CIO REPORT NUMBER 31 I. OVERVIEW During this reporting period CPCo St'op Work Orders (FSW 33-41) related to FCR/FCN processing remained in effect. CIO activity, therefore, was at a relativelylow level corresponding to overall site work. '

Principal CIO activities included: Continuation of personnel training i

evaluation; review of individual training records; monitoring progress in the FCR/FCN resolution process, including CPCo planning for resolution

of document control issues identified during the Phase III (Register Re-conciliation) evaluation of FCR/FCN Processing; and planning for future

, CIO activity. Additionally, CPCo has notified.CIO that documents in the Electrical, Mechanical and Instrumentation and Controls disciplines are available for CIO evaluation of Phase II (FCR/FCN Processing) and planning for this activity was initiated.

Other CIO activities in this period included:

Participation in the monthly NRC, CPCo and CIO Public meeting Evaluation of continuing work in the Spatial Systems Interaction Program (SSIP)

Review of MPQAD audit reports in preparation of an evaluation of the audit program Attendance at various site meetings CIO action to close out open Items and NIRs as responses became available i

JOB NO. 14509 Page 2 of S MIDLAND NUCLEAR POWER PLANT WEEKLY REPORT NO. 31 Present information indicates that CPCo is nearing finalization of plans for corrective action on Phase III and it is anticipated that implementation will commence in the near future. When received CIO will evaluate the plans and, if acceptable, establish the appropriate CIO assessment actions based on CPCo activities and schedules.

A.

Architectural Stations 14, 48A, B and D and 55 were released for status i

assessment on December 13, 1983. Subsequently CIO was informed that vendor documents and FSKs were complete, available for review and could be included in future CIO assessments. Additionally, document control problems (CIO Item No. 043,NIR-011 and NIR-012) were identified at othe stations.

CIO determined that it would be necessary to re-assess our release of these stations in view of the above to verify that there was no impact on the released stations.

A review was performed of Document Registers and Field Document Distribution Lists (FDDLs) which indica

Further review revealed that use of the " Retired Document Program" did not affect these stations. Since CIO had addressed all other areas of concern in the original assessment, it was concluded that no further action was necessary and the release of these stations for status assessment should stand.

As a point of information, use of Station 55 for document distribution has been discontinued so there are currently four stations released for architectural status assessment; 14, 48A, 48B and 48D.

B. CIO personnel attended and evaluated the below listed construction training classes. The classes were conducted in an effective manner and in accordance with FPG-2.000, " Training of Construction Personnel".

Based on this, CIO evaluation indicates satisfactory performance (Re: Checklist No. MP-MIS-001E).

Classes Construction Work Plans (FPG-7.300, Rev. 3) - 2 Sessions Concrete Anchors and Grouting BPCo Quality Orientation P & ids Large Bore Piping Isometric Drawings M-400 and M-600 Series. .

C. CIO performed a sampling inspection of 465 training records which rep-resented 9 construction personnel prior to their release for status l

assessment work. The records examined were satisfactory and in com-pliance with FPG-2.000, Rev. 6 " Training of Construction Personnel"

(Re: Checklist No. MP-MIS-001E). Inis brings to 21 the number of -

personnel whose training records CIO has reviewed and found to be satis-factory and have been released to conduct status assessment when this activity begins.

SSIP walkdown of the Auxiliary Building continued in accordance with Mark Technology Corporation (MTC) Procedure WTP-3-Q, " Procedure for Performing Walkdowns of SSIPs/ Targets." Five (5) physical interactions were analyzed and documented by MTC. CIO personnel witnessed all five. CIO evaluation of the SSIP implementation is that it continues to be performed in a satisfactory manner (Re: Checklist No. MP-MIS-MJ-002).

E. CIO received and initiated a review of MPQAD audit reports. This is preparatory to an evaluation of the MPQAD Audit Program.

IV. GENERAL A. CIO has revised the method for reporting and obtaining resolution of Action, Information and Recommendation Items and Hold Points. A CIO Item Report Form has been developed and is l'n use for reporting, obtaining responses and closing Items and Hold Points. A sample of this form is provided as Exhibit I to this report. Initial use of this mechanism indicates that minor modifications will be needed to increase its use-

, fulness; however, no substantial changes are contemplated at this time.

Using this reporting methou is expected to expedite administrative pro-cessing of Items and Hold Points and aid in their tracking and statusing.

B. Mr. R. G. Burns, Corporate Quality Assurance Manager, was on site January 10, 11 and 12, 1984.

C. Mr. W. Grieves, Supervisor, Quality Systems Division, was on site during the week to assist in the development of the trending program.

D. CIO personnel staffing remained at 29 for this reporting period. No changes in staff occurred.
