3 86-001-00 IC RELAYX A While conducting a turbine independent electrical overspeed protection system (IEOPS) analog test a stuck blocking relay (LTB-1) in the IECPS circuit allowed main turbine generator steam stop and control valves to close on the test signal. This caused steam generator levels to drop below trip set point.
