DESCRIPTION I
This modif.
.a.
erformed the following:
1 A.
Additv of valves I.lCV8114 and ICV 8116) and interlocks at the centrifugal charging pumps' ainiflow lines to provide charging flowpaths during high reactor coolant system pressure conditions, preventing charging pump deadheading. This includes the installation of the necess'ary reector coolant system wide range pressure instrumentation.
B.
Addition of/ Main Ccotrol Room indication and alarms for seal water heat exchanger tube side temperature and shall side flow.
C.
Replacement of the reactor coolant wide range pressure instrumentation which is interlocked with the pressuriser power operated relief valves, required for cold overpressure protection.
SAFEIT EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated-in the Final Safety Analysis Report (FSAR) is not increased by this modification because the Chemical and Volume Control System (CV) system will be operated as presently described in the FSAR. The new valves increase pump reliability by providing CV flowpath during high reactor coolant pressure conditions and prevents pump deadbeading. The pressuriser Power Operated Relief Valve (PORV) logic is not changed from its description in the FSAR. Existing pressure transmitters have been replaced with comparable safety related transmitters.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the operation of the CV system is not being changed from that as described in the FSAR. The miniflow line does not close unless a low pressure signal is received from the reactor coolant transmitters in conjunction with a safety injection signal. The new CV valves provide the redundancy
(
required for isolating the miniflow line when applicable. The safety related function of the pressuriser PORV's is not changed.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because no unreviewed safety questions esist. The CV system continues to be operated in accordance with the existing Technical Specification. The additions increase the reliability of the CV pumps, increasing the margin of safety afforded by the system.
Pressuriser PORY operation is not changed relative to the pre %nt Technical Specification. (1884M/0215M)

MODIFICATION M6-0-85-003 DESCRIPTION This modification replaced 54" butterfly dampers with 54" bubble-tight dampers on the Control Room ventilation system. The dampers are part of the smoke removal system used to purge the Control Room with maximum outside air and exhaust smoke to the Turbine Building following extinguishment of a fire. The bubble-tight dampers were installed to prevent unfiltered in-leakage to the Control Room.
SAFFIY EVALUATIN
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the bubble-tight dampers prevent unfiltered in-leakage of potentially contaminated air to the Control Room ventilation system. The zero leakage dampers enable the VC system to operate to maintain habitability in accordance with FSAR assumptions. The purge dampers can be opened to remove smoke from the Control Room following extinguishment of a fire within the Control Room envelope.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the bubble-tight purge dampers operate in accordance with the design needed to satisfy the requirements of the FSAR. The zero leakage dampers prevent unfiltered in-leakage when closed and provide smoke removal capability when opened.
3.
The margin of safety, as defined in the basis for any Technical l
Specification is not reduced because the normally closed dampers are j
tested to fail CLOSED on receipt of a High Radiation MCR Intake test signal to ensure isolation of the Control Room.

MODIFICATIOM M6-1-05-049 DESCRIPTION This modification was installed to replace the Pressurizer Power Operated Relief Valve (PORV) Actuator Nitrogen Supply System. The high pressure nitrogen supply system and accumulators were replaced with a low pressure instrument air supply and accumulators.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence 'of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the new instrument air supply system as installed is safety related and seismic related.
This installation is from the supply side of the accumulators to the PORV control solenoids. Furthermore, the new system allows operation of the PORV's within their designed operating parameters. Also, the reactor coolant system and pressurizer safety valves were not modified.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the operation of the pressurizer and its PORV's is unchanged. The new instrument air system is safety related and seismic related. The PORV's continue to.
operate within their specified operating parameters.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because the pressurizer and the PORV's remain opsrable as defined in the Technical Specifications. The new instrument air system supplies a more reliable control system for the PORV's than did the previous nitrogen system.

