Dear Mr. Miraglia:
Attached is information which addresses your staff's concerns re-garding supports for several B2 TDI diesel generator components and the B2 diesel exhaust shroud.
During a 24-hour endurance run of the B2 TDI diesel generator, cracks developed in an original turbocharger restraint bracket and air start system piping supports.
Additionally, seepage of jack-et cooling water occurred from the exhaust shroud.
(Most of the seepage was attributable to leaking fittings and Dresser coup-ling.)
Attachment I contains information about the turbocharger bracket.
Attachment II addresses the air start system piping supports.
Attachment III supplies information about the exhaust shroud.
When the corrective actions outlined in the attachments have been accomplished, the District will perform another 24-hour endurance run on the B2 TDI diesel generator.
This endurance run will be completed prior to restart.

MAR 21 1988 Frank J. Miraglia, Jr. GCA 88-193 Following restart, the District will conduct a long-term monitor-ing and evaluation program on both the A2 and B2 TDI diesel gen-erators.
This program was committed to in letter GCA 88-032, dated February 1, 1988, and directs the monitoring of affected equipment on a 90-day interval.
The program will ensure that an unanticipated increase in vibration levels will be detected in a timely manner.
This monitoring will be performed in addition to the surveillance testing required by Technical Specifications.
A long-term study was also committed to in the referenced letter.
This study will evaluate the entire design of the TDI diesel piping and auxiliary skid arrangement to determine if an alter-nate design is more advantageous.
This study is considered a long-range item and will be conducted after Rancho Seco restart, but priot-to the next refueling outage.
It is the District's intent, using the results obtained from the long-term study, to modify the piping, restraints, support con-figuration, and/or change material on the A2 and B2 TDI diesel generators to meet the requirements of ANSI / ASME OM3-1986, if approved.
Shculd the 1986 version of the code not be approved, then the latest draft sha:1 be used as criteria.
These mod-ifications will be completed prior to startup from the next refueling outage.

DEFICIENCY The original support bracket was installed by TDI.
Subsequently, additional supports were installed on the recommendation of Duke Power Company.
See Figure 1.
After completion of a 24-hour endurance run, a small crack was discovered in the original TDI turbocharger support bracket base support.
See Figure 2 (photographs) and NCR S-7880.
II.
EVALUATION OF FAILURE Investigations concluded that the original component was designed with a high stress intensity factor and very low allowances for torsional load.
Based on measured displacement data, equivalent loads were calculated.
These equivalent loads were then applied to each support and the stresses calculated at the failure loca-tion.
The calculations show that stresses at each failure loca-tion were above the endurance limit and that the failure was caused by the cyclic displacement of the turbocharger.
III. CORRECTIVE ACTION Vibration measurements were taken which confirm the acceptability of removing this support bracket.
These calculations show that vibration criteria would be met without the use of the support; therefore, the support is acceptable in its existing configura-tion.
There was a marginal increase in calculated vibration without the support; therefore, the DL trict will replace the bracket with a design which will accept 9: eater torsional loading.
A 24-hour endurance run will be performed on the B2 TDI diesel prior to criticality.
During this run, detailed vibration monitoring of the replaced supports will be performed.

Purcose of Desion Chance Tne purpose of tnis design change is to prevent recurrence of tne failures excerienced on the "A" TDI diesel curing tne 24 nour run completed on 3/17/88.
In addition to Ine deficiencies icentified following tne 24 nour run, a modification is being made to tne pneumatic control system to prevent a lack of shutdown signal reset after a manual test of tne overspeed trip funtion.
Inis pneumatic control system problem was excerienced on tne "A" TDI ciesel and is documented on NCR S-7841.
Tne following deficiencies were documented following tne 24 nour run.
O A crack appeared in the support bracket for tne starting air solenoid valves (NCR S-7880).
O A small crack developed in the turbocnarger support bracket (NCR S-7880).
4 A small quantity of jacket water was discovered to be seaping from tr.<! exnaust shroud (NCR 7881).
O Cracks were found on two additional supports on tne starting air header, 10110-3"-xx (NCR 57894).
Shroud Modification
~
To prevent future leakage of the jacket water system, the entire exhaust shroud will be removed.
Tne jacket water supply and return lines will be capped.
Tne exnaust pipes will be insulated with 2" of fiberglass insulation to reduce the neat load in the building.
Succort >'caifications Jacket Water System - Four new brackets will be added to support the 4" jacket water neader whicn was previously supported from tne exnaust snroud. A new support will be added to the 6" jacket water pipe to prevent vibration which is 1800 out of pnase with the east / west motion of the top of the engine.
Turbocharger Support - Tne turbocnarger support bracket wnicn cracked will be replaced and stiffner plates will be added for additional strengtn.
Starting Air Supports - Tne support bracket for tne starting air solenoid valves will be redesigned.
Five supports on the starting air header which runs the lengtn of the engine will also be redesigned to prevent future failure.

