INTRODUCTION This document summarizes the applicable rules and requirements for periodic system -
I pressure testing at Point Beach Nuclear Plant (PBNP) in accordance with the American 1
Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code,Section XI,
'i
" Rules for Inservice Inspection of Nuclear Power Plant Components," 1986 Edition. This document also outlines the proposed schedule for the implementation of the program, and lists the technical basis for each individual pressure test.
2.0 SCOPE This document is limited to periodic system pressure testing performed to sausfy the requirements of ASME Section XI, Tables IWB-2500-1 (Class 1 Piping /Compcaents), IWC-i 2500-1 (Class 2 Piping / Components), and 'IWD-2500-1 (Class 3 Piping / Components). It k not the intent of this document to include or cover the requirements for post-maintenance
/ post-modification pressure testing. Requirements for post-maintenance / post-modification
'j pressure testing are covered in Administrative Procedure PBNP 3.2.5, " Pressure Test Procedure."
Maintenance of this document is the responsibility of the Plant Performance Engineering 1
(PPE) Group - Inservice Engineering (ISE). An individual currently assigned to this group shall serve as the Pressure Testing Program Coordinator / Engineer. The PPE Group Senior Project Engineer shall be the approval authority for revision of the program document.
3.0 DEFINITIONS d

3.1 System Leakace Test

Ensure primary system pressure retaining integrity.
Frequency:
Following opening and closing of a component within the ISI class 1 pressure retaining boundary. Typically this is at the end of each refueling outage.

Purpose:
Verify pressure retaining integrity of a system or component that is not.
required to operate during normal plant operation.
Frequency:
Once each period (periods defined by IWB-2412, IWC 2412, and l
IWD-2412).
l Boundaries: Include only those pressure retaining components within the system l
boundary pressurized under the test mode defined by the valve alignment checklist and procedures associated with system tests.
Rules:
Test pressure shall be the test mode pressure or other parameters defined by the test procedure.
A ten minute hold time is required after attaining test pressure.
3.3 System Inservice Pressure Test

Verify the pressure retaining integrity of a system while the system is in service and at normal operating pressure.

Once each period (periods defined by IWB-2412, IWC-2412, and IWD-2412).

Test pressure shall be the normal operating pressure.
Normal operating pressures are defined by:

- 1) Operating Instructions (OIs)
- 2) Other written operating procedures
- 3) Isometric Drawings such as the "P" drawings
- 4) NRC IE Bulletin 79-14 Final Report No holding time is required, provided the system has been in operation for at least four hours. The test procedure will document that the system has been in operation for four hours prior to examination.
3.4 System Hydrostatic Pressure Test
Purpose:
Verify pressure retaining boundary integrity. This test is typically __
conducted during a plant shutdown at a pressure above nominal operating pressure or above the system pressure for which overpressure protection is provided.
Frequency:
Once each interval, near the end of the interval (intervals defined by IWB-2412, IWC-2412, and IWD-2412).
Boundaries: Defined by the ISI classification boundary.
Rules:
Hy'drostatic testing shall be performed in accordance with the following:
- 1) ISI Class 1 pressure retaining components will be tested at system i
leakage test pressures as allowed by ASME Code Case N-498. -
- 2) ISI Class 2 pressure retaining components will be tested at system functional or inservice test pressures as allowed by ASME Code Case N-498.
- 3) ISI Class 3 pressure retaining components will be tested at 110

Purpose:

operating pressure or above the system pressure for which overpressure protection is provided.

4.1 SCHEDULING The proposed schedule for the completion of the Code requirements is shown in the Appendices to this program, as discussed in Section 6.0.

4.1.1 ISI Class 1
~
I Table IWB-2500-1, Examination Category B-P, requires that a system leakage test be performed on ASME Code Class 1 pressure retaining components following each refueling outage. A system hydrostatic pressure test must be performed at or near the end of the 10-year inspection interval. The hydrostatic pressure test may be performed in lieu -
of the system leakage test.
4.1.2 ISI Class 2 Table IWC-2500-1, Examination Category C-H, requires that a system functional pressure test or a system inservice pressure test be performed on ASME Code Class 2 pressure retaining components each 40 month inspection period. A system hydrostatic \\ pneumatic pressure test must be performed at or near the end of the inspection interval. The hydrostatic \\ pneumatic pressure test may be performed in lieu of the system functional pressure test or an inservice pressure test.
4.1.3 ISI Class 3 Table IWD-2500-1, Examination Categories D-A, D-B, and D-C require that a system functional pressure test or a system inservice pressure test be performed on ASME Code Class 3 pressure retaining components each 40 month inspection period. A system hydrostatic \\ pneumatic pressure test
'l must be performed at or near the end of the inspection interval. The hydrostatic \\ pneumatic pressure test may be performed in lieu of the system functional or inservice pressure test.

