detection zone shown in Table 3.3-12 shall be OPERABLE.

- b. Removing one wire from each of a dome, vertical and hoop tendon checked for lift off force and determining that:
- 1. The corrosion level over the entire length of the tendon wires has not progressed since the original insta11ation'or the previous surveillance.

- 3. A minimum tensile strength of 240,000 pounds per square inch for at
.least three wire samples (one from each end and one at mid-length) cut from each removed wire. Failure of any one of the wire samples to meet the minimum tensile strength test is evidence of abnormal degre.dation of the containment structure.
,i 4.6.1.6.2 End Anchorages and Adjacent Concrete Surfaces The structural integrity of the end anchorages and adjacent concrite surfaces shall be

4.6.1.6.3 Liner Plate The structural integrity of the containment liner plate shall be detennined during the shutdown for the first Type A containment leakage rate test only (reference Specification 4.6.1.2) by a visual inspection of the

- c. Startup sources and fission detectors - Each sealed startup source and fissfor. detector shall be tested within 31 days prior to being subjected to core flux or installed in the core and following repair or maintenance to the source.

- b. Separate water supplies, each with a minimum contained volume of 250,000 gallons, and c.. An OPERABLE flow path capable of taking suction from each tank and transferring the water through distribution piping with OPERABLE sectionalizing control or isolation valves to the yard hydrant curb ~

POWER DISTRIBUTION LIMITS BASES The radial peaking factor Fxy(Z), is measured periodically to provide additional assurance that the hot channel factor, F9 (Z), remains within its limit. The Fxy
-limit for RATED THERMAL POWER (FNP) as provided in the Radial Peaking Factor limit report per Specification 6.9.1.11 was determined from expected power l control maneuvers over the full range of burnup conditions in the core.
3/4.2.4 QUADRANT POWER TILT RATIO The quadrant power tilt ratio limit assures that the radial power distribution satisfies the design values used in the power capability analysis. Radial power distribution measurements are made during startup testing and periodically during power operation.
The limit of 1.02, at which corrective action is required, provides DNB and
-linear heat generation rate protection with x-y plane power tilts.
The two hour time allowance for operation with a tilt condition greater than 1.02 but less than 1.09 is provided to allow identification and correction of a dropped or misaligned control rod. In the event such action does not correct the tilt, the margin for uncertsinty on Fq is reinstated by reducing the maximum allowed power by 3 percent for each percent of tilt in excess of 1.0.

3/4.4.6 STEAM GENERATORS The Surveillance Requirements for inspection of the steam generator tubes ensure that the structural integrity of this portion of the RCS will be maintained. The program for inservice inspection of steam generator tubes is based on a modification of Regulatory Guide 1.83, Revision 1. Inservice inspection of steam geaerator tubing is essential in order to maintain surveillance of the conditions of the tubes in the event that there is evidence of mechanical damage or progressive degradatien due to design, manufacturing errors, or inservice conditions that lead to corrosion. Inservice inspection of steam generator tubing also provides a means of characterizing the nature and cause of any tube degradation so that corrective measures can be'taken.
The plant is expected to be operated in a manner such that the secondary coolant will be maintained within those chemistry limits found to result in negligible corrosion of the steam generator tubes. If the secondary coolant chemistry is not maintained within these limits, localized corrosion may likely result in stress corrosion cracking. The extent of cracking during plant operation would be limited by the limitation of steam generator tube leakage between the primary coolant system and the secondary coolant system (primary-to-secondary leakage =
500 gallons per day per steam generator). Cracks having a primary-to-secondary leakage less than this limit during operation will have an adequate margin of safety to withstand the loads imposed during normal operation and by postulated accidents. Operating plants have demonstrated that primary-to-secondary leakage of 500 gallons per day per steam generator can readily be detected by radiation monitors of steam generator blowdown. Leakage in excess of this limit will require plant shutdown and an unscheduled inspection, during which the leaking tubes will be located and plugged.
Wastage-type defects are unlikely with proper chemistry treatment of the secondary coolant. However, even if a defect should develop in service, it will be found during scheduled inservice steam generator tube examinations. Plugging will be required for all tubes with imperfections exceeding the plugging limit of 40% of the tube nominal wall thickness. Steam generator tube inspections of operating plants have demonstrated the capability to reliably detect degradation that has penetrated 20% of the original tube wall thickness.
Whenever the results of any steam generator tubing inservice inspection fall into Category C-3, these results will be reported to the Commission pursuant to l 10CFR50.73 prior to resumption of plant operation. Such cases will be considered by the Commission on a case-by-case basis and may result in a requirement for analysis, laboratory examinations, tests, additional eddy-current inspection, and revision of the Technical Specifications, if necessary.

