l Office of Nuclear Reactor Regulation Items of Interest Week Ending January 14, 1994 NRC/INP0 Coordination Meetina On January 12, 1994, representatives from the Division of Reactor Controls and Human Factors met with representatives from the Institute of Nuclear Power Operations (INPO) to discuss training-related activities. These periodic meetings are held to coordinate NRC and INP0 training-related activities and to share information in an effort to increase overall training effectiveness and to avoid unnecessary duplication of activities. Topics under discussion included implementation of the Training Rule (10 CFR Part 50.120), NRC treatment of INP0-accredited facilities with training programs on probation, and feedback from the NRC requalification program inspection program.

System 80+

On January 7, 1994, the Probabilistic Safety Assessment Branch completed its review of the System 80+ PRA and transmitted its final safety evaluation report (FSER) to the Division of Reactor Projects. The estimated core damage frequency (CDF) for System 80+ is about 2E-6 per reactor year for internal events.

CDF estimates for external events were not generated since margins-type methods were used to assess the risk from these challenges. The conditional cor.tainment failure probability is about 11%, with the majority of failures coming from containment basemat melt-through.

FSER on the PRA, staff evaluations of Severe Accident Mitigation Design Alternatives and Accident Management for System 80+ were also transmitted.

1 Meetina with Naval Reactors A meeting was held at Crystal City on January 7,1994, between members of the-j Reactor Systems Branch (SRXB) and officials from Naval Reactors and

.i Westinghouse-Bettis Laboratory to present the design and discuss questions raised in SRXB'S ongoing review of the Naval reactor project. The meeting was highly informative and effective in resolving the majority of questions in the areas of reactor design and fuels.

Due to the long agenda, an addi_tional meeting will probably be scheduled to expedite resolution of remaining questions in other areas within the SRXB scope. The SRXB request for i

additional information is being revised based on the information gathered at the meeting prior to being issued formally. No major problem areas likely to impact the review schedule were identified during the meeting.

ACRS Meetina on ATWS/ Stability Issues On January 6, 1994, the Reactor Systems Branch staff met with the ACRS full committee to discuss the remaining issues and status of the ATWS/ Stability review and related Emergency Procedure Guideline (EPG) issues. We informed them of our plans and schedule to proceed with issuing our safety evaluation report on the review of these issues. A Commission paper, based on tue report, will also be issued (by RES). 2 concerning final water level control and boron mixing will continue independeret of the stability issue. The Committee did not object to our plans, but a subcommittee meeting for more detailed review of the latter issues will be scheduled soon.

Revised Part 20 Violationg On January 6, 1994,-Radiation Protection Branch issued the first summary package of revised 10 CFR Part 20 notices of violation (NOV). This summary of NOV's was distributed to the Regions to ensure uniform implementation of the revised Part 20.

Copies of the package were sent to RES, NMSS and the public document room (industry /NUMARC expressed interest in this information).

Fermi-2 As a precaution, the licensee has recalculated the results of a potential seismic event. The results of this analysis are that~ if the postulated event were to occur the activity at the Monroe water intake structure would be 6E-02 of the maximum permissible concentration (MPC). The analysis in Chapter 15 of the Final Safety Analysis Report for the same seismic event concludes that the activity would be 3E-03 of the MPC. The licensee is evaluating the potential for water to seep into the ground water system.

The turbine vendor, English Electric, has also performed an analysis indicating that there may be potential hydrogen pockets in the generator which could ignite. The licensee is currently trying to restore their C02 system in order to purge the generator prior to allowing access to the immedicte generator vicinity.

The licensee will be entering the hotwell of the main condenser to remove debris and prepare for transfer of Radwaste Building basement water to the hotwell.

An additional problem arose with reactor coolant conductivity cleanup efforts.

The licensee had estimated that it would take about 2 weeks to reach the administrative limit of 2R/hr contact dose reading on the portable demineralizers in use to restore conductivity. The limit was exceeded (2.2R/hr contact) during the first 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> of operation. The licensee has temporarily suspended the cleanup operation until they can investigate further.

