Rejacking and crack mapping activities were performed, as required, by the project procedures.

Assessment Team Observations The Stone & Webster CIO Team has completed their Phase III overview of the civil documents at the five Documen,t Control Stations associated with the Soils Remedial activities. As a result of the overview , Stone & Webster issued an NIR detailing nonconformances found at three of the five Document Control Stations. MPQAD issued a QAR based on nonconformances detailed in the NIR. Once the QAR is satisfactorily resolved, Stone & Webster will conduct another overview of the Soils Document Control stations.

An MPQAD audit noted document control problems similar to those identified in the NIR resulting in the preparation of a CPCo Management Corrective Action - '

Request / Report. The purpose of this report is to initiate evaluation of recurring document control problems.

An update on corrective actions being undertaken by US Testing was provided by Bechtel. A review of US Testing by Bechtel Field Engineering found that

Cross training of US Testing personnel and development of Work Instructions are the two major items that are still open. Bechtel reported that a revised schedule would be provided next week targeting completion dates for cross referencing of inspectors and development of work instructions. The Assessment Team believes that US Testing should strive to complete the work on these two items in accordance with the revised schedule.

_ PURPOSE This meeting is held each day to discuss items regarding the Independent Soils Assessment at the Midland Plant , Units 1 & 2.

DISCUSSION Status Items Item 68 Stop Work Order Update.

The Stone & Webster (CIO) overview of the 5 Soils Document Control stations found non-conformances with stations, 8, 10, and 13. Stone &

Webster wrote an NIR detailing nonconformances found during the overview.

MPQAD will respond to the NIR today and Stone & Webster will again overview the above referenced three (3) stations. (INFORMATION ITEM)

Item 68 Rejacking of Piers.

J. Fisher reported that as of today, no piers were on active jacks.

Item 68 US Testing Corrective Action.

J. Fisher stated that an update on Corrective Actions undertaken by US Testing would be forthcoming this week. (ITEM WR-64, pg. 1 REMAINS OPEN) (INFORMATION ITEM) .

1 J. Fisher inquired as to the status of Stone & Webster's review of the '

referenced drawings required for drill & excavation permits. P. Majeski responded that the Assessment Team had completed its review of attachments to the reference mechanical & electrical drawings required for processing drill & excavation permits to support underpinning activities over the next 30 days. The review verified that attachments to the referenced drawings had adequately completed the FCR/FCN review cycle. (CLOSED ITEM) l

DISCUSSION Status Items Item 68 Stop Work Order Update.

A QAR was issued yesterday as a result of the NIR written by Stone & Webster (CIO) detailing overview findings of the 5 Soils Documents Control Stations. '

When the QAR is resolved, Stone & Webster will again overview the 5 Soils Document Control Stations. An MPQAD audit noted similar document control problems, resulting in preparation of a CPCo Management Corrective Action l Request / Report. This report was prepared to initiate evaluation of recurring document control problems by CPCo and Bechtel. (INFORMATION ITEM)

Item 68 Update on US Testing Corrective Action.

R. Cleary and J. Werner provided a status report on Corrective Actions being undertaken by US Testing. J. Werner reported that new personnel in key positions (i.e. site manager) have been assigned to the site to implement c orrective a etions. Also it was reported that cross training of technicians is ongoing and will be for the forseeable icture. It was agreed that a schedule will be issued next week targeting a realistic completion date for cross training of inspectors on site.

J. Werner stated that US Testing is in the process of developing a total of 60 Work Instructions for use by US Testing personnel in performing required inspections. These work instructions are being prepared to simplify procedures presently being used by US Testing to perform inspections. It was agreed that a schedule will be provided next week showing completion dates for the Work Instructions. Bechtol reported that a restatus on Corrective Action taken by US Testing would be issued next week. (ITEM WR-64, Pg. 1 REMAINS OPEN) (INFORMATION ITEM)

l Notes of Daily Meeting Independenc Assessment of Underpinning Midland Plant Units 1 & 2 Consumers Power Company Held at Midland Site 1 Midland, Michigan l January 6, 1984 New Items Item 68-11 - Cracking on the Tontrol Tower el. 685' Slab.

Three cracks were found yesterday in the control tower on the el. 685' slab. These cracks were found in an area of the control tower that was not previously included in the crack mapping program. It is not known whether these cracks existed prior to or are a result of underpinning activities. One 3.5 in. diameter core was taken at a crack location and is presently being evaluated by Bechtel. Initial examination of the core has shown the cracks to be at least 12 in. deep. (INFORMATION ITEM)

The Assessment Team has completed review of attachments to the referenced electrical & mechanical drawings required for processing drill & excavation permits needed to support underpinning activities over the next 30 days.

The review verified that attachments to the referenced drawings have adequately completed the FCR/FCN review. Also grout plans formulated to date for the SWPS and the excavation permit required to implement the grouting program have been received by Stone & Webster. (CLOSES ITEM 67-15)
