HIGHLIGHTS OF WEEKLY INFORMATION REPORT Week Ending August 10, 1990 Installation of Hardened Wetwell Vents for Isolation Condenser Plants On August 7, 1990, Division of Systems Technology staff and other members of NRR met with Dr. Murley to discuss the staff course of action regarding the installation of hardened wetwell vents for isolation condenser Mark I contain-ment plants. Dr. Murley agreed that the request by the plant owners who had not yet voluntarily committed to the installation (0yster Creek, Millstone 1 and Dresden 2, 3) to delay this decision until completion of their Individual Plant Examinations was not substantiated by the additional supporting infor-mation provided; and that these utilities should be informed of the staff's intent for them to proceed without delay with the installation of the hardened wetwell vents.

Atomic Vapor Laser Isotope Separation (AVLIS) Briefing The purpose of the August 2, 1990, AVLIS briefing presented by the Lawrence Livermore National Laboratory (LLNL) was to familiarize the NRC with AVLIS technology, and the application of the technology in uranium enrichment.

LLNL discussed the Department of Energy's (DOE) proposed AVLIS production plant for low enriched uranium as it relates to safety, safeguards, security and other regulatory issues. Mr. Philip G. Sewell, Deputy Assistant Secretary for Uranium Enrichment, DOE, and several of his staff were also present at the briefing. Mr. Sewell indicated that DOE is definitely committed to proceed with the AVLIS initiative and they would want the NRC to perform a license-

ability review of the plant.

Evaluation of Low-level Waste Regulatory Program in February 1990, the Commission directed the staff to evaluate the NRC regu-latory program for low-level waste to confirm that it meets or exceeds the guidelines for Agreement State programs.

The staff issued proposed low-level waste guidelines for Agreement States for comment in the Federal Register in March, and a number of comments have been received and are being reviewed before the staff issues the final guidelines. As part of this process, the staff has begun its evaluation of its program against the proposed guidelines and will factor the results of the evaluation into the final guidelines.

A team composed of staff from NMSS, GPA, and Region 111 will be conducting the review, which is expected to be completed by the end of September 1990.

Cintichem, Tuxedo, New York Cintichem has a 5 MW pool reactor, hot cells and laboratory for production of radioisotopes for use in radiopharmaceuticals.

On February 23, 1990, NRC issued CAL No. 1-90-005, as a result of an incident involving the discharge of con-taminated water from the licensce's onsite retention pond to the Indian Kill Reservoir. The CAL included licensee commitments to be implemented in the event that heavy rains or surface run-off necessitated that the retention pond I

be emptied or lowered to preserve its integrity. On July 30, 1990, between approximately 1730 and 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />, heavy rains forced the licensee to begin continuous pumping from the retention pond to a point downstream of the Reservoir. On August 7,1990, at approximately 0010 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br />, the licensee notified Region I that continued heavy rains necessitated the diversion of surface run-off water from storm drains around the retention pond to a point downstream of the Reservoir. Appropriate monitoring and sampling of the dis-charge was performed.

Office of Nuclear Reactor Regulation items of Interest Week Ending August 10, 1990 Carolina Power & Light Company (CP&L) Organizational Change From telephone discussions with the CP&L licensing staffs for Harris and BrunswicP. and a formal organization change release by the South Texas Project, it was learned that the following organizational changes will take place at CF&L effective on the dates indicated.

Effective August 13, 1990, Gerald E. Vaugin will replace Alan B. Cutter Vice Presi u nt, as Manager, Nuclear Services.

In the CP&L organization, the title of Vice President must be bestowed by a vote of the Board of Directors.

Therefore, Mr. Vaughn will be a Manager, rlthough he is replacing Mr. Cutter, a Vice President. Mr. Vaughn is currently with the South Texas Project as i

Vice President, Nuclear Generation. Mr. Vaughn's home state is North

Carolina, in additior., Mr. Vaughn was with Duke Power Company and was on the INP0 board of directors and INP6 accreditation board.