The full augmented inspection team arrived onsite Monday, January 10, 1993.

They are continuing to monitor the licensee's actions for water management and investigation of the event. A member of the Michigan Public Service Commission is onsite to observe.

Prairie Island Installation of the Independent Spent Fuel Storage Installation (ISFSI) at the Prairie Island Nuclear Generating Plant requires Minnesota State Legislative ap,mt:ye?

The state legislature plans on convening in February 1994.

During the past few months, the Minnesota Senate has been holding subcommittee JANUARY 14, 1994 ENCLOSURE A

3 hearings to listen to supporting and opposing testimony for the ISFSI.

This month the Minnesota House of Representatives started conducting similar hearings. The licensee estimates that approximately 30 percent of the legislators are firmly against the dry cask storage project and 30 percent of the legislators support it. The remaining 40 percent appear undecided.

Interest in the debate has increased considerably among local area residents due to the' realization that the legislature could disapprove the ISFSI, which-would likely result in permanent plant shutdown.

Several national groups are' involved in opposition efforts. These include Greenpeace, the Union of Cacerned Scientists, and the Sierra Club.

In addition, local chapters of the Audubon Society and various local church groups have taken a position against dry cask storage.

The project manager and senior resident inspector have de; eloped a detailed inspection plan for the ISFSI for use in verifying that licensing requirements have been met.

Inspection activities will include a review of cask fabrication, onsite storage facilities, cask handling equipment, ISFSI security, preoperational testing, and actual cask loading operations.

The plan also addresses routine follow-up inspections.

Oconee Unit 3 - Missina Steam Generator Pluas Oconee Unit 3 is presently defueled for a regularly scheduled refueling outage. After defueling, the licensee discovered eight tube plugs in the Sottom of the reactor vessel.

An inspection was performed of the upper and lower tube sheets of both steam generators on January 11, 1994. The licensee found 14 S/G plugs missing from the B S/G lower head. The licensee also found some leaking tube plugs. Duke Power uses B&W rolled plugs. All of the missing and leaking tube plugs were installed by a particular B&W crew in one shift.

The licensee is continuing their inspection to locate the remaining i

plugs, determine cause of failure, and identify corrective action.

Oconee Units 1 and 2 continue to operate at full power.

TVA On January 6, TVA named Oliver Kingsley, Jr. as President, TVA Nuclear and Chief Nuclear Officer, reporting directly to the TVA Board, with responsibility for all nuclear power activities. Mr. Kingsley was previously President of the Generating Group. The new organization is effective on February 7.

Kingsley has been tasked with performing a study over the next year to examine the possibility of separating TVA's nuclear power program as a 4

separate business unit.

Office of Nuclear Material Safety and Safeguards items of Interest Week Ending January 14, 1994 l

Vehicle Barrier Final Rule Staff mernbers of the Division of fuel Cycle Safety and Safeguards are supporting the Office of Nuclear Reactor Regulation's (NRR's) Safeguards Branch in analysis of comments received on the Vehicle Barrier Proposed Rule.

The final rule, which will apply to power reactors, is expected to involve a change in the Design Basis Threat and is being handled on an expedited basis.

Safeauards Reouirements for Geoloaical Reoository Staff of the Divisions of Fuel Cycle Safety and Safeguards and H' igh-Level Waste Management are formulating a response to a letter from the Department of Energy requesting clarification of the Nuclear Regulatory Commission's safeguards and physical security requirements for the Geological Repository.

t Because of the age of the spent fuel to be sent to the repository, and the difficult reprocessing that would be necessary to extract weapons grade plutonium from it, requirements for protection against theft of special nuclear material or for protection against radiciogical sabotage are expected to be minimal.

The dominant safeguards requirements are expected to be those necessary to satisfy U.S. agreements with the International Atomic Energy Agency.

Claiborne Enrichment Center. Homer. Louisiana The manuscript for the Safety Evaluation Report (SER) for the Claiborne Enrichment Center, NUREG-1491, has been completed.

The SER will be issued as a final document by the end of January 1994. A press release and a Federal Recister notice will announce availability of the SER when it is issued.