The principal gamma emitters for which the MDC specification applies exclusively are the following radionuclides: Mn-54, Fe-59, Co-58, Co-60, Zn-65, Mo-99, Cs-134, Cs-137, Ce-141 and Ce-144 for particulate emissions. This list does not mean that only these nuclides are to be detected and reported. Other peaks which are measureable and identifiable, together with the above nuclides, shall also be identified-and reported.
- h. Deviations from MDC requirements of Table 4.11-2 shall be reported per l Specification 6.9.1.8.
- i. A composite particulate sample is one in which the quantity of air sampled is proportional to the quantity of air discharged. Either a specimen which is representative of the air discharged may be accumulated and analyzed or the individual samples may be analyzed and weighted in proportion to their respective volume discharged.
- j. The principal gamma emitters for which the MDC specification applies exclusively are the following radionuclides: Kr-87, Kr-88, Xe-133, Xe-133m, Xe-135 and Xe-138 for gaseous emissions. With the calculated doses from the release of radioactive materials in liquid or gaseous effluents exceeding twice the limits of Specification 3.11.1.2.a, 3.11.1.2.b, 3.11.2.2.a, 3.11.2.2.b, 3.11.2.3.a, or 3.11.2.3.b prepare and submit a Special Report to the Director, Nuclear l Reactor Regulation, U.S. Nuclear Regulatory Comission, Washington, D.C. 20555, within 30 days, which defines the corrective action to be taken to reduce subsequent releases to prevent recurrence of exceeding the limits of Specification 3.11.4. This Special Report shall include an analysis which estimates the radiation exposure (dose) to a member of the public from uranium fuel cycle sources (including all effluent pathways and direct radiation) for a 4 consecutive quarter period that includes the release (s) covered by this report. If the estimated dose (s) exceeds the limits of Specification 3.11.4, and if the release condition resulting in violation of 40CFR190 has not already been corrected, the Special Report shall include a request for a variance in accordance with the provisions of 40CFR190 and including the specified information of $ 190.11(b). Submittal of the report is considered a timely request, and a variance is granted until staff action on the request is complete. The variance only relates to the limits of 40CFR190, and doas not apply in any way to the requirements for dose limitation of 10CFR Part 20, as addressed in other sections of this technical specification.
- b. The provisions of Specifications 3.0.3 and 3.0.4 are not applicable.

---3 The radial peaking factor Fxy(Z), is measured periodically to provide additional assurance that the hot channel factor, Fq (Z), remains within its limit. The Fq limit for RATED THERMAL POWER (FgTP) as provided in the Radial Peaking Factor limit report per Spt.cification 6.5.1.11 was determined from expected power control maneuvers over the full range of burnup conditions in the core. l 3/4.2.4 QUADRANT POWER TILT RATIO The quadrant power tilt ratio limit assures that the radial power distribution satisfies the design values used in the power capability analysis. Radial power distribution measurements are made during startup testing and periodically during power operation.
The limit of 1.02, at which corrective action is required, provides DNB and linear heat generation rate protection with x-y plane power tilts.
The two hour time allowance for operation with a tilt condition greater than 1.02 but less than 1.09 is provided to allow identification and correction of a dropped or misaligned control rod. In the event such action does not correct the tilt, the margin for uncertainty on Fq is reinstated by reducing the maximum allowed power by 3 percent for each percent of tilt in excess of 1.0.
3/4.2.5 DNB PARAMETERS The limits on the DNB related parameters assure that each of the parameters are maintained within the nonnal steady state envelope of operation assumed in the transient and accident analyses. The limits are consistent with the initial FSA7, assumptions and have been analytically demonstrated adequate to maintain a l minimum DNBR of 1.30 throughout each analyzed transient.