4.2 REOUIREMENTS ASME Section XI, Articles IWA-2000 and IWA-5000 identify the general i
requirements for pressure testing. ASME Section XI, Articles IWB-5000, IWC-5000, and IWD-5000 define the requirements that are specific to ISI Class 1, 2, and 3 respectively.

4.2.1 Examination Item Number
~
The Pressure Testing Program does not reference the examination item number determination as described by ASME Section XI, Table IWx-2500-1 for the purpose of tracking pressure tests. However, examination i
categories have been assigned in' the Unit I and 2 Program Schedules I
(Appendices A and B) that link the pressure test procedures to the applicable paragraphs of Section XI. All of the pressure test procedures that comprise the pressure test program meet all of the requirements of f
ASME Section XI, including the requirements of individual item numbers.
l 4.2.2 ASME Code Case N-498 i
ASME Code Case N-498, " Alternative Rules for 10-Year Hydrostatic Pressure Testing for Class I and 2 Systems" allows ISI Class 1 and 2 l
hydrostatic pressure tests to be conducted at the normal system operating j
pressure.
i 4.2.3 Authorized Inspection Agency O
PBNP has retained the services of an authorized inspection agency to provide inspection services in accordance with IWA-1400(f). The Authorized Nuclear Inservice Inspector (ANU) shall witness or otherwise verify the performance of system pressure tests within the scope of this i
program. In addition, the ANII shall review every completed pressure test / test document package. The ANII review shall be documented and retained with the pressure test / test document package.
The ANII shall be notified in advance of all upcoming pressure tests in accordance with IWA-2130. Such notification should be not less than 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> prior to the performance of any test.
4.2.4 Qualified Personnel All system pressure tests referred to in this document will have a VT-2 visual examination, as discussed in IWA-5240, performed by qualified personnel having a certification of Ievel II or level III. Department maintains qualified personnel on staff and/or arranges for the services of qualified personnel under contract.

4.2.5 Test Equipment Calibration All pressure gauges used for system hydrostatic pressure tests and system pneumatic pressure tests, shall.
- a. Be accurate to within 0.5 percent of full scale.
- b. Be calibrated within 14 days of the test.

- c. Have an indicating range of at least one and one half (1.5) times, but not more than four times the maximum test pressure,
- d. Be recalibrated upon completion of the hydrostatic test to verify data i
obtained is accurate.
System leakage tests, system functional pressure tests, and system inservice pressure tests do not have test gauge calibration requirements.
Normally installed system instrumentation may be used in the performance of these tests.
4.2.6 Test Pressures
-l The maximum test pressure at any point within the system or portion of a i
system being tested during a hydrostatic pmssure test (i.e., the low point pressure due to hydrostatic head) shall not exceed 106 percent of the test j
pressure unless the appropriate engineering evaluation has been completed prior to the performance of the test. Overpressurization may be prevented by subdividing the system. If it is impractical to subdivide the system to prevent overpressurization, the piping and components at the lowest part of' the area being tested will be reviewed by the Pressure Testing Program Coordinator / Engineer to determine if the test pressure is acceptable.

4.2.7 Test Temperature The hydrostatic pressure test temperature for systems constructed of ferritic steel shall satisfy the requirements of fracture prevention crite-ia.
Components constructed entirely of austenitic steel or ferritic steel and have a nominal wall thickness s 0.625" are not required to satisfy -
fracture prevention criteria. The temperature-pressure criteria for the reactor coolant system and the steam generators are addressed in the PBNP Technical Specifications and vendor literature. For carbon steel piping with a nominal wall thickness 2 0.625", (piping classification DB 2 10" nominal pipe size, EB 2 16" nominal pipe size, and 2503 2 5" nominal pipe size), the test temperature shall be 2 70*F. For other components such as tanks, heat exchangers, and pump casings, refer to the applicable vendor manuals for the minimum hydrostatic pressure test temperatures.
Additional information on the lowest service metal temperature may be found in ASME Section III, Nx-2300 and Appendix R.
-l The test fluid shall be warm enough to prevent the component from sweating so that leakage can be identified. If no guidance is available, the I
minimum test fluid temperature shall be 70*F.
4.2.8 Buried Components In nonredundant systems where the buried components are isolable by -
means of valves, the visual examination shall consist of a leakage test that determines the rate of pressure loss. Alternatively, the test may determine the change in flow between the ends of the buried components. In both cases, the acceptable rate of pressure loss or flow shall be established b'y the Pressure Testing Program Coordinator / Engineer, or a designated alternate, and documented in the pressure test basis prior to the performance of the test. In nonredundant systems where the buried components are nonisolable, such as return lines to the heat sink, the visual examination shall consist only of a verification that the flow lduring operation is not impaired. In redundant systems where the buried components are nonisolable, the visual examination shall consist of a test I
that determines the change in flow between the ends of buried components, j