The Draft Environmental Impact Statement (DEIS) on the Claiborne Enrichment

.I Center was published in November 1993.

The public comment period on the DEIS, which was extended by 15 days, closes on January 25, 1994.

The Final Environmental impact Statement is scheduled for publication in August 1994.

The staff met with representatives of the U.S. Department of Agriculture l

(USDA) and Chem-Nuclear Systems, Inc. on January 11, 1994.

The subject was

USDA's transportation quality assurance program for the USDA irradiators used for insect control.

USDA informed the NRC that they intend to arrange for Chem-Nuclear to conduct quality assurance activities on. behalf of the certificates of compliance for USDA's transportation packages.

Chem-Nuclear and USDA expect to submit applications within the next few months.

Human Research Information The Office of Nuclear Material Safety and Safeguards forwarded to the Department of Energy information from files maintained by the Nuclear Regulatory Commission concerning the Atomic Energy Commission's licensing of JANUARY 14, 1994 ENCLOSURE B-1

2 the Walter E. Fernald School (now known as the Eunice Kennedy Shriver Center for Mental Retardation), Waltham, Massachusetts. The transmittal included three documents:

(1) A copy of the human use license issued by the AEC in 1965 (expiring in 1968) and backup information submitted to support the license request; (2) a copy of the current NRC license for the Eunice Kennedy Shriver Center which does not authorize human studies; and (3) a short regulatory sunnary of the two Fernald School AEC licenses (the human use and an animal use license) and the Eunice Kennedy Shriver Center AEC/NRC license continuing the animal use studies.

Observation of the U.S. Department of Enerav (D0E) 90 Percent Desian Review for Exploratory Studies Facility Packaae 2B j

Division of High-Level Waste Management (HLWM) staff observed a U.S.

Department of Energy (D0E), Office of Civilian Radioactive Waste Management, 90 percent design review for Exploratory Studies Facility (ESF) Package 2B in Las Vegas, Nevada, January 5-7, 1994.

Package 2B design includes the North Ramp studies, analyses, and specifications. The purpose of the design review i

was to provide a complete overview of the design package and process, followed by a formal presentation of the package by the designers, covering the input i

used, the assumptions made, applicable codes and specifications, any analyses, and the resulting design output. The scope of this design review included procurement studies for muck storage, excavation methods, ESF ventilation, l

surface conveyer system, drawings and specifications for precast concrete j

invert segments, rail haulage system specifications, tunnel boring machine platform specifications, and communication systems.

The Nuclear Waste Technical Review Board, State of Nevada, and affected units of local government provided reprer,entatives to observe this design review.

Based on limited observations of the design review, the HLWM observers provided verbal and written comments to D0E. The comments related generally to the effectiveness and adequacy of the design interface among design disciplines of the Management and Operating Contractor (M&O) and the M&O/ DOE contractors prior to the 90 percent design review. DOE responses will be provided by the end of January 1994.

f On January 7, 1994, a proposed rule to amend 10 CFR 50.55a, Codes and Standards, was published in the Federal Register for a 75 day public-comment period. _ The proposed rule would amend these regulations to incorporate by reference the 1992 Edition with the 1992 Addenda of Subsection IWE, " Requirements for Class MC and Metallic Liners of Class CC Components of Light-Water Cooled Power Plants,"

and Subsection IWL,

" Requirements for Class. CC Concrete Components of Light-Water Cooled Power -

Plants," of Section -XI, Division 1, of the ASME Code.

Endorsement of these subsections at this time is considered necessary because corrosion and degradation of containments has occurred and the rate of occurrence has been increasing.

The proposed rule would require licensees to expadite implementation of the Subsection IWE and Subsection IWL containment examinations by completing the j

first examination within 5 years of the effective date of this rule.

This expedited examination schedule is necessary to prevent a delay in the implementation of Subsection IWE and Subsection IWL and to establish an early baseline for future examinations.

IRB will evaluate it over the next few months for adequacy of systems operation and data.

