BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - POTENTIAL DEFECTS IN WKM VALVES

- NCR'S BLN NEB 8002 AND 1316 - REVISED FINAL REPORT On February 26, 1980, R. W. Wright, NRC-0IE Region II, was informed that NCR BLN NEB 8002 was determined to be reportable in accordance with 10 CFR 50.55(e). This was followed by interim reports dated March 27 and May 13, 1980. Since that time, related NCR 1316 has also been determined to be reportable in accordance with 10 CFR 50.55(e). This was followed by our combined interim report dated February 9,1981 and our final report dated April 30, 1981. As discussed with E. H. Girard by telephone on July 23, 1982, enclosed is our revised final report. The reason for this revision is to provide additional information to clarify the report. We consider 10 CFR Part 21 applicable to this nonconformance.

BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 POTENTIAL DEFECTS IN WKM VALVES NRC'S BLN NEB 8002 AND 1316 10 CFR 50.55(e)

REVISED FINAL REPORT Description of Deficiency Casting defects were found in isolation valves supplied by WKM, Division of ACF Industries in Houston, Texas, to Washington Public Power. The valves were manufactured by WKM to the requirements of ASME III, Class 1 and 2.

B&W has supplied TVA with four WKM valves for use on Bellefonte as control valves. The Bellefonte valves are the control valves in the decay heat cooler outlet (valve mark numbers 1DH-HV3A, 1DH-HV3B, 2DH-HV3A and 2DH-HV3B). Although defects were found in isolation valves at Washington Public Power and the Bellefonte valves are control valves, TVA requested B&W to take action to ensure similar defects are not present in the Bellefonte valves. B&W provided instructions for onsite inspection of the four WKM valves in the DHR system. Evaluation of the four valves at Bellefonte has been completed and casting defects were found during the visual inspection of these valves. Nonconformance report 1316 was generated to document this deficiency. The cause of the deficiency is a breakdown in the vendors' QA program.

Safety Implications The valves are required to control the flow from the Decay Heat Removal (DHR) System to the Reactor Coolant System. Failure of these valves could result in failure of the DHR System to perform its intended function both during normal operation and emergency operation. This condition, had it remained uncorrected, could have affected adversely the safety of operations of the plant.

Correction Action TVA employees completed the inspection of valves 1DH-HV3A, and 1DH-HV3B to the requirements outlined by B&W. In order for the acceptance criteria to be met, it was necessary to grind selected areas of the bottom closure for valve 1DH-HV3B. This grinding was completed without infringement on minimum wall thickness. The bottom closure for 1DH-HV3A was replaced with a nea closure. The Bellefonte unit 1 valves have been reassembled and are awaiting disposition of NCR BLN NEB 8207, wnich has also been reported under 10 CFR 50.5S(e).

Valves 2DH-HV3A and 2DH-HV3B were inspected at Bellefonte. Because of the numerous indications observed, the valves were returned to WKM for repairs. Valve 2DH-HV3B is awaiting disposition of NCR 1315 which has also been reported under 10 CFR 50.55(e).

As these are the only WKM valves at Bellefonte and there are no other WKM valves in other TVA plants, no corrective action to prevent further recurrence is proposed.