THIRD INTERVAL PROGRAM DISCUSSION 4.2.9 Tanks Piping from a tank to the first isolation valve is considered to be an extension of the tank. In cases where a check valve in series with the first.
4 isolation valve prevents pressurization of that portion of piping between the valves, the tank boundary will'be extended to the first valve'that will i
allow pressurization of the connecting system classified for ISI without overpressurizing the tank. Open-ended portions of suction or drain lines from a storage tank are considered to be an extension of the tank.
When performing a hydrostatic pressure test of an atmospheric storage ~
tank, the pressure developed due to the static head when the tank is filled to its design capacity will ba acceptable as the test pressure. For a 0-15 psi storage tank (i.e., a tank that is not vented to atmosphere), the test pressure will be 1.10 times the design pressure of the vapor or gas space above the liquid level for which overpressure protection is provided by the relief valve. Tanks with a design pressure greater than 15 psi will be tested using the applicable system hydrostatic pressure test rules.'
4.2.10 Open ended piping Hydrostatic pressure tests on open ended portions of discharge lines beyond the last shutoff valve in non-closed systems (eg. containment spray 3
header) shall demonstrate an open flow path. Demonstration of an open flow path is not required during system functional pressure tests. ASME Section XI,- Article IWA-5222 requires the pressurization and examination of only those pressure retaining components that are pressurized in the test 1
mode. Therefore, these portions of ISI class systems will be tca:d only once, near the end of the inspection interval.
4.2.11 Insulated Components Pressure retaining bolted connections in systems borated for the purpose of-controlling reactivity shall have insulation removed for VT-2 examination.
Other components may be examined without removal of insulation by examining accessible and e.xposed surfaces and joints, as discussed in IWA-5242.

THIRD INTERVAL PROGRAM DISCUSSION 4.2.12 Pumps and Valves For the purposes of hydrostatic pressure testing of centrifugal pumps, the low pressure piping boundary shall terminate at the first isolation valve on the discharge side of the pump. Likewise, the high pressure piping boundary shall begin at the first discharge isolation valve.
Where leakages from components are normally expected and collected (such as valve stems and pump seals), the visual examination shall be conducted by verifying that the leakage collection system is operable.
4.2.13 Containment Penetrations While all containment penetrations are included within the Pressure Test Program, not all are actually tested under the program. Containment penetrations which are not part of larger ISI classed systems are tested under the 10 CFR 50, Appendix J, testing program, as discussed in relie.

4.3 BOUNDARIF3
- (
The ISI classification boundaries used to develop and implement this program are defined in the " Basis for ASME Section XI Classification Boundaries" for the Third Inspection Interval (Units 1 and 2). There are two types of boundaries that are used when describing a pressure test. These are the " pressure retaining boundary" and the 4
" examination boundary." The pressure retaining boundary is generally defined by the location of the ISI Class boundary valves within the system. These valves are then shut if needed to provide a pressure retaining barrier. The extent of the examination boundaries vary due to the ISI Classification and type of pressure test being performed.
Establishing appropriate pressure boundaries which conform exactly to examination boundaries cannot always be done while adhering to the principles of ALARA and maintaining safety. For these instances a request for relief will be prepared and submitted in accordance with 10 CFR 50.55a(g)(5).