Office of Administration Items of Interest Week Ending January 14, 1994 Eneray Policy Act Meetina i

On January 10, 1994, staff from the Office of Administration attended the Department of Energy's Retention of Savings meeting to discuss issues involving the Energy Policy Act of 1992 [Public Law 102-486]. This Act encourages increased conservation and energy efficiency by government, industry and consumers, to reduce the Nation's cumulative energy demand by the equivalent of about eight billion barrels of oil between now and the year.

2010.

The Act is also estimated to increase the use of renewable energy by more than 20 percent and alternative fuels by more than 50' percent over projected 2010 levels. The next meeting will be held in April 1994.

l Contract Awards On January 12, 1994, a contract was awarded on a competitive basis to Scitran Company of Santa Barbara, CA, at an estimated cost of $292,446 for foreign language document translation services.

The period of performance of the basic ccatract is two years with three one-year options available to the Government.

A contract was awarded to IDC Government on December 14, 1993 entitled, "IRM.

Research and Advisory Services." The contractor shall provide research and advisory services to include current information on a wide range of computer related topics, the creation of specific reports, market analysis material, hardware and software evaluations, and user access to publications.

The contract was awarded for $85,590 and the period of performance is for one year.

-t On January 11, 1994, the Office of Information Resources Management (IRM).

responded to the National Archives and Records Administrat'on's (NARA) request for the permanent transfer of 1,557 cubic feet of. NRC records to NARA.

Consistent with the NRC Comprehensive Records Disposition Schedule, IRM transferred only 142 cubic feet of NRC records. The NRC requested that 133 cubic l

feet of records be retained at the Washington National Records Center pending i

further review.

Additionally,1,254 cubic feet were ineligible for transfer because their required retention period had not been met, and 28 cubic feet of records were not eligible for transfer pursuant to Section 10(b) of the Federal Advisory Act, which requires records of each advisory committee be made available for public inspection and copying at a single location in the offices of the advisory committee or the agency to which the advisory committee reports until i

the advisory committee ceases to exist.

Fiber Optic Transmission System The procurement of the fiber optic transmission system between the two White Flint buildings has been completed.

Although the estimated delivery and installation date is February 10, 1994, the Telecommunications Branch is attempting to accelerate a delivery schedule.

7 URF0 Records Transfer to Headouarters The Uranium Recovery Field Office (URF0) will transfer their records to the.

i Office of Information Resources Management File Center as originally scheduled beginning with the first shipment occurring on February 15, 1994.

The File Center is identifying space to accommodate the URF0 records that will be transferred prior to the move to Two White Flint North (TWFN) and has revised its TWFN plan to accommodate the additional collection.

Office of Enforcement Items of Interest Week Ending January 14, 1994 Sianificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $25,000 was issued on January 13, 1994 to Duke Power Company. This action l

was based on the violations identified as a result of the follow-up of a steam leak in the Unit 2 containment on August 31, 1993..The violations, which were caused by personnel error resulting from inadequate control of maintenance and operations activities, have been classified as a Severity Level III problem.

Civil Penalties Paid Entergy Operations, Inc. (Waterford 3) paid the civil penalty in the amount of 525,000.

The action was based on two violations associated with the inoperability of one of the two trains of containment spray due to a header -

J isolation valve that was unable to open upon demand when high differential pressure existed across the valve.

Northeast Nuclear Energy Company (Millstone, Unit 2) paid the civil penalty in the amount of $237,500. The action was based on the violations associated with the licensee's repeated attempts to repair a body-to-bonnet-leak on a letdown valve that was unisolable from the reactor coolant system.

Ultimately the repeated repair attempts resulted in the failure of one of the valve's i

four bonnet studs which led to a reactor shutdown when the technical specification limit for reactor coolant system leakage was exceeded.

Items of Interest Week Ending January 14, 1994 Second Buildina Status During the week, carpentry and electrical work continued in the auditorium complex. TWFN Freight elevator service was interrupted on January.13 and 14 by an electrical control system anomaly, which backed up delivery of furniture and demountable walls to the upper floors.

On January 10, the Director and staff met with representatives of the furniture.

i and demountable wall manufacturers and installers and the electrical contractor to discuss various coordination issues.

