l 4 2. Exit Interview The inspection scope and findings were summarized on June 28, 1985, with ! those persons indicated in paragraph 1 above. During the reporting period 1 frequent discussions were held with the General Manager and/or his assistants concerning inspection findings. The licensee acknowledged the i findings and took no exception. The licensee did not identify as proprie- . tary any of the materials provided to or reviewed by the inspectors during i l this inspection. , t i 3. Licensee Action on Previous Findings The following item has been reviewed by the inspectors and is considered ' resolved: i j (Closed) Violation (366/82-24-01); Failure to Declare Fire Doors Inoperable l and Post a Fire Watch. 4. Unresolved Items (URI)* , ! i (Closed) URI (321,366/85-05-03); This has been reviewed by the inspectors i and it was found that the diesel lube oil temperature is maintained between 1 130 and 135 F by an automatic system. Also, it was determined that the
local lube oil thermometer is in the keep warm flow path and, even though it is not thermally insulated, it is representative of engine lube oil j temperature. ! l

. 2 ' The licensee's engineering department had recommended that the lube oil temperature be recorded at least daily in the outside rounds procedure. This URI is closed and Inspector Followup Item (IFI) (321,366/85-18-01) is opened to track the procedural incorporation of diesel lube oil temperature recordings. (Closed) URI (321,366/85-05-04); This has been reviewed by the inspectors and it was found that the diesel water jacket temperature control is satis- factory. The daily outside rounds procedure lists the normal range of this parameter as 90 to 115 F while the thermostat controls the temperature with heater on at 103 F and off at 110 F. The licensee is reviewing the outside rounds procedure for this and other possible normal range revisions. This URI is closed and IFI (321,366/85-18-02) is opened to track the procedural incorporation of the diesel water jacket temperature limits and other possible normal range revisions. (Closed) URI (321,366/85-05-05); This has been reviewed by the inspectors and it was determined that the full standby mark has been added to the level gauges. The licensee is reviewing the need to add a note to the outside rounds procedure to state that the oil level should be between the full standby and the full running marks when the engine is shutdown, and between the full running and the add marks when the engine is running. Also, the licensee is reviewing the need to add a running oil level check to their procedures. This URI is closed and IFI (321,366/85-18-03) is opened to track the procedural incorporation of a diesel lube oil running level check. (Closed) URI (321,366/85-05-07); This has been reviewed by the inspectors and it was found that the licensee performed their own oil analysis at the Georgia Power Technical Laboratory. Site analysis capability is being obtained. This URI is closed. No new unresolved items are opened in this report. 5. Plant Tours (Units 1 and 2) The inspectors conducted plant tours periodically during the inspection interval to verify that monitoring equipment was recording as required, equipment was properly tagged, operations personnel were aware of plant conditions, and plant housekeeping efforts were adequate. The inspectors also determined that appropriate radiation controls were properly estab- lished, critical clean areas were being controlled in accordance with procedures, excess equipment or material was stored properly and combustible material and debris were disposed of expeditiously. During tours the inspectors looked for the existence of unusual fluid leaks, piping vibra- tions, pipe hanger and seismic restraint settings, various valve and breaker positions, equipment danger and caution tags, component positions, adequacy of fire fighting equipment, and instrument calibration dates. j r - . 3 The inspectors routinely conduct partial walkdowns of ECCS systems. Valve and breaker / switch lineups and equipment conditions are randomly verified both locally and in the control room. During the inspection period the inspectors conducted a complete walkdown in the accessible areas of the Unit 2 residual heat removal system to verify that the lineups were in accordance with licensee requirements for operability and equipment material conditions were satisfactory. Within the areas inspected, no violations or deviations were identified. 6. Plant Operations Review (Units 1 and 2) The inspectors periodically during the inspection interval reviewed shif t logs and operations records, including data sheets, instrument traces, and records of equipment malfunctions. This review included control room logs and auxiliary logs, operation orders, standing orders, jumper logs and equipment tagout records. The inspectors routinely observed operator alertness and demeanor during plant tours. During normal events, operator performance and response actions were observed and evaluated. The inspectors conducted random off-hours inspections during the reporting interval to assure that operations and security remained at acceptable levels. Shift turnovers were observed to verify that they were conducted in accordance with approved licensee procedures. On May 24, Unit 2 scrammed on main steam isolation valves (MSIVs) not fully open. The removal of a clearance was in progress which would have removed the air supply from drywell pneumatics and left the drywell pneumatics supplied by nitrogen. The secondary containment isolation valves (2T48-F026 and 2T48-F027) were aligned open and independently verified on May 14, 1985. On May 24 they were found to be shut. These two valves being out of position (shut) isolated the normal nitrogen supply to the MSIVs which, as the pressure decreased, held the MSIVs open and they drifted toward the shut position. A scram occurred due to the MSIVs not fully open with power above 30L The Nitrogen valves out of position is a violation (366/85-18-04). 7. Technical Specification Compliance (Units 1 and 2) During this reporting interval, the inspectors verified compliance with selected limiting conditions for operations (LCOs) and results of selected surveillance tests. These verifications were accomplished by direct observation of monitoring instrumentation, valve positions, switch posi- tions, and review of completed logs and records. The licensee's compliance with selected LC0 action statements were reviewed on selected occurrences as they happened. Within the areas inspected, no violations or deviations were identified. 8. Physical Protection (Units 1 and 2) The inspectors verified by observation and interviews during the reporting interval that measures taken to assure the physical protection of the e
