE0D provided support to the IAEA course on "Safety in Nuclear Power Plant Operations" held at Argonne National Laboratory. Lectures by the Deputy Director, AE00 covered recent U.S. Transients, the U.S. program for the collection, assessment and feedback of operational experience, and pertinent observations and lessons learned from studies of U.S. and foreign operating experience. (Note that there is no expiration date for the use of such devices.)

It was agreed that AE00 would develop a position paper on how to approach the subject of aging.

Some aspects that will have to be considered are the effects of wear on the safe use of the device, and the effect of aging on materials (plastics) used in the devices.

RES, NRR, NMSS and AE00 could all be expected to make presentations. There will be a dry run for the EDO on May 19.

IRS Reportino During April, eight reports on operating experience at U.S. plants were sent to the OECD Nuclear Energy Agency's Incident Reporting System (IRS). The topics addressed were: (1) an update to IRS-811 on the reactor trip breaker (Westinghouse Model 05-416) failure at McGuire Unit 2, (2) a generic report on defects in Westinghouse circuit breakers, (3) an cccurrence of a safety cell switches, (5) fires in annunciator control cabinets (ype circuit breaker injection pipe cracking, (4) failures of Westinghouse W-2 t6) inadequate qualification and documentation of fire barrier penetration seals, ENCLOSURE E MAY 6, 1988

. d (7) mechanical failure of indicating type fuses, and (8) reduced reliability of steam-driven auxiliary feedwater pumps caused by instability of Woodward PG-PL governor. The infonnation provided was developed from feedback to the U.S.

industry.

j Diagnostic Evaluation and Incident Investigation On May 2, the NRC Team evaluating the Independent Management Appraisal (IMA) of Turkey Point and Florida Power and Light (FP&L) met with the ENERCON Team Manager and two of the ENERCON Team Members, including their behavior specialists, as part of the team effort to evaluate the scope, objectives, process, and results of the IMA of Turkey Point and FP&L.

During the week of J

May 2 additional IMA documents were received and reviewed by the Team. On May 5, the team met with the Director, AE0D, to discuss final preparations and plans for the Team visit to the ENERCON offices in Atlanta for indepth face-to-face discussions with the IMA team counterparts and data reviews. The Director, AE0D, discussed the background, purpose, goals, and objectives for the NRC team evaluation, including evaluating the need for a diagnostic evaluation of FPAL and Turkey Point.

Incident Response Branch On May 5, an IRB staff member gave a course at the Federal Emergency Management Agency's (FEMA) Emergency Management Institute in Ennitsburg, Maryland, to State and local offices on appropriate protective actions in response to severe nuclear plant accidents. This course is part of an ongoing series of courses presented by FEMA to State and local officials that are supported by IRB that l

ITEMS OF INTEREST OFFICE OF GOVERNMENTAL AND PUBLIC AFFAIRS WEEK ENDING MAY 6, 1988 INTERNATIONAL PROGRAMS Foreign Visitors On Mon 6y a senior management / technical team from the Chinese National Nuclear Safety Administration (NNSA) was welcomed to NRC by Commissioner Roberts and Mr. Denton and then met with staff from NRR.

China is constructing its first nuclear power reactor (300 MWe PWR) that is scheduled to connect to the grid in the winter of 1990. The NNSA is completing its final licensing review of the reactor and will meet with the NRC technical staff for the next three weeks to discuss licensing issues that exist at their plant.

On Monday the Belgian technical team (which attended the signing ceremony),

consisting of F. Motte (CEN/SCK), H. Dresse (Intercom), H. Dopchie (Assoc.

Vincotte) and J. P. Poncelet (Ministry of Economic Affairs), visited NRC. The visitors wanted to learn more about NRC's position on hydrogen control in PWRs i

with large dry containments and filtered vented containments.

NRR and J. Hulman of RES participated in the discussions. The Belgian team i

went on to site visits and discussions at ORNL and Sandia.

On Tuesday P. Sagnon and R. MacLellan, members of the Canadian Parliament, visited NRC to gather information on NRC's regulatory system and the limits of the Price-Anderson liability fund as regards impacts abroad of US nuclear accidents.

H. Denton, GPA, and S. Treby and J. Scinto, OGC, briefed the visitors.

On Wednesday Prof. B. McHugh, Institute of Technology, Goeteborg, Sweden, met with staff members of AEOD, NRR and RES to talk about various Human Factors issues, including crisis management and artificial intelligence / expert systems in reactor control rooms.