Effective. August 13, 1990, Alan B. Cutter will assutb e newly created position of Vice President / Director of Special Nuclear Projects reporting to R. A.

Watson, Senior Vice President, Nuclear Generation Group. The scope of Mr.

Cutter's job is to deal with outside issues confronting CP&L.

'in Power Ascension Program - On July D. M O, supplement 1 to CAL 88-17 was issued authorizing the licensee to proceed with startup.

Criticality was achieved on July 29.

The plant was restarted on August 2 following the July 30 shutdown to correct drywell leakage caused by a damaged electromagnetic relief valve (ERV) downcomer vacuum breaker and leaking ERVs.

As.of August 6, the plant was operating at approximately 20% of rated power with 2 to 3 bypass valves open.

At 1732 hours0.02 days <br />0.481 hours <br />0.00286 weeks <br />6.59026e-4 months <br /> on August 6, the reactor was rer.ually scranned due to high vibration on a main turbine bearing. The vibra-tion was detected during preparations for placing the main turbine on-line.

Main condenser vacuum was broken to more quickly reduce turbine speed to prevent further damage to the bearing. The operators performed well during the event and all plant systems responded as required.

Preliminary indications' are that a blank flange, left in the oil supply line to the bearing, was the cause of the event.

The licensee expects the unit to be down for about a week to replace / repair the turbine bearing.

Augmented Startup Inspection Coverage is in effect.

Unit 2 The unit is at the 755 power level, coasting down to the first refueling outage.

Oyster Creek On August 6,1990, at 3:15 pm the licensee of the Oyster Creek plant notified HRC Region 1 of an Unusual Event which occurred during testing of the Oyster Creek containment spray system. The plant began a shutdown when unidentified leakage exceeded the 5 gpm Technical Specification limit.

It was subsequently postulated that the likely cause was a contair, ment spray isolation valve that 4

failed to f ully close during a flow test, resulting in 300 gallons of water sprayed into containment over a 21 minute period.. The operability of the MSIVs were varified when the plant was reduced in power to 40%. After reducing the leakage to within Technical Specification limits the-licensee terminated the unusual event at 5:50 pm.

During the course of the event, the containment was pressurized to 0.3 - 0.4 l

psig from the effects of added water vapor inside containment. The licensee.

subsequently vented the containment out of the reactor building exhaust system to the stack.

No increase ht the 1mr/hr background reading at the stack monitor was observed. Later in the evening after the event, State of New Jersey sensors located 1.4 miles northeast of the site detected a spike of 120 - 130 microrem/hr of radioactivity for about 2 minutes compared with the normal background reading of 5 - 10 microrem/hr.

A Comission Assistant's conference call was held on August 7,1990, to provide l

background and status on this event.

Region I inspectors have been dispatched to the site to review licensee actions, l

1 venting procedures, sequence of events, operability test results of affected plant equipment, and the-reasons behind the observed spike in the offsite radiation monitor. As of 11:00 am, August 8, 1990 the plant had returned to l

about 95% power.

-YAEC Meetint On August 7,1990, Yankee Atomic Electric Company (YAEC), the licensee of Yankee Rowe Nuclear Power Station, met with the staff in Rockville NRC offices to discuss issues relative to its analyses of Pressurized Thermal Shock and Low Temperature Overpressure Protection. During this meetir.g,

-YAEC committed to providing additional analyses and evaluation (by the end of this week) to support its conclusion that the Yankee Rowe design meets the requirements of 10 CFR 50.61. The staff will perform an assessment of the licensee's submittal.

supporting information provided; and that these utilities should be informed of the staff's intent for them to proceed without delay with the installation of the hardened wetwell vents, Public Meeting Between The Staff And The BWR Regulatory Response Group (EMEB) (Update) t On August 1, 1990, the staff held a public meeting with the BWR Regulatory Response Group (RRG) to discuss the staff's concerns regarding the capability of the RCIC, HPCI and RWCU containment isolation MOVs to properly function. At-the meeting, the staff presented an overview of its evaluation of the MOV data supplied by the BWROG as compared to the thrust required to close the valves during the NRC-sponsored tests. The BWR RRG presented a safety assessment assuming that the.MOVs might be unable to close under design-basis conditions.

