Dear Mr. Collins:

Steam Generator Tube Failure Draft Report The purpose of this letter and the attached draft report is to summarize and document information concerning the Fort Calhoun Station steam generator tube failure incident. The attached draft report contains a description of the event, actions taken by the District, results of those actions, the District's 10 CFR 50.59 evaluation, and conclusions reached based upon those results. The report assumes successful completion of several actions during the remainder of this outage (i.e., tube plugging, tube removal, etc.). If the District is unable to complete any of the above actions, your office will be notified. The attached draft report also documents the completion of those items stated in the letter from J. T. Collins to W. C. Jones dated June 5, 1984.

This draft report has undergone reviews by a variety of District personnel and is under final review by the Plant Review Committee (PRC). The final report will be forwarded to your office on Tuesday, June 19, 1984.

The above referenced draft report documents a very comprehensive and thorough effort conducted by the District in order to establish an extremely high confidence level that the Fort Calhoun Station can be safely returned to power operation.

In addition, the District believes it has taken every reasonable and practical action to not only demonstrate that the Fort Calhoun Station can be safely returned to power operation, but that it can be operated until the next refueling outage.
