APPENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Co.
License No. CPPR-94 Based on the results of the NRC inspection conducted on December 11-14, 1978, it appears that certain of your activities were not conducted in full compli-ance with NRC requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
10 CFR Part 50, Appendix B, Criterion V, as implemented by South Carolina Electric and Gas Company (SCE&G) FSAR Section 17.1.5, states in part that,
" Activities affecting quality shall be prescribed by documented instruc-tions, procedures or drawings.
and shall be accomplished in accordance with these instructions, procedures or drawings." Maintenance of termina-tion tools per manufacturer's instructions is performed in accordance with Daniel Construction Company (DCC) Work Procedure (WP)-XIII-01 by contractor personnel. SCE&G Field Quality Control Procedure (FQCP) 7.1.2 requires an Electrical QC inspector review of the contractor's " Tool Control Program" records weekly to verify that the tools are being maintained in accordance with manuf acturer's instructions.
Contrary to the above requirements, on December 12, 1978, a review of
" AMP Tool Inspection Reports" between July 12 and December 8, 1978, revealed weekly inspections were not performed during the weeks beginning and ending on the following dates:
September 10-16, 1978 October 22-28, 1978 October 29 - November 4,1978 November 5-11, 1978 November 26 - December 2,1978 A review of contractor calibration records for several tools indicated that tools were within calibration during the periods listed.
This is a deficiency.
