SUBJECT: NRC INSPECTION REPORT 50-456/96019;50-457/96019
Dear Mr. Stanley:
On November 29,1996, the NRC completed an inspection at your Braidwood 1 & 2
reactor facilities. The enclosed report presents the results of that inspection.
During the period covered by this report, the inspectors concluded that the plant was
operated safely; however, we are concerned about several incidents of poor performance
by operations and maintenance.

The inspectors identified that the procedure was not followed when the 1C Steam
Generator primary manway was being replaced which was in violation of NRC
requirements. We are concerned that the individual performing the work, the individual
dedicated to monitor the procedure steps, and the quality control inspector did not detect
this procedural violation. In responding to this specific violation please also address the
actions you are taking to improve your oversight of significant activities. The inspectors
also identified a violation regarding the adequacy of your facilities controls to prevent
introduction of foreign materials into safety-related systems. This resulted in foreign
material being dropped into the essential service water system intake structure.
The violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement ection is necessary to ensure compliance with
regulatory requirements.

Two other issues arose during this inspection period. One resulted in poor reactivity

control and the other involved electricalleads that were reversed in an emergency diesel
generator control circuit. These licensee identified and corrected issues are Non-Cited
Violations.