MODIFICATICal M6-0-86-095 DESCRIPTION This modification revised the Process Radiation Main Control Room Ventilation System interlock logic from the current design of one out of two monitors for radiation detection, and one out of two monitors for monitor operational failure to one out of two monitors for radiation detection, and two out of two monitors for monitor operational failures. This minimized spurious actuations of the system.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the Main Control Room Ventilation System Makeup Fan and Dampers will automatically actuate during radiation detection from either monitor (1/2) on the ventilation trains. Automatic actuation of the Makeup Fan for monitor operational failures is not addressed and revising their logic to 2/2 will not affect any accident analysis.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the Main Control Roon Ventilation System Makeup Fan and Dampers will automatically actuate during radiation detection from either monitor (1/2) on the ventilation fans. Automatic actuation of the Makeup Fan and Dampers for monitor operational failures is not specifically addressed and revising the interlock logic to 2/2 monitors will not affect any accident analysis.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because Technical Specification 3.7.6 only l
requires automatic actuation of the Makeup Fan and Dampers from a Main Control Room Process Radiation Monitor high radiation signal. Automatic l
actuation of the Makeup Fan and associated equipment from monitor operational failures is not addressed in Technical Specifications 3.3.3.1 and 3.7.6.
Thus, implementation of this modification will not reduce the margin of safety per any Technical Specification. (1884M/0215M)
T MODIFICATION M6-0-86-123 DESCRIPTION This modification revised the Auxiliary Building Ventilation System (VA) supply and exhaust airflows to ensure adequate flows to all areas of the Auxiliary Building. The VA System was originally balanced to support Unit-1 operation only.
SAETTY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the VA System is now operating as originally designed, supporting all areas of the Auxiliary Building. To achieve this, ainor changes were made to ensure adequate airflows and proper airflow directions. No changes were made which affect the operation of any equipment important to safety as evaluated in the PSAR.
The possibility for an accident or malfunction of a different type than 2.
any previously evaluated in the FSAR is not created because this modification does not change the operation of the VA System as described in the FSAR, therefore, no unanalysed safety condition will be created._

.o MODIFICATION M6-0-86-181 DESCRIPTION This modification modifies the laterals in the resin separation vessel to accommodate a 3:1 Cation to Anion resin ratio. The work initiated raising the lateral to the new interface level created by the adjustment to the resin ratio.
SAIT1'Y EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the PSAR is not increased by this modification because the ulteration to the separation is only a mechanical change to reflect the new resin cation to anion interface level. No other equipment or function is affected.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the modification is to the internals of the resin separation tank. No additional equipment is added or safety equipment removed. The function of the equipment is enhanced because it will provide a better separation of cation and anion resin for regeneration.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because the internals of the resin separation vessel and the effectiveness of the resin separation does not affect any Technical Specification nor is it referenced by any Technical Specification.

MODIFICATION M6-1-86-166 DESCRIPTION This modification removed various pipe whip restraints and jet deflectors from tne Unit 1 Reactor Coolant System (RCS) piping inside containment. This modification was based on a letter from B. J. Youngblood (NRC) to D.L. Farrar (CECO) dccumenting NRC approval of the CECO request for exemption from a portion of General Design Criteria (GDC) #4. Appendix A, 10CFR50.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because a "leak before break" analysis has been approved by the NRC as an exemption from a portion of General Design Criterion (GDC) #4, Appendix A,10CFR50, which requires protective jet deflectors and whip restraints as described in the FSAR. The analysis demonstrates that the probability of pipe breaks in Primary Coolant Piping is sufficiently low to eliminate pipe breaks as a design criteria for the installation of deflectors and restraints. The conditions and ausumptions of other plant design bases as defined by the FSAR are not impacted.
2.
The possibility for an accident or malfunction of a different type than~
any previously evaluated in the FSAR is not created because this modification is based on the subject "leak before break" analysis, which demonstrates that small piping flaws will be detected through Inservice Inspection Programs or leakage inonitoring systems long before critical conditions which may result in large pipe breaks are created.
3.
The margin of safety, as defined in the basis for any Technical l
Specification is not reduced because this modification removes whip
(
restraints and jet deflectors which are not addressed in the Technical Specifications. The removal of jet deflectors and whip restraints does not impair the plant capability to be operated within the Technical Specification operating basis. Inservice Inspection and ALARA considerations will be improved with the absence of the restraints and deflectors. (1884M/0215M) l