Calculations nave been completed to demonstrate tnat tne additional neat load into the building is acceptable.
Tne insulation being added will provide adequate personnel protection.
Support Modifications Calculations nave been completed to demonstrate that tne jacket water and starting air system piping meet thermal, weight, and l
seismic design requirements based on tne new support configuration.

I.
DEFICIENCY After completion of a 24-hour endurance run, a large crack was discovered in the support bracket for the start air lines near the starting air solenoid valves.
See Figure 1 and NCR S-7880.
Additionally, cracks were discovered in two other air start sys-tem line supports.
See Figures 2 and 3 and NCR S-7894.
II.
EVALUATION OF FAILURE Investigations concluded that the original component was designed with a high stress intensity factor.
Based on measured displace-ment data, equivalent loads were calculated.
These equivalent loads were then applied te each support and the stresses calcula-ted at the failure location.
The calculations show that stres-ses at each failure location were above the endurance limit and that the failure was caused by the cyclic displacement of the piping.
III. CORRECTIVE ACTION Vibration measurements were taken which confirm the acceptability of these support brackets in their as-found condition.
Stress levels were within allowable limits.
The supports are acceptable in their existing configuration for thermal, dead weight, and seismic design requirements.
There was a marginal increase in calculated vibration without the supports; therefore, the District will replace the supports.
A 24-hour endurance run will be performed on the B2 TDI diesel prior to criticality.
During this run, detailed vibration non-itoring of the replaced supports will be performed.

Purcose of Design Cnance Tne purpose of tnis design change is to prevent recurrence of tne failures experienced on tne "A" TDI diesel during tne 24 nour run completed on 3/17/88.
In addition to tne deficiencies icentified following tne 24 nour run, a modification is being nace to tne pneumatic control system to prevent a lack of snutcown signal reset after a manual test cf tne oversceed trip funtion.
Inis pneumatic control system cronlem was excerienced on tne "A" TDI ciesel and is documented on NCR S-7841.
Tne following deficiencies were documented following tne 24 nour run.
O A crack appeared in the support bracket for tne starting air solenoid valves (NCR S-7880).
O A small crack developed in the turbocnarger support bracket (NCR S-7880).
O A small quantity of jacket water was discovered to be seaping from the exhaust shroud (NCR 7881).
O Cracks were found on two additional supports on the starting air neader, 10110-3"-xx (NCR S7894).

Tne jacket water supply and return lines will be capped.
Tne exnaust pipes will be insulated with 2" of fiberglass insulation to reduce the neat load in tne building.
Support Focifications Jacket Water System - Four new brackets will be added to support the 4" jacket water neader wnicn was previously supported from tne exnaust shroud.
A new support will be added to the 6" jacket water pipe to prevent vibration wnich is 1800 out of pnase with the east / west motion of the top of the engine.
Turbocharger Support - Tne turbocnarger support bracket wnich cracked will be replaced and stiffner plates will be added for additional strength.
Starting Air Supports - Tne support bracket for tne starting air solenoid valves will be redesigned.
Five supports on the starting f
air header wnich runs the lengtn of the engine will also be redesigned to prevent future failure.

Pneumatic Control Modifications The control system logic will De verified oy performing procedure TEMP-004 and SP-57A. A leak tigntness of tne tubing and fittings will be performed at an internal pressure of 60 psig.

DEFICIENCY During the performance of a 24-hour endurance run, seepage was observed from the exhaust shroud.
The seepage was discovered at the stiffeners located between cylinders 2 & 3 and 3 & 4.
No cracks in the shroud could be found.
Leakage was also observed at several mechanical joints (swagelock fittings and Dresser coupling).
See NCR S-7881.
In a previous 24-hour run, cracks were found in the heat-affected zones which were repaired and subsequently examined.
The shroud was hydrostatically tested for pressure-retaining capacity.
II.
EVALUATION OF FAILURE Total leakage was observed to be approximately 1.5 gal / hour.
This is considered minimal and within makeup capability.
III. CORRECTIVE ACTION To provide added conservatism on vibration issues with respect to the exhaust shroud, the B2 TDI exhaust shroud will be removed and the jacket cooling water to the shroud blanked off.
The exposed exhaust header will be insulated and supports installed on the 4-inch jacket water return header.
A 24-hour endurance run will be performed on the B2 TDI diesel prior to criticality.
During this run, detailed vibration mon-itoring of the replaced supports will be performed.
