Core spray pump 2B auto start failure during the first phase of the Emergency Core Cooling System simulated automatic actuation surveillance test.

CONDITIONS PRIOR TO OCCURRENCE:

Unit 2 was in the cold shutdown mode for refueling.

DESCRIPTION OF OCCURRENCE:

On April 18, 1975 at 8:00 p.m. the Emergency Core Cooling System (ECCS)

Simula'ted Auto Actuation and Diesel Generator Auto-Start Surveillance test (QTS 110-3, Revision 1) was being run. When the reactor vessel water lo-lo level signal was simulated, the 2B core spray pump did not auto start prior to the bus 24-1 trip. After bus 24-1 was tripped and diysel generator 2 auto closed into bus 24-1, the 2B core spray pump did auto start.

DESIGNATION OF APPARENT CAUSE '0F OCCURRENCE:

The cause of this occurrence was a loose terminal connection on control panel 902-33, terminal AA-43 ANALYSIS OF OCCURRENCE: -

Although 2B core spray pump would not start on automatic initiation from its normal source,-it did auto start from its emergency source and could have been manually started. Had an initiation signal been received during normal operation, 2A core spray pump would have started and supplied 100% of the required co're spray flow. All four Residual Heat Removal System pumps also would have started and added water to the reactor. In view of this, the inability of 2B core spray pump to start in one of its three modes poses minimal safety implications and had no effect on'the health and safety of the public. .

The inmediate corrective action was to determine the cause of the f ailure of the 2B core spray pump to auto start ir, one instance and not the other.

The loose connection was subsecuently found and tightened. Auto-start of 2B core spray pump on its normal source was successfully tested on April 20, 1975 FAILURE DATA: -

There have been no similar auto start failures of the 28 core spray pump or the other core spray pumps due to a loose terminal connection. There-fore there are no safety implications based on cumulative experience.