The BWR RRG asserted that the safety significance of this issue is sufficie" low as to allow time to correct any identified deficiencies as part of the long-term program of Generic letter 89-10, " Safety-Related Motor-Operated Vahe Testing and Surveillance." The BWR RRG stated that the NRC-sponsored test data would be evaluated for applicability to the M0Vs currently installed in the HPCI, RCIC, and RWCU systems, but had no specific schedule for that evaluation or plans for action following that evaluation.

In light of the lack of any specific plans in this regard by the BWR RRG, the staff stated that it would consider the need to prepare a generic communication to the BWR licensees. The possible generic communication was discussed with the RRG, and it would contain the following elements for individual licensees to perform (1) plant specific safety assessment to demonstrate that the generic BWR RRG safety assessment is t

applicable, (2) determine the applicability of the test data to the individual plant's HPCI, RCIC and RWCU isolation valves (as well as the MOVs used in lines to the isolation condenser), (3) determine whether the MOVs are capable of performing their safety function, and (4) establish a schedule for the performance of any necessary corrective actions.

On August 6,1990, the International Atomic Energy Agency (IAEA) connenced a

. physical inventory verification (PlV) at the General Electric Low-Enriched Uranium fuel Fabrication Plant in Wilmington, North Carolina. Ten IAEA inspectors are on-site to carry out PIV activities which will include a ledger audit, item counts, nondestructive assay measurements, weighings, and sampling for destructive analyses. On-site activities are expected to be completed by the end of the week.

IAEA conclusions relative to the acceptability of the PIV are expected to be finalized in several months after all destructive e

measurement results are completed and evaluated.

Licensee Training for Use of Nuclear Materials Management and Safeguards Systems (NMMSS)

Thirty Nuclear Regulatory Commission (NC) licenseer participated in a NMMSS training course, August 7-10, 1990, in Gatlinburg, Tennessee. This course was last offered in July 1986. The focus was on the users' interactions with HMMSS, the basic fundamentals of the system and inprovements in the editing procedures. Class material included recently revised NRC instructions NUREG/BR-0006 Revision 3 and NRC instructions NUREG/BR-0007 Revision 2.

Representatives from the Offices of Nuclear Material Safety and Safeguards and Information Resources Management also participated in the course.

Safeguards Meeting With Louisiana Energy Services (LES)

On August 9, 1990, LES technical staff and NRC safeguards staff held a' pre-application discussion of the safeguards requirements for the' proposed LES enrichment facility. Guidance was presented on the preparation of their Fundamental Nuclear Material Control and security plans as required by Parts 73 and 74.

A license application from LES is expected by January 1991.

Package Supplier Inspection On July 30 - August 2,1990, Transportation Branch staff conducted an

-inspection at B&W Fuel Company, Lynchburg, Viqinia.

supported by two specialists from Idaho National Engineering Laboratory.

The inspection focused on quality assurance in the fabrication, maintenance and use of. transport containers for fuel pellets and fuel assemblies.

Preliminary findings were discussed with company representatives at the

exit meeting.

Nuclear Fuel Services, Inc. (NFS)

On July 31, 1990, re resentatives of the Office of Nuclear Material Safety and Safeguards (NMSS, Region 11 and the State of Tennessee met with NFS k-representatives to review the status of the plutonium decomissioning project.

Currently, NFS is negotiating with the Department of Energy on an alternative recipient for the plutonium waste. Testing of the volume reduction facility

-is nearing completion, and NFS plans to complete the decomissioning project within the next 20 months. A tour of the waste processing facility and a g

volume reduction demonstration were provided.

Representatives of HMSS, Region II, and NFS also reviewed the status of the decomissioning of the three inactive waste ponds.

