---The unit is in mode 3 with the RCS at 530*F after tripping on high turbine vibration via ARTS during turbine valve testing at 0605 Sunday, June 2, 1985.
---Trip anomalies include:
Both MFPs tripped, cause undermined.
Both AFW pumps started on SFRCS low level, but after being placed in manual,
- 1 AFP stuck at low speed stop. The problem appeared to be a bad governor clutch and a burned motor.
A MSSV stuck, but reseated at approximately 950 psig.
- The main turbine wouldn't go on gear due to blown fuses.
---Damper HV5736 has been repaired.
---#2 SAC is idling, the Atlas diesel is carrying the load. #2 SAC tripped on motor everload on Saturday, June 1, 1985.
---RPS Ch. 4 has had the RCS flow filter module installed, but the output signal is noisy. Testing continues.
---Walt Rogers, Resident NRC Inspector, is going to attend Simulator Training in Chattanooga, Tennessee, the weeks of June 2 and June 9, 1985.
---Toledo Edison welcomes Scott Saunders, Assistant Engineer, Nuclear Facility Engineering. Scott started work at Davis-Besse last week.

---The unit is in mode 1 with the power level of about 10%. Startup is in progress.
---Shell and chest warming is in progress. Turbine roll is expected this morning.
---#1 AFP has been repaired. A broken pin on the governor clutch was found. The governor motor was replaced.
---It has been determined one MFT will be lef t in manual indefinitely after startup to decrease the likelihood of tripping both MFPs on a reactor trip.
---3000 hr PM is in progress on SAC #1.
---Water treatment demineralizer trains are no longer available due to reliefs that cannot be gagged. Currently we have 20,000-25,000 gallons of demin, water.
---Due to a damaged magnetrol deaerator #2 level control may be taken out of service for maintenance. This would require manual level control.
---Control room EVS train 1 is out of service for PMs.
---SW #3 is out of service for work on breaker ACDS. Pump is expected back later today.

---The unit is in mode 1 generating 390 megawatts electric net. Power is being held to troubleshoot the Main Feed Pump Control Oil Pressure problems.
---While the plant was at approximately 56% power last night, HFP #1 tripped. The pump is tripping after the standby oil pump is shutdown. Investigation is continuing.
---LSHH 405 for Deaerator #2 level control was fixed yesterday and has been returned to service.
---SW Pump #2 is out for PM's. SW Pump #3 has been returned to service.
---A relief valve on a water primary demineralizer train has been repaired.
---NI 1 is inoperable.
---Control Room EVS Train 1 is back.

---The unit is in 50% reactor power with a net electr.ical output of 410 megawatts.
P:wer continues to be limited pending the Main Feed Pump Control Oil problem resolution.
Additional instrumentation was installed on No. 1 Main Feedwater Pump last night to cupport continued vendor testing.
---Last night, Main Turbine Control Valve 1 drif ted closed. The other valves opened to compensate, increasing electrical output by three to four megawatts.
---EDG #2 is out of service for PM's.
---An Expanded Functional test on AFW train 1-2 is being set up. Hopefulily, data will b2 provided to end hanger / snubber problems.
---Troubleshooting on MFP #1 continues. Yesterday, the pump was induced to trip by starting and stopping the auxiliary lube oil pump. This induced a pressure spike in the system, presumably due to a check valve or pressure control valve problem. The cruse of both MFP's tripping following the last reactor trip appears to be related to en overly fast RFR target speed setpoint.

---The unit is in Mode 1 generating 790 megawatts net. Power was increased from 390 megawatts net overnight after completion of troubleshooting on #1 Main Feedwater Pump Turbine.
---#1 Control Valve has been fixed and is open. Loose terminal connections were found.
---EDG #2 was returned to service after PMs.
---SW Pump #2 is back in service after PMs.
---Incores are inoperable.
---MFPT #2 is in manual to prevent RFR from possibly tripping. Both MFPs on a Rx trip.
---RTS Channel 4 tripped to install fix for RCS flow noise problem.
---MS349 W Heater 6-2 Extraction NFV air line was fixed.
---Main Generator H2 drop test in progress.

