SER SUPPLEMENT TO SER INPUT FOR SEQUOYAH CATEGORY QUALITY ASSURANCE ELEMENT REPORTS

REFERENCE:

Memorandum from J. Zwolinski to B. D. Liaw dated October 22,1987, "SER Input for Sequoyah C6tegory Quality Assurance Element Reports" The referenced SER addressed the staff evaluation of the following 15 OA related Element Reports (ER):

80000, 80101, 80102, 80104, 80201, 80202, 80203, 80205, 80303, 80401, 80402, 80404, 80501, 80503 and 80504 This supplement provides an updated status of those ERs which were identified as carrying open issues.

The evaluation of 6 ERs, 80000, 80102, 80104, 80205, 80401 and 80404, remain complete and acceptable as previously stated. Therefore, there are no open issues pertaining to these ERs.

The status of the open issues pertaining to the remaining 9 ERs follows:

1.

ER 80101 - CATO 80101-SQN-01 addresses pre-restart and post-restart activities pertaining to the rereview and evaluation of procurement packages and hardware to detennine if adequate controls and dedication have been applied.

This activity is complete for the restart items, however, for post-restart items cngoing evaluations continue.

The staff finds these actions acceptable and sufficient to support resta"t.

TVA needs to close out the post-restart actions.

ER 80201 -

(a) CATDs 80201-SQN-02 and 03 address actions to improve installation procedures and training methods associated with instrument tube fitting.

These two issues remain open in that TVA needs to document in a memo that the actions are completed and have been verified by the ECTG.

These are not restart issues.

(b)

The generic evaluation of the adequacy of compression fitting installation has been adequately evaluated and documented in Revision t to ER 80201.

This issue is closed.

ER 80202 - TVA has adequately addressed the evaluation and acceptability nf conduit bends in Revision 5 to the Element Report.

TVA needs to document in a memo that this CATD has been verified and closed out by the EC'G.

This is not a restart issue.

EC002b3-CAQRNo. 870565 has been adequately resolved.

The vendor 4

welds on the C.W. Heat Exchanger has been evalu ted through the Independent Design Inspection activity ard has been found to be acceptable.

This action is sufficient to support restart. As a post-restart action TVA needs to address the close-out of this CAQR.

5.

ER 80303 - CATO's 80303-SON-01, 02 and 03 addresses actions which are not restart issues.

These three issues remain open and again are not restart items.

ER 80402 - CATD 80402-SQN-01 was issued describing action to develop instructions on the purpose and use of the observation log.

This action has been completed except for verification and closecut of the CATO.

This issue remains open but is not a restart item 7.

ER 80501 - CATO 80501-SQN-01 was issued descrit,ing pre-restart and post-restart actions.

The pre-restart actions have been completed.as '

confirmed through TVA's letter of January 26,198F.

This action is sufficient to support restart.

Post restart actiJn remains open.

ER 80503 - CATD's P0503-SON-01. -02, -03, -04, -05

-07, and -08 describe actions that are still ongoing.

These actions are not considered restart issues since they are programmatic in nature.

The DBVP, IDI and the overall actions underway to satisfy the NPP comitments give sufficient confidence of the as-built configuration and design documentation to adeouately support restart.

These issues remain open but again are not restart items.

ER 80504 - CAR No. SQ-CAR 86-01-004 and CATD's 80503-SON-05 and -07 were issued to track and correct inconsistencies in vendor drawings and manuals to as-built conditions and to correct cregram and procedure deficiencies.

These issues renain open but are not restart items.

Ir summary, the staff finds the TVA actions to satisfactorily close-out the restart is hes pertaining to the above ER's acceptable and sufficient to supoort restart.