Because of the solid waste and water contamination issues, State of Tennessee health and environmental officials were present. Topics of discussion included the status of the decomissioning project, the development of HRC decommissioning criteria, and a recent Environmental Protection Agency inspection of the ponds under the Resources Conservation and Recovery Act.

A tour of the sludge dewatering and packaging facility was provided.

Construction and equipment installation are nearing completion, and initial testing of the facility is scheduled to commence in the near future.

The staff also met with NFS representatives to discuss the revised application for license renewal.

Evaluation of Lcw-Level Waste Regulatory Program In February 1990, the Commission directed the staff to evaluate the NRC regulatory program for low-level waste to confirm that it meets or exceeds the guidelines for Agreement State programs. DOE Quality Assurance Workshop Staff from the Division of High-Level Waste Management (HLWM) observed a workshop in Denver, Colorado, sponsored by the U.S. Department of Energy (00E), on problems associated with the implementation of quality assurance (QA) programs by the national laboratories and other participants on the DOE high-level waste repository program.

A general need for better education in QA program development _and successful methods of implementation was evident.

DOE anticipates further meetings with program participants to resolve the problems-discussed at the workshop.

i Individual-Plant Examination for Severe Accident Vulnerabilities On November 23, 1988 the NRC issued Generic Letter 88-20,_ INDIVIDUAL PLANT EXAMINATION FOR SEVERE ACCIDENT VULNERABILITIES. The generic letter requested all licensees holding operating licenses for nuclear power reactor facilities to perform individual plant examinations for severe accident vulnerabilities individualplantexaminationsofexternalevents(IPEEE)proposedrequestfor due to internal events, including internal flooding. A for severe accident vulnerabilities and a supporting NUREG document were issued on July 25, 1990 l

for public comment. The NRC plans to conduct a workshop in September 1990 to discuss the IPEEE objectives and solicit questions and points for clarification on the draft request and guidance document, NUREG-1407, " Procedural and Sub-mittal Guidance for Individual Plant Examinaticn of External Events (IPEEE) for Severe Accident Vulnerabilities." The staff plans to issue the request and NUREG-1407 before March 1991.

I INTRAVAL Phase 2 Study Group The Office of Nuclear Regulatory Research received an invitation from Dr. Lars 4^

Hognerg, Director General, Swedish Nuclear Power Inspectorate (SKI), to partici-pate in the proposed Phase 2 effort of INTRAVAL (an international cooperative effort in studying) validation of geosphere t o nsport models for assessing geo-logic repositories managed by SKl.

NRC staff and contractors have participated for three years in the Phase 1 effort. The Phase 1 studies have made a signifi-cant contributions both in communicating ideas and information on validation issues related to geosphere transport models associated with geologic disposal of radioactive waste, and in helping to develop an international confidence-building study whereby laboratory and field experimentalists directly interact with mathematical modelers on site-specific experiments. The Phase 1 effort has been of great assistance to NRC's research and licensing programs on radio-active waste disposal, The proposed Phase 2 work involves a three-year additional period in which field-scale studies of ground-water and transport experiments will be conducted and modeled by the various INTRAVAL scientific teams.

Eric Beckjord, Director, RES has designated Thomas J. Nicholson of RES to be NRC's representative to the INTRAVAL Coordinating Group.

Consistent with participation in INTRAVAL's Phase 1, RES has designated various NRC technical and contractor staff for analyzing l

several INTRAVAL problems simultaneously, and has identified RES projects and responsible Principal Investigators for individual INTRAVAL test cases (i.e.,

Las Cruces Trench study for low-level waste and the Apache Leap Tuff studies for high-level weste).

NRC Participation in the PHEBUS-FP Program The Division of Systems Resear:1, is now ready to participate in the Commissariat a 1"Energie Atomique of France (CEA) PHEBUS-FP Program Phase-l. The PHEBUS-FP Program will perform in-pile severe fuel damage experiments and study the fission 5

. product behavior and transport in the reactor system and the containment system.

Phase-I-is the preparation phase whereby the facility.and ex)eriment design is being formulated. The experiment phase is conducted under P1ase-II and we are in the process of entering into a separate agreement with the CEA.