' REACTOR COOLANT SYSTEM BASES

!3/4.4.6 STEAM GENERATORS The' Surveillance Requirements for inspection of the steam generator tubes ensure that.the structural integrity of this portion of the RCS will -be maintained. The program'for-inservice inspection of steam generator tubes is based on a modification of Regulatory Guide 1.83, Revision 1. Inservice inspection of steam
- generator tubing is essential in order to maintain surveillance of the conditions of the tubes in the event that there is evidence of mechanical damage or-Lprogressive degradation due to design, manufacturing errors, or inservice-conditions that-lead to corrosion. Inservice inspection of steam generator Ltubing also provides a means of. characterizing the nature and cause of any tube -
degradation so that corrective measures can be taken.
The plant is expected to be operated in a manner such that the secondary coolant will be maintained within those chemistry limits found to result in negligible corrosion of the steam generator tubes. If the secondary coolant chemistry is not maintained within these limits, localized corrosion mcy likely result in stress corrosion cracking. - The extent of cracking during plant operation would be limited by the limitation of steam generator tube leakage between the primary coolant system and the secondary coolant. system (primary-to-secondary leakage =
.500' ga11ons per day per steam generator). Cracks having a primary-to-secondary
^ 1eakage less than this limit during operation will have an adequate margin of safety to withstand the loads imposed during normal operation and by postulated accidents. Operating plants have demonstrated that primary-to-secondary leakage of 500 gallons ver day per steam generator can readily be detected by radiation monitors of. steam generator blowdown. Leakage in excess of this limit will require plant shutdown and an unscheduled inspection, during which the leaking tubes wi11 be located and plugged.
Wastage-type defects are unlikely with proper chemistry treatment of the secondary coolant. However, even if a defect should develop in service, it will be found'during scheduled inservice steam generator: tube examinations. Plugging will be required for all tubes with imperfections exceeding the plugging limit of 40%-of the tube nominal wall thickness. , Steam generator tube inspections of operating plants have demonstrated the capability to reliably detect degradation-that has penetrated 20% of the original tube wall' thickness.
Whenever the'results of any steam generator tubing inservice inspection fall into Category C-3, these results will be reported to the Connistion pursuant to
- 10CFR50.73 prior to resumption of plant operation. Such cases will be considered
- by the Commission on 'a case-by-case basis and may result in a requirement for
' analysis, laboratory examinations, tests, additional eddy-current inspection, and revision of the Technical Specifications, if necessary.

MONTHLY OPERATING REPORT 6.9.1.10 Routine reports of operating statistics and shutdown experience, including documentation of all challenges to the PORV's or safety valves shall be submitted on a monthly basis to the Director, Office of Management and Program Analysis, U.S. Nuclear Regulatory Comission, Washington, D.C. 20555, with a copy to the Regional Office of Inspection and Enforcement, no later than the 15th of each month following the calendar month covered by the report.
Any changes to the 0FFSITE DOSE CALCULATION MANUAL shall be submitted with the Monthly Operating Report within 90 days in which the change (s) was made effective. In addition, a report of any major changes to the radioactive waste treatment systems shall be submitted with the Monthly Operating Report for the period in which the change was implemented.
RADIAL PEAKING FACTOR LIMIT REPORT 6.9.1.11 The F limit for Rated Thennal Power (FgR P) shall be provided to the Director of thhRegional Office of Inspection and Enforcement, with a copy to the Director, Nuclear Reactor Regulation, Attention Chief of the Core Performance Branch, U.S. Nuclear Regulatory Comission, Washington, D.C. 20555 for all core planes containing bank "D" control rods and all unrodded core planes at least 60 days prior to cycle initial criticality. In the event that the limit would be submitted at some other time during core life, it.will be submitted 60 days prior to the date the limit would become effective unless otherwise exempted by the Commission.
Any information needed to support Fff%ill be by request from the NRC and need not be included in this report.

Unusual or Important Environmental Events Any occurrence of an unusual or important event that indicates or could result in significant environmental impact causally related to plant operation shall be recorded and reported to the NRC in accordance with 10CFR50.72(b)(2)(vi) or by a written report per Subsection 5.4.2, as appropriate. The following are examples:
. excessive bird impaction events, onsite plant or animal disease outbreaks, mortality or unusual occurrence of any species protected by.the Endangered i Species Act of 1973, fish kills, increase in nuisance organisms or conditions and