Interagency Briefing on US-Soviet Safety Cooperation On Tuesday GPA participated with the Departments of State and Energy in a briefing of the Interagency Coordinating Comittee for US-Soviet Affairs (ICCVSA) on the recent meetings with the visiting Soviet nuclear delegation and the Memorandum of Cooperation signed on April 26. The ICCUSA forum, of which GPA is a regular participant, facilitates interagency discussion and review of US government cooperative programs with the Soviet Union.

Belgian Agreement Signing On Monday, in a ceremony at the Ambassador's official residence, Chairman Zech signed a five-year renewal of the NRC-Belgian Government information exchange and cooperation arrangement with Belgian Ambassador to the U.S. Herman Dehennin. Also attending from NRC were E. Beckjord, J. Cortez and C. Serpan of RES; J. Shea, R. Hauber, D. Chaney, and H. Schechter of GPA/IP; A. Vietti-Cook and J. Dunn-Lee of the Chairman's office; and D. Chairman Zech and other Comissioners and join for thru lunch lunch.

Aug. 1-5 Shi Guangchang, Executive Deputy Director General, PRC NNSA, will meet with the Comissioners, followed by appointments with NRR, RES, AE0D and NMSS from 8/1-8/5. Shi will also visit a Regional Office, two labs (possibly BNL and SNL), INP0 and General Atomics before returning to China on 8/19.

l STATE, LOCAL AND INDIAN TRIBE PROGRAMS Meeting with Governor Kunin Thomas Murley, Director, NRR; William Russell, Regional Administrator; Marie Miller, Regional State liaison Officer; and John MacDonald, Vermont Yankee Resident Inspector, met on May 3 with Governor Madeleine Kunin of Vermont to discuss NRC activities regarding Vermont Yankee. The discussion centered on Mark I containment issues and NRC's draft schedule for recomending improvements to these containments.

Week Ending May 6, 1988 The following Significant Enforcement Actions were taken the week of May 2,1988.

A Notice of Viulation and Proposed Imposition of Civil Penalty in the amount of $150,000 was issued to Commonwealth Edison Company (Oresden, Units 2 & 3).

The action was based on violations of equipment qualification requirements of 10 CFR 50.49. The Comission approved this action on April 27, 1988.

(EN 88-32) 1 An Order Imposing a Civil Penalty in the amount of $6,000 was issued to St. Louis University, St. Louis, Missouri.

The action was based on (1) a whole 4

body exposure of at least 7.5 rems during the fourth quarter of 1987, (2) failing to assess radiation hazards or observe three warning lights which J

could have alerted the individuals, prior to entering the room, that the 8900 curie cobalt-60 teletherapy source was in an exposed position, and (3) failing to report to the NRC within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> that an exposure event had occurred.

The licensee responded to the Notice, which was issued January 29, 1988, on February 16, 1988. After considering the response, the staff concluded that a sufficient basis was not provided for reduction of the civil penalty amount.

A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $112,500 was issued on May 4, 1988 to Omaha Public Power District (Ft.

CalhounStation). The action was based on a number of procedural violations in the area of radiation protection including the failure to control a very high radiation area which is a repeat violation. The violation involving failure to secure the door for a vary high radiation area was increased by 25%.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued on May 4, 1988 to Florida Power Corporation (Crystal River, Unit 3). The action involves the failure to take appropriate corrective action to resolve a deficiency regarding the electrical loads placed on one of the two plant emergency diesel generators.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued to Carolina Power and Light Company (Grunswick Units 1 and 2). The action was based on violations of equipment qualification requirements of 10 CFR 50.49.

Civil Penalties Paid Public Service Electric and Gas Company (Salem Unit 2) paid the proposed civil penalty in the amount of $50,000. The action was based on several examples of the failure to satisfy fire protection requirements, including the separation of redundant trains of equipment, cabling, and associated circuits necessary to achieve and maintain hot shutdown in the event of a fire.

Micromedic Systems, Iiic., Horsham, Pennsylvania paid the proposed civil penalty in the amount of $500. The action was based on improper disposal of radioactive waste materials and three examples of the failure to perform adequate surveys as required at the facility.

Arkansas Power and Light Company (Arkansas Nuclear One Unit 1) paid the proposed civil penalty in the amount of $100,000. The action was based on a failure to take carrective action for a longstanding problem of containment building temperatures in excess of those assumed in the plant design basis.

On May 6, Houston Lighting and Power representative reported that data reduction is complete on 15 of 58 bottom mounted instrument thimble tubes.

j No indication of waar was indicated on these thimble tubes. All 58 thimble tubes have been tested. Of the remaining thimble tubes, two have indications in the flow limiter area that are being evaluated to determine if actual wear is present.