On January 11, the Director met at GSA's regional office with the head of the Concessions unit and his staff together with the Plaza Level design team to discuss the design and layout of the proposed 300-seat cafeteria for TWFN. At that meeting, the design presented by the Weihe Partnership and Cini-Little,-

International was accepted with suggested revisions. This meeting completed the preliminary space planning phase for the plaza level.

On January 12, the Director met with the incoming Employee Welfare and Recreation Association President and other board members to discuss the design of the EWRA store for TWFN.

On January 13, the Director met with NTEU stewards to discuss space plans for TWFN and to provide the stewards with drawings of Floors 4-10.

On January 13, CONS staff and GSA staff met at GSA's Franconia, Virginia warehouse to begin evaluating ergonomic seating samples submitted by chair j

manufacturers and dealers.

the State of Ohio Department of Health on January 11, 1994 in Columbus, Ohio.

The. purpose of the meeting was to discuss with the State officials the requirements and process for obtaining Agreement State status.

State officials indicated that they anticipate submitting the application for an agreement, including authority for the regulation of low-level radioactive waste disposal, in 1998.

Roland lickus, Region 111 State Liaison Officer, and other Region 111 staff met with representatives.from the Illinois Department of Nuclear Safety (IDNS) and Commonwealth Edison on January 13, 1994 in Braidwood, 111inbis.

The purpose of the meeting was to discuss the status of the IDNS resident inspector program and issues associated with the resident program, including 3

staffing and training plans, risk-based inspection guides and involvement in emergency preparedness exercises.

NRC briefed on the status of activities at-Quad Cities and LaSalle.

Office of International Programs Items of Interest Week Ending January 14, 1994 Visit by Walter Hohlefelder. Germany Dr. Walter Hohlefelder, Director General for Nuclear Safety, Ministry of Environment, Nature Protection and Nuclear Safety, met with the Commissioners on Thursday, January 13. Topics discussed included results of the meeting of the G-7 Nuclear Safety Working Group, future direction of the G-24, assistance activities with the prospects for concluding an International Safety Convention, and the French / German safety approach for an advanced PWR.

Public Meetino at B&W Acollo On January 11, 1994, representatives from Region I and NMSS conducted an inspection exit meeting with the Babcock and Wilcox Company relative to the ongoing decommissioning activities at the Apollo facility and operations at the Parks Township (PA) site.

In accordance with a commitment made by the Chairman during a site visit in 1992, the exit meeting was open to public observation.

No violations were identified during the October 25-December 31, 1993 inspection period, but some areas for additional attention relative to housekeeping in the radiological areas at the Parks site and to completion of remediation activities at the Apollo site were noted.

Following a brief intermission, the NRC staff opened the meeting to questions from the approximately 25 members of-the-public who attended.

The questions from the public focused on community concerns about the perceived health effects from releases of uranium from the facility during the approximately 30 years of operation' by B&W and its predecessors, the Nuclear Materials and Equipment Corporation (NUMEC) and i

the Atlantic Richfield Company (ARCO). The NRC staff agreed to follow up on some specific questiuns raised by the public. The meeting lasted from 7:00 to 10:30 pm.

On January 13, representatives of Duke Power Company, including the Vice President, Nuclear Production Department and the Security Managers for each Duke site, were in Region 11 for an Enforcement Conference concerning recent failures to protect safeguards information.

During-i the conference, licensee representatives presented their analyses of the safeguards significance of each failure to control safeguards information and outlined their corrective actions.

Union Carbide Meetino (Previonsiv Terminated Licensee) f On January 13, a Branch Chief and other members of the Division of Radiation Safety and Safeguards met with representatives of UCAR Carbon

Company in Lawrenceburg, Tennessee, to. discuss the adequacy of the.

previous. decommissioning of the Union Carbide Lawrenceburg facility, _

l UCAR is owned by Union Carbide (50 percent) and Mitsubishi (50 percent).

This is the first of several meetings which will be held with p'.tviously terminated licensees for which there is inadequate documentation.to show that the f acilities meet NRC decontamination cr"eria. Two i