Phase-I covered a two-year period from October 1, 1989 to October 1, 1991 at a cost of

$250,000 U.S. to the USNRC.

This agreement was negotiated some time ago, but its signature was delayed due to the State Department clearance, and need to negotiate with CEA on the new Intellectual Property language.

The final report is expected to be completed on schedule by August 20.

IRB sponsored a Severe Reactor Accident Course on August 6-7 in Region 111 and on August 7-8 in Region V.

This Course was conducted by representatives from Research and the TTC.

On August 8, a Materials event drill was held in the Operations Center.

Part Oipation in the drill included the Protective Measures Team (PMT) and a one member Executive Team (Guy Arlotto was Director of the ET).

EPA also provided a representative to act as a player / controller. The main objective of the drill was to train a number of new PMT members in their response roles during a materials event.

Office of Governmental and Public Affairs items of Interest Week Ending August 10, 1990 State Programs EPA Interagency Meeting on Indian issues Rosetta Virgilio, State Programs, participated in an Environmental Protection Agency Interagency Discussion Group meeting on Indian Issues on August 6, 1990 in Washington, DC. HRC representatives presented a summary of the recent survey of State and Tribal emergency response ct.pabilities for radiological transportation incidents. Meeting participants discussed other Indian-related environmental and health issues.

North Anna Emergency Planning Exercise Robert Trojanowski, Region 11 State Liaison Officer, participated in an emergency planning exercise at the North Anna Power Station on August 6-10, 1990 in Mineral, Virginia. Mr. Trojanowski served as a member of the Regional Assistance Committee which evaluated the exercise.

NCSL Annual Meeting Robert Trojanowski, Region 11 State Liaison Officer, attended the National Conference of State Legislatures annual meeting on August 8, 1990 in Nashville, Tennessee. Mr. Trojanowski attended a roundtable discussion on low-level waste. The session included a presentation by Richard Hodes, Chairman of the Southeast LLRW Compact Commission, on the status of siting and characterizing a disposal facility in the Southeast. A roundtable discussion followed on the success or failure of the other ragional compacts in siting facilities, current litigation and efforts to revisit the Low-Level Radioactive Waste Policy Act and Amendments Act to alter the number of compacts or facilities required to meet the nation's disposal needs. John Bartlett, Director, Office of Civilian Radioactive Waste Management, briefed a joint meeting of the Committee on Science, Energy and Environmental Resources and the Environment and Energy Committees on the status of the high-level radioactive waste program.

' International Visitors Mr. James Rixon from Nuclear Installations Inspectorate (Nil), United Kingdom, visited NRC on August 9, 1990.- He met with the technical staff from RES and NRR to discuss the status of review and assessment of advanced reactors.

He also met with AE00 regarding operational experience with the existing 2-loop plants.

On Thursday, S. Schwartz and D. Office of' Administration items of Interest Week Ending August 10, 1990 Enhanced Security Measures for OWFN Based on recent thefts of certain high value equipment from OWFN, SEC is cot sidering options designed to enhance building security, increase security visibility and deter theft.

Such options include, a search of vehicles exiting s

the 0WFN garage, greater use of security technology (e.g., closed circuit TV, video motion detection and electronic article surveillance), and coordination with the 0WFN facilities staff.

SEC is preparing.an announcement to employees which will include a reminder against unauthorized removal of equipment from the building.

Increase in Fingerprint Processing Fees The FBI has advised they are again increasing the cost for processing fingerprints for Criminal History Checks. Last March 1, 1989, the cost for processing fingerprint cards under the Criminal History Program 00 CFR 73.57) was increased from $15.00-to $21.00.

Effective October 1, 1990, the cost will increase to $23.00. To pass along this cost increase to affected licensees, it will be necessary to revise 10 CFR 73.57. NRR has been notified of this requirement. The fee increase will also have to be passed along by rule change to licensees who are processed for an NRC access authorization (security clearance) under 10 CFR Part 25 or special nuclear material access authorization under Part 11.