the system boundary containing pressurized reactor coolant under th'e plant mode of normal reactor startup. (i.e., No special valve line-up is required.)
- b. Hydrostatic Pressure Test The boundary will extend to the ISI classification boundary within the system. This means that when two valves are in series, the inner valve must be opened to pressurize piping and components to the extent of l
the ISI class boundary. (i.e., A special valve line-up is required.)
4.3.2 ISI Class 2
- a. Functional Pressure Test j
The boundary will include only those pressure retaining components within the system boundary pressurized under the test mode required during the performance of a periodic system (or component) functional test. (i.e., No special valve line-up is required.)
- b. Inservice Pressure Test The boundary will include those pressure retaining components within the system boundary that are pressurized under operating pressures during normal system service. (i.e., No special valve line-up is required.)
- c. Hydrostatic Pressure Test The boundary will extend to the ISI classification boundary valves within the system or each portion of the system within which the components have the same minimum classification and design pressure.
This means that all systems or portions of systems that are classified for ISI will be appropriately pressurized and examined. Therefore, valve positions may need to be altered in order to perform this pressure test.

4.3.3 ISI Class 3 c
- a. Inservice Pressure Test The boundary will include those pressure retaining components within the system boundary that are pressurized under operating pressures during normal system service. (i.e., No special valve line-up is required.)
- b. Hydrostatic Pressure Test The boundary will extend to the ISI classification boundary valves within the system or each portion of the system within,which the l
components have the same minimum classification and design pressure.
This means that all systems or. portions of systems that are classified

4.4 DOCUMENTATION Inservice Test procedures (ITs), Operating Refueling Tests (ORTS), and Tech Spec Tests (TS) are used to implement the system pressure test program at PBNP. They are also used to document the performance of the test. Other working documents, such as Maintenance Work Requests (MWRs), may be utilized to document test performance at the discretion of the Pressure Testing Program Coordinator / Engineer, i

PRE-TEST REVIEW AND TEST PREPARATION Prior to conducting an ASME Section XI periodic pressure test, the test procedure shall be-reviewed by the Pressure Testing Program Coordinator / Engineer, or a designated alternate, to ensure compliance with ASME Section XI requirements. The Pressure Testing Program Coordinator / Engineer, or a designated alternate, shall review the test valve line-up to ensure that all portions required to be examined by the procedure will be pressurized as required.
i Development of actual testing procedures should be accomplished by Operations personnel knowledgeable in the operation of the subject system, with technical support provided by the Pressure Testing Program Coordinator / Engineer, or a designated alternate. PROGRAM DISCUSSION 8.0 ACCEPTANCE CRITERIA Acceptance criteria for each pressure test shall be specified within the test procedure or test working document. The Pressure Testing Program Coordinator / Engineer, or a designated alternate, shall ensure that appropriate acceptance criteria is set forth for each test during his pre-test review. In general, acceptance criteria for each pressure test shall specify that the test is to be considered acceptable as long as the pressure retaining piping and components within the test boundary are able to withstand test pressure with no loss of structural integrity. Additionally, acceptance criteria will also generally specify that the test is to be considered acceptable as long as there is no leakage from the system other than controlled leakage or leakage through boundary valves.
The term " leakage" has been defined by the Point Beach Manager's Supervisory Staff (MSS) to be " wet, spraying, or steaming." The term " residue" is used to describe dry material deposits, such as boric acid, and may not have the same acceptance criteria that is specified for leakage.
Since the pressure tests accomplished under this program do not measure or record any numerical data, no numerical acceptance criteria need be specified.
The Pressure Testing Program Coordinator / Engineer, or a designated alternate, shall review the results of each pressure test within 96 hours0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br /> of completion. This review shall compare the test results obtained to the specified acceptance criteria, and note any corrective actions which may be required.

An indication of "TBD" denotes that the procedure number has not yet been assigned. An indication of "MWR" denotes that the test is accomplished under a maintenance work request and associated work plan. " APP J" denotes that testing is accomplished under the 10 CFR 50, Appendix J, testing program.

.40 = 40 month interval 120 = 120 month interval (once during ten-year interval).
i PLANT Special plant conditions required to complete the test procedure.
CONDITIONS A = No Special Conditions Required B = Refueling Outage C = Defueled j
DRAWINGS The Primary P&ID drawings of the system / component to be tested.
OUTAGE Relates to the designated outages within the inspection interval. Note that test SCHEDULE procedures which due not require an outage condition to accomplish may be e

The source of leakages detected during the conduct of a system pressure test shall be located and evaluated by the Owner for corrective measures as follows:
(2) ifleakage occurs at a bolted connection, the bolting shall be removed, VT-3 visually examined for corrosion, and evaluated in accordance with IWA-3100; PROPOSED ALTERNATE REOUIREMENT:

