In response to the above captioned report, we have reviewed the informa-i tion contained there-in and find that it contains no proprietary information.

In addition, w: have evaluated the circumstances relating to the item identified as 78-30-01 in report 78-395/78-30, which dealt with a deficiency in the QC verification of calibration of termination tools. Our evaluation has produced the following results:

Cause i

Two inspectors were charged with inspections on the termination Cl tools. Due to confusion and lack of coordination between them, no inspections were made on the NRC noted weeks.

Immediate Corrective Steps Taken and Results 1

The Contractor's Log was checked to determine the status of the

i tools for the subject weeks. The records showed that all j

termination tools were calibrated. Also, all tools were re-j, turned within calibration in weeks subsequent to the subject weeks.

From this investigation, it was concluded that lack of inspection far the subject weeks had no effect on the cable terminations.

Corrective Steps Taken to Avoid Future Noncompliance The inspectors have been reindoctrinated to the requirements of the procedure. A program has been established to insure that inspections are carried out at prescribed intervals. This program consist of a chart posted at the entrance of the QC field office with type of inspections, frequency and inspector's initials.

It has been determined there was no effect on the quality of cable terminations due to our lack of inspections and corrective action has been accomplished as of this date.