MODIFICATION M6-1-86-206 DESCRIPTION This modification performed snubber reduction on 67 piping subsystems on Unit 1.
Three-hundred (300) snubbers were deleted and an additional 116 snubbers were replaced with struts.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the snubber reduction was performed using revised damping values defined in ASME Code Case N-411 for seismic loadings, as approved by the N E,.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because this modification included post-installation thermal monitoring during heat-up to verify the new design thermal movements are within design limits.
Seismic interaction walkdowns were performed to verify that the new design seismic movements did not aversely affect the piping or adjacent structures, components, and equipment.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because this modification reduces the number of snubbers in the plant. Existing safety margins for seismic and thermal piping movements are maintained, therefore, the margin of safety asstaned in Technical Specifications 3.4.10, Structural Integrity, and 3.7.8, Snubbers, is not affected. (1884M/0215M)
l-MODIFICATION M6-1-86-212 DESCRIPTION This modification deletes the Containment Spray Additive Tank Nitrogen Supply Check Valve, ICSO44. This deletion allows an adequate nitrogen blanket to be applied to the containment spray additive tank. previously, the minimum opening pressure of the check valve could not be overcome.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the removal of this check valve insures the nitrogen blanket is applied to the containment spray additive tank. The containment spray system continues to operate per design.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the removal of this check valve enhances the containment spray system. Its removal does not impair the containment spray system from performing its safety related function. Its removal ensures the ability to apply a nitrogen blanket to the containment spray system.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because it is unaffected. The containment spray system operability continues to be maintained per the Technical Specifications.

The possibility for an accident or malfunction of a different type than any previously evaluated in the PSAR is not created because no new hazards are created or introduced by this modification.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because the ability to contain a potential fire is enhanced by this modification.

The probability of an occurrence or the consequence of an accident, or malfunccion of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because addition of manual fire fighting capability can only serve to lessen the consequences of a fire.
2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because this equipment has no function until a fire occurs at which time it increases fire fighting capability.

.. f MODIFICATION M6-0-87-Af6 DESCRIPTION This modification installed filters on the Condsnsate Polisher (CP) regenerate discharge line to the flume. The filtern were installed to remove suspended solids from the waste streem to assure compliance with the plant NPDES perinit.
SAFETY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to reafety as previously evaluated in the FSAR is not increased by this modification because the new filters will only remove dirt and solids from the waste stream to produce a cleaner water for discharge. The improved waste stream will not affect the probability of an occurrence or any safety function as previously evaluated in the FSAR.
q 2.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because the filters affect only the CP regenerate waste stream. The new filters do not affect any safety related equipment or the safety function of any equipment.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because the filtration of the waste stream does not affect any Technical Specification.
It only produces a cleaner
(
waste stream to comply with the NPDES permit or any Technical Specifications that limits radiological release from the Ifquid waste streams of the plant.

.s MODIFICATION M6-2-ft7-ll7 DESCRIPTION This mydification eliminated the Reactor Trip from Turbine Trip below P-8 (30%). All required Technical Specifi:.:ation changes have been received.
SAFErY EVALUATION
SUMMARY
1.
The probability of an occurrence or the consequence of an accident, or malfunction of equipment important to safety as previously evaluated in the FSAR is not increased by this modification because the additional required steam dump capacity will be handled by the ducps since they are designed to handle 40% of plant capacity. Also, the FSAR Section 15 analyses do not assume the Turbine Trip functional for any analysis.
The possibility for an accident or malfunction of a different type than any previously evaluated in the FSAR is not created because this modification did not alter any plant equipment required by any safety analysis in the FSAR Section 15.
Those analyses assume no Turbine Trip on Reactor Trip or vice versa. OIAT Reactor Trips function in each case to maintain DNBR.
3.
The margin of safety, as defined in the basis for any Technical Specification is not reduced because this modification did not alter any equipment specified in the basis of any Tech Spec.
