Subject:

Shearon Harris Nuclear Power Plant, Unit No. 1, 10CFR50.55(e) and 10CFR, Part 21, reportable deficiency. Check valves supplied with internal parts which may come loose and cause loss of operability.

Item:

ASME Code Class 2 and 3 and non-nuclear check valves supplied under Purchase Order NY-435013.

Surplied By:

Anchor Darling Valve Co., 701 First Street, Williamsport, PA 17701 Nature of Deficiency:

Site Nonconformance Report-85-2003 documents that 150 and 300 pound pressure class swing check valves utilize a set screw in the hinge support for the purpose of keeping the hinge pin from rotating.

Also, this set screw limits lateral movement of the pin which, if loose, could allow the hinge support and disc to become separated from the valve bonnet.

Date Problem Occurred:

Site Nonconformance Report-85-2003 was written October 9, 1985 documenting the condition.

Date Problem Reported:

On October 25, 1985, Mr. K.V. Hate' notified the NRC (Mr. F. Cantrell) that the item was reportable per the provisions of 10CFR50.55(e) and 10CFR, Part 21.

Scope of Problem:

Twelve (12) ASME Code Class 2 and 3 check valves and twenty-seven (27) non-nuclear check valves.

Safety Implication:

There are one (1) each of these valves upstream of the A and B safety train containment spray pumps.

Ingestion of the loose parts into the pumps could cause loss of both containment spray pumps.

Reason Deficiency is Reportable:

Corrective Action:

The set screws have been inspected and tack welded as required to prevent loosening in the twelv-)

(12) ASME Code Class 2 and 3 valves. The twenty-seven (27) non-nuclear valves have also been inspected and corrected with the exception of one

' (1). The remaining valve is located in a system where loose parts and operability are not a safety consideration. Repair of this valve is scheduled for a window in pre-op testing. With the actions completed as stated above, this item is considered resolved.