DAVIS-BESSE STATION -
---The plant is in Mode 4. The reactor tripped Sunday morning at 0135 hours0.00156 days <br />0.0375 hours <br />2.232143e-4 weeks <br />5.13675e-5 months <br /> after runback due to the #1 Main Feedwater Pump tripping. The plant is currently being cooled down to Mode 5 to repair various equipment.
---The reactor tripped on RCS high pressure. After the trip, it was noted that the MSIVs had closed.' SFRCS automatically was then actuated on SG Lo Level but was manually actuated on Lo MSL pressure by accident. The AW pumps tripped on overspeed and AF599, 608 subsequently torqued out. The #1 AFW pump was reset and FW was reestablished to SG's along with the SUFP to No. 1 OTSG. RCS pressure increased during trip recovery to open the PORV several times. HPI was lined up in the piggyback mode. The #1 AFP was declared inoperable but was used in local manual control and the cooldown proceeded using the 2 AFW pumps and one SUFP.
---When AW 1 was actuated, its suction swapped over to SW.
---Numerous problems have been experienced with the Aux. Boiler tripping.
---The yoke on SP13A2 (TBV) has been broken apparently due to a water slug.
---RCPs have been shut off. 240*F (Mode 4).
---Outage items:
- Testing of MSIV solenoids.
MFP speed indication.
Ch 2 Hi/Lo Level trip.
AFP trip throttle valve linkages.
Root cause of AFPT overspeeding.
235 # Desuperheater controller.
- Deaerator level trip.
Cause of MSIV shutting.
SPDS.
Overspeed tripping of No. 1 MFP.
NI-1.
RE4598BA spiking.
- AF 599, AF 608 torquing out.
SUFW valve would not respond to block.
- Engineering analysis of feeding dry OTSG with SUW.
---Will leave vacuum on, and a bubble in the pressurizer.
---Toledo Edison welcomes James McNally who begins work this week in Davis-Besse Maintenance.

---The plant is in Mode 5 with the Reactor Coolant System at 110 degrees and 50 psig.
The major outage work is on hold until the NRC arrives on site for their investigation, however, maintenance efforts are continuing. A National Inspection Examination Board will be investigating the events which occurred on Sunday.
---DH Loop 2 is ongoing through makeup and purification.
---The OTSGs are at wet layup levels, recircing chemicals.
---The turbine is off gear.
---Vacuum is broken.
---RE4598BA has been jumpered out to prevent tripping the CTRM ventilation.
---The cire water system will be shut down when residual chlorine levels drop of f.
---Makeup and purification system is off.
---There is an intermittent ground on DCMSC 2.
---The Aux. Boiler is shutdown.
---CTMT Purge is in progress. .
---SAC #1 still remains out of service.
---Condensate pump is on.

---DH Loop #2 on through MU&P.
---The deep end of the Cire. Water Canal is being drained.
---Maintenance in containment is to commence today. Containment work will be on day shift only. Personnel Hatch inner door interlock defeated.
---Normal HA flowpath from BAAT's is inoperable due to work on HA check valves.
1 ---RCS flaw transmitters for RPS Channels 3 and 4 are being recalibrated.

Caution must be utilized on the Fifth Floor and Southeast corner of the Station while the Office Building roof replacement is in progress. The corner by the Maintenance Shop has been barricaded due to the dust / debris and overhead hoisting being done.
The Maintenance Shop rollup door should also remain closed between 0700-1000 hours during the main disposal of the old roofing particles. Project is expected to last two ,
1 weeks.

- 1. Review the' licensee's Guidelines to Action Item Lead Individuals.
- 2. Review specific action plan assigned to you.
- 3. Determine if maintenance personnel have been properly briefed.
- 4. Confirm that the person assigned to do the work is qualified and is at the job site.
- 5. Ensure that proper communications have been established between the person assigned and the individual having prime responsibility.
- 6. Confirm that the proper procedures are at the job site and are being used.
- 7. Ensure that the licensee is working within the guidelines specified in Item 1 above.
- a. Determine that MWO's have proper review and approval and are adequate to do work.
- b. Determine current drawings and controlled vendor manuals are used.
NOTE: To satisfy this item, document the drawing revision and vendor manual date or revision and compare them against the control copy.
- c. Ensure all "as found" conditions are documented on the MWO.
- d. Ensure that all results of the investigation are clearly documented on the MWO.
- e. Assure personnel work within the guidelines of the MWO.
