Box 128 San Clemente, California 92674-0128 SUBJECT: NRC INSPECTION REPORT 50-361/97-02:50-362/97-02 AND NOTICE OF VIOLATION Dear Mr. Ray: An NRC inspection was conducted January 12 through February 22,1997, at your San Onofre Nuclear Generating Station, Units 2 and 3 reactor f acilities. The enclosed report presents the scope and results of that inspection. During the 6-week period covered by this inspection, your conduct of activities at the San Onofre f acilities was generally characterized by safety-conscious operation, sound engineering and maintenance practices, and careful radiological work controls. During this inspection period two violations were identified in the operations area, three situations involving f ailure to follow procedural or drawing requirements were identified regarding: (a) the order of valve manipulations while realigning shutdown cooling, resulting in the inadvertent transfer of about 1000 gallons of water from the refueling water storage tank to the reactor coolant system; and (b) maintaining reactor coolant system pressure within prescribed parameters. These violetions were the result of weaknesses in communication and operator attention to detail. In the engineering area, personnel conducting steam generator tube hydrostatic tests failed to maintain pressure for the length of time specified by procedure. These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding the violations are described in detailin the enclosed report. Please note that you are required to respond to this letter and should follow the , instructions specified in the enclosed Notice when preparing your response. The NRC will /! use your response, in part, to determine whether further enforcement action is necessary l to ensure compliance with regulatory requirements.
