Operations Summary January 1986 The following summary describes the signific^ ant operational activities -
for the month of January. In support of this summary, a chronological --
log of significant events is included in this report.
The units remained in an administrative shutdown the entire month due to documentation concerns relating to the environmental qualification of various electrical equipment (NUREG 0588). Outage related maintenance and modifications are being performed. Unit I has been off-line 162 dcys. Unit 2 has been off-line 163 days.

Fuel Performance Unit 1 The core average fuel expesure accumulated during January was 0 MWD /MTU with the total accumulated core average fuel exposure of 0 MWD /MTU.
Unit 2
- The core average fuel exposure accumulated during January was 0 MWD /MTU with the total accumulated core average fuel exposure of 8097.51 MWD /MTU.
Spent Fuel Pit Storage Capabilities The total storage capability in the spent fuel pit (SFP) is 1,386. However, there are five cell locations which are incapable of storing spent fuel. Four locations (A10, All, A24, A25,) are unavailable due to a suction strainer conflict and one location (A16) is unavailable due to an instrumentation con-flict. Presently, there are a total of 348 spent fuel bundles stored in the SFP. Thus, the remaining storage capacity is 1,033.
PORVs and Safety Valves Summary No PORVs or safety valves were challenged in January 1936.
Licensee Event Reports The following licensee event reports (LERs) was reported to the Nuclear Regulatory Commission in January 1986.
LER DESCRIPTION OF EVENT 1-85050 A containment ventilation isolation (CVI) occurred at 2350C, December 7,1985, due to loss of power to radiation monitor 1-RM-90-112A. It was determined that dirty connection pins were the probable cause.
1-85049 On December 6,1985 a review was made of the surveillance schedule, and it was found that the monthly functional' test for the waste gas hydrogen and oxygen analyzers was not performed for October 1985 within the required timeframe. The method of scheduling monthly and quarterly performances of this SI caused the error.
1-85051 On December 6, 1985, due to boron solidification and buildup in the drain line of the floor drain collector tank from the condensate demineralizer waste evaporator, water backed up into the area of the overflow drain. Health Physics declared the 2A-S auxiliary -.
feedwater (AFW) terry turbine room, 2A centrifugal changing pump room, unit two penetration room, and 669 pipe chase as contaminated areas with stringent dress out requirements for these areas. As a result, fire watches were not conducted for seven hours in AFW terry turbine room and for twelve hours in the centrifugal charging pump room, penetration room and pipe chase.

9 LER DESCRIPTION OF EVENT (cont.)
2-85012 With both units in mode 5, it was discovered that the reagent air for the unit two "A" train containment hydrogen analyzer was connected to a nonessential air source. It was declared inoperable --
at 1200 CST, December 11, 1985. The reagent air had been changed -
from essential air to a nonessential air source as part of a ~
modification performed during the unit two cycle two refueling j outage. ,

INSTRUMENT MAINTENANCE Unit 1 Completed refilling of reactor vessel level instrume.ntation system (RVLIS). -
upper plenum sense lines on both trains A.and B. The high point vents and -
reactor cavity sensor bellows fill tee caps were seal welded by Mechanical Maintenance to prevent air inleakage. All RVLIS and reactor coolant system (RCS) wide range pressure transmitters were calibrated after welding was pe rfo rmed .
A varped cover was found on Barton model 763 t ransmitter for RCS wide range pressure (1-PT-68-69). Received a memo from Festinghouse stating that covers could be interchanged on Barton model 763 and 764 transmitters. A cover from a spare transmitter was used to replace the defective cover and the I.D. tags were swapped for proper identification.
During performance of SI-94.1, pressurizer pressure transmitter 1-PT-68-340 was found out of calibration so as to cause the associated loop bistables to be out of Tech Spec allowable limits (PRO-1-86-009). The transmitter was recalibrated and returned to service.
UNIT 2 Completed workplan 11887 which finishes the CVI logic modification on unit 2.
However, during performance of this workplan, the train A RHR suction valve was inadvertantly isolated. PRO 2-86-007 was written on this event and it explains that removal of a fuse in the output bay of the SSPS cabinets caused a separation relay to deenergize which caused the RHR valve to isolate. The fuses will be tagged to alert personnel in the future to prevent reoccurrence.
Diodes were found shorted on GE-MAC power supply outputs in the Gould trans-mitter loops 2-F-3-147, 155, 163, 170 during initial EQ verification (Binder IFT-001). Six (6) diodes have been found defective to date. Investigation of this problem was performed and the probable cause is a metal clip in the rear of the cabinet that appears to have shorted across the outputs of the power supplies. Proposed solution is to fabricate and install a plastic shield over the rear connector. This work is being coordinated with 0.E. and is still ongoing.
COMMON Completed the field conversion of the strain gage connection from slip connectors to hard solder joints for Barton models 763 and 764 Lot 2 transmitters. This was required for environmental qualification and the work was performed by special maintenance instructions SMI-1-317-23 and 2-317-23.
Continued support for the environmental qualification program. Major work items involve verification of QMDS requirements and resolution of equipment deficiencies. Current status on EQ verification indicates approximately 62% com;lete on unit 1.and 64% complete on unit 2 and Common. ,

service. Some minor hanger work remains. An additional support in unit 1 valve room was identified as requiring modification to allow access to a motor operator. A workplan was prepared and is in the

ECN 6561 - Modify Valve Room Doors and HVAC for Floo[1 Protection A workplan was written, approved, and placed in work for this activity. Work continues.
