i CIO REPORT NUMBER 32 i

I. OVERVIEW During this reporting period, principal CIO activities were directed toward an evaluation of CPCo plans and actions to implement corrective action for document control problems identified in CIO Item Number 043.

On January 16, CPCo provided CIO with their plan for completion of

. Phase III (Register Reconciliation) activities, corrective action relative to document control, lifting of CPCo Stop Work Orders 34, 36, 39, 40 and 41 and commencing status assessment and Quality Verification Program (QVP) activities. Major points in the CPCo plan include: ,

  • Disestablishing all document control stations except for those necessary to support the Remedial Soils Program (8, 10, 13, 24 and 64) and the Field Document Control Center (Station 59, FDCC).
  • Establishing a Status Assessment Print (SAP) Program, 1 controlled by FDCC for verification of documents as they are issued for status assessment.

Establishing a Work Print (WP) program controlled by FDCC for verification of documents as they are issued for work other than that in the remedial soils area.

  • Procedure revision to reflect the current approach to document control.

CIO reviewed the CPCo plan and considered it to be an acceptable approach for resolution-of identified problems and commencement of status assessment and QVP.

, lift the Stop Work Orders and to activate FDCC to issue SAPS and WPs.

Additionally, Phase III work was completed for Stations 8, 10, 13, 24 and 64 CIO completed our review of these activities and as a result of satisfactory performance lifted Hold Point Number 012. Additionally, CIO commenced an evaluation of FDCC to detemine their readiness to begin issuing SAPS and WPs and to service the remedial soils stations.

Satisfactory completion of this evaluation led to CIO closure of Hold Points 013 and 014. Several recommendations were made to CPCo during this review which will be forwarded to CPCo next week and as a follow-on to this, CIO will evaluate perfomance during the next reporting period when the activity is expected to commence. Additionally, based upon our review, there a're two areas which CIO concluded should be checked during the reactivation of the remedial soils stations. These related to personnel training and document distribution to vendors, and will be referred to the Remedial soils Assessment Team for follow-up.

Overview of Status Assessment /QVP activities for structural coatings in Module 340.

  • Planning for Phase III review of FCR/FCNs in the Electrical and Mechanical disciplines.

III. ACTIVITY DETAILS (Paragraphs correspond to items in Section II)-

B. At Remedial Soils Stations (8,10,13, 24 and 64) CIO perfomed a review of Phase III Register Reconciliation of vendor documents, FSKs. design drawings, specifications and the Retirement Documenta-tion Program in the civil discipline. One discrepant item was

JOB NO. 14509 Page 4 of 7 MIDLAND NUCLEAR POWER PLANT WEEKLY REPORT NO. 32 identified at Station 8. The discrepancy related to drawing number CLOO4, Rev. 6 which had FCN C3832 attached. Six copies of the same drawing had the correct Change Notice and were signed by the Project Engineer. On one copy of the drawing, the FCN had not been signed by the Project Engineer. After evaluation of overall results, CIO did not consider this item significant and, once it had been corrected, CIO released these five stations for resumption of work by closing CIO Hold Point Number 012.Section I of this report contains further infomation on this topic.

C. CIO performed sampling inspections of Phase I and Phase II Instrumen-tation and Control FCR/FCNs and associated Generic (G) specifications.

, The, reviews showed satisfactory performance. Hold Point Number 011, established for completion of Phase II activities, is now lifted for this discipline.

D. CIO performed a sampling inspection of 689 training records which represented 102. construction personnel prior to their release for status assessment work The records examined were satisfactory and in compliance with FPG-2.000, Rev. 6 " Training of Construction Personnel" (Re: Checklist Number MP-MIS-001E). To date, CIO has reviewed training records representing 245 personnel, found them to be satisfactory and released the personnel to conduct status assessment as activity warrants.

E. CIO Weekly Report Number 26 documented the release of two personnel to perfom status assessment for Phase I activities.

l Status assessment of decontaminable concrete coating and miscellaneous Q structural steel coatings in Module 340 commenced on December 23, 1983 by these two personnel.

CIO monitored these activities on January 4 and 10, 1984 and observed that the team members were working from appropriate I documents and were checking for proximity violations as defined in Specification C-305 and for construction damage.

Items and areas found to be incomplete are satisfactorily being entered into the Punch List Program.

CIO Weekly Report Number 26 also documented the closure of NIRs 002, 003 and 004, Item 31 snd Hold Point Number 006.

This combination released MPQAD personnel to perform QVP activity. After status assessment, MPQAD began QVP activities in the inspection of decontaminable coatings for concrete.

During the period December 20, 1983 through January 13, 1984, CIO overviewed these activities. The QVP inspections performed 1 were detemined to be in compliance with PQCI C-8.51 Rev. 4 upon review of VIRs C-8.51-3,5,6,7. CIO verified that NCRs, the DIIA form used for Inaccessible Attributes and other pertinent documents were generated as required by the VIR.

One discrepancy was noted by CIO on the completed VIR. The DIIA numbers for the Generic DIIAs were not recorded in Block

i i MPQAD comenced QVP on structural steel coatings in Module 340K l' on January 13, 1984. CIO performed periodic overviews of this activity and detemined that the inspections were perfomed in

accordance with PQCI 8.50 Rev. 13. MPQAD compliance with the requirements of PQCI 8.50 will be detemined upon the CIO review of the completed Inspection Reports.

During the above reviews, CIO also verified through the review of Training Records that the specific personnel assigned to

! perfom QVP were trained and certified to perfom Q'VP to PQCI C-8.51. .

F. CIO has been placed on distribution of MPQAD Audit Reports. -In addition to the activity reported in Weekly Report 31, reviews of these are also being conducted in preparation for an evaluation of the MPQAD Audit Program.
