At 1330 on 9-19-85, with the unit suberitical, it was discovered during a review of a plant problem report '(Condition Report) that a level transmitter for the scram discharge volume had been replaced with a transmitter that did.not meet IEEE Standard 323-1974 qualifications. The Shift Supervisor was notified and the channel was placed in the trip condition. The incorrect transmitter was replaced with the correct transmitter on 9-20-85 2nd declared operable.

The error was due to maintenance personnel not using controlled documents to specify a replacement spare component. The governing procedure will be revised to specify the use of only controlled documents.

. a sm.wamewmewnn On 9-19-85 while investigating Condition Report 85-311, it was discovered that an incorrect scram discharge volume level transmitter had been installed. The incorrect transmitter did not meet IEEE Standard 323-1974.

Further investigation revealed that the incorrect transmitter had been installed as a result of utilizing the "Intermat Report-C." This report does not necessarily specify presently approved plant equipment device numbers.

The cause of the error was failure to use controlled documents in the analysis of parts required to implement Maintenance Work Requests (MWRs).

Immediate corrective action was taken. The Shift Supervisor was informed of the error on 9-19-85.

The transmitter was declared inoperable and the associated trip unit was placed in the trip condition. MWR 10233 was initiated to replace the correct transmitter and was completed on 9-20-85.

In an effort to prevent recurrence, a revision to MSP-0007 (Maintenance Planning and Scheduling) will be made to direct maintenance department personnel to use controlled documents when pecifying spare parts for installation and not to use the "Intermat Report-C" except for general information.

A review of safety related MWRs where a replacement component from the GSU warehouse was used for repair and installed in the plant against controlled documents is to be performed to ensure that the proper components were used.

The review is to be complete by January 15, 1986.