(i) If the bolting material is fabricated of a material other than an austenitic stainless steel (e.g. low alloy B-7, etc.), one bolt shall be removed, VT-1 examined, and evaluated in accordance with IWA-3100. He bolt selected shall be the one closest to the source of leakage. When the removed bolt has evidence of degradation, all remaining bolting in the connection shall be removed, VT-1 examined, and evaluated in accordance with IWA-3100. The cause of the leakage shall be corrected. An evaluation of the effect of the leakage on adjacent components and equipment shall be made.
NOTE - All removed bolting shall be subject to a surface examination when required by Wisconsin Electric's response to IE Bulletin No. 8242.
(ii) If the botting material is fabricated of an austenitic stainless steel material a VT-3 examination of the bolted joint shall be performed and evaluated in accordance with IWA-3100. In addition, an examination of adjacent components and equipment will be conducted to determine the effects of the boric acid. He leakage shall be evaluated in accordance with the requirements of the Owner's limits acceptable for continued service, ne evaluation shall consider the effects of the leakage on adjacent components and equipment The cause of the leakage shall be corrected when the Owner's limits for continued service are exceeded or when practical.

(i) The bolted connection shall be VT-3 examined and evaluated in accordance with the

requirements of IWA-3100. The leakage will be evaluated in accordance with the requirements of the Owner's limits for continued service. He evaluation shall consider the effects of the leakage on adjacent components and equipment. The cause of the leakage shall be corrected when the Owner's hmits for continued service are exceeded or when practical.
BASIS FOR RELIEF:
The leakage of boric acid onto carbon and lov alloy steel bolting is known to cause excessive corrosion rates. The attemate examination requirement will reci: ire evaluation ofleakage from bolted connections of boric acid systems.
The bolt closest to the leakage will bc the bolt that exhibits the maximum corrosion and degradation. Therefore, examination of this bolt will be a conservative representation of the remainder of the bolts in the bolted connection. If the bolt shows evidence of degradation all remaining bolting in the connection shall be removed and examined. His is consistent with the reouirements of IWA-5250 of the 1989 edition of ASME Section XL A VT-1 examination, in lieu of the ASME Section XI requirement of a VT-3 examination, is a more accurate and qualitative examination of bolting. By removing only one bolt of a bohed connection that exhibits leakage significant man-rem may be saved without compromising the integrity of the bolted connection or the pressure boundary.
Leakage of boric acid on austenitic stainless steel bolting does not indicate a great potential for excessive corrosion. A VT-3 examination of the bolted connection will adequately determine any degraMon of the bolted connection or of the

pressure boundary as a result of the leakage from that connection. Evaluation of the effuts of the leakage on adjacent components and equipment is above and beyond the requirements of ASME Section XI nd serves to further insure that the leakage has no adverse effects on surrounding components.
Technical Specification requirements have been established to permit acceptable limits of leakage. Leakage at a bolted connection does not indicate failure of a pressure boundary. Ifleakage occurs at an austenitic sialnless steel bolted connection that does not exceed these requirements, then continued operation shall be permitted provided the leakage does not affect adjacent components or equipment and that the integrity of the pressure boundary is not affected.
For those systems that do not contain boric acid, leakage at a bolted connection does not indicate a great potential for excessive corrosion of the bolting as a result of the leakage. By conducting a VT-3 examination of the bolted connection, evidence of any corrosion / degradation of the bolting or bolted connection can be adequately made to insure the integrity of the pressure boundary.

Awaiting NRC Response. Request originally submitted to the NRC as ISI Program Relief Request RR-1-15 (Unit 1) and RR-2-16 (Unit 2) on March 4,1993 (VPNPD-93-056/NRC-93-032). Request incorporated into Third Interval Pressure Testing Program, Revision 0, with the issuance of that document.

PROPOSED AITERNATE REOUIREMENT:
ASME Section XI pressure testing requirements for ASME Class 2 containment penetrations associated with non-ASME classed systems shall be satisfied by the testing performed to meet the requirements of 10 CFR 50, Appendix J.
Containment penetrations included within larger ASME classed systems shall be pressure tested normally in accordae:e with Code requirements as part of the larger system pressure test.
I BASIS FOR RELIEF:
All containment penetration piping, regardless of whether or not the penetration is part of a larger ASME classed system, is classified as ASME Class 2. For those containment penetrations which are not part of larger ASME classed systems, the only function important to safety associated with the ASME classification of this piping and/or conponents.
