Subject: NRC Region I Inspection Report No. 50-293/88-07

This refers to the routine safety inspection (50-293/88-07) conducted by

Messrs. C. Warren, J. Lyash and T. Kim of this office on January 20,1988 to

March 5,1988, at the Pilgrim Nuclear Power Station, Plymouth, Massachusetts.

Areas examined. during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter.

In regard to the control of overtime at' Pilgrim Station, the inspection con-

firmed that -your program, as currently documented, meets the guidelines

promulgated in Generic Letters 82-12 and 83-14. However, program implementa-

tion had been such that it did not appear to have been fully meeting the intent

of NRC guidelines. As documented in Section 5, we understand that you have

agreed to supplement the response to this item provided in your October 28, 1987

letter. Please provide this supplemental response within 30 days' of the date

of this letter.

Based on the results of this inspection, it appears that certain maintenance

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation enclosed herewith as Appendix A, Failure to

establish adequate procedures and to . perform adequate technical review were

evident during the blackout diesel generator testing and the plant process

computer point tie-in activities. In addition, ' poor preplanning and control-

of maintenance resulted in an unnecessary engineered safety feature actuations

during an electrical relay replacement. Weaknesses in control of maintenance

was the subject of a previous violation as documented in the inspection report

50-293/87-50.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