NRC employee and contractor cases, including reinvestigations, will also go up slightly.

Deaconess Hospital, Oklahoma City, Oklahoma paid the civil penalty in 1

the amount of $3,750. The action was based on violations involving falsification of an NRC-required record of a dose calibrator constancy r

test and failure to perform the constancy test on a number of occasions.

Thomas Jefferson University, Philadelphia, Pennsylvania paid the civil penalty in the amount of $3.125.

The action was based on an incident in which a 53 millicurie cesium-137 brachytherapy source could not be accounted for and is presumed to have been disposed of in the normal trash.

(EA90-013) 3.

Georgetown University, Washington, DC paid the civil penalty in the amount of $625. The-action was based on a transfer of a 2.1 curie source without proper authorization by the Radiation Control Officer and without proper controls being established. Other violations concerned transportation requirements. The base civil penalty was mitigated 25% for identification and 50% for comprehensive corrective actions.

s Office of Consolidation items of Interest Week Ending August 10, 1990 Workstation Standards Review Representatives of the Office of Consolidation and Dewberry & Davis, the architect / planners for Two White Flint North (TWFN), met with members of the Office of. Nuclear Reactor Regulation, Office of the General Counsel, and the Office of Governmental and Public Affairs on August 7, 1990, to review res-ponses to their recently submitted Workstation Standard Questionnaires. The questionnaires solicited comments from a cross section of employees regarding the effectiveness of the general office environment in One White Flint North (OWFN), as well as the perceived efficiency of the standard open and closed workstation configurations applied in OWFN.

Of particular interest were impressions of the level of visual and acoustical distraction in the open environment, as well as the utility of the systems furniture components in the workstations.

Conclusions drawn _from the questionnaires and intervies process may help in identifying changes in the standard configurations to be used to plan TWFN.

Interaaency Consolidation Managers' Meeting A meeting of the Interagency Co,solidation Managers was held on August 9, 1990, at the IRS Building in Washington, DC. The purpose of this meeting is to periodically bring together facility management professionals and provide a forum for sharing common experiences and to present timely topics regarding ongoing or pending agency consolidation projects.

Consolidation managers from SBA, NOAA, IRS, NARA, EPA, DOT, NASA, GSA, FCC, DOL, NSF, and others partici-

-pate.

Cintichem. Tuxedo, New York 1

Cintichem has a 5 MW pool reactor, hot cells and laboratory for production of radioisotopes for use in radiopharmaceuticals. On February 23, 1990, NRC issued CAL No. 1-90-005, as a result of an incident-involving the discharge of contaminated water from the licensee's onsite retention pond to the Indian Kill Reservoir. The CAL included licensee commitments to be implemented in the event that heavy rains or surface run-off necessitated that the retention pond be emptied or lowered to preserve its integrity.

On July 30, 1990, between apprcximately 1730 and 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />, heavy rains forced the licensee to begin continuous pumping from the retention pond to a point downstream of the Reservoir. On August 7, 1990, at approximately 0010 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br />, the licensee notified Region I that continued heavy rains necessitated the diversion of surface run-off water from storm drains around the retentie pond to a point downstream of the Reservoir'.

Approp-riate monitoring and empling of the discharge was performed. The con-ditions of the CAL were me+.

CONTROL R0D B0 WING AT YANKEE NUCLEAR POWER STATION On June-23, 1990, two Group D control rods in the core peripheral positions-17 and 18 failed to fully insert into the core when the reactor was manually.

tripped.. The trip was part of a planned shutdown sequence for the Cycle 21 refueling outage. The licensee implemented abnormal operating procedures and fully inserted all control rods.

The control rods at Yankee Rowe are of a cruciform design having a neutron absorbing section and a Ziracioy follower that is not used at any other domestic commercial plant.

The sticking problem experienced by these rods is apparently due to bowing of the rods due to irradiation growth. The licensee has experienced sticking of control rods in the past and had a control rod management program in place since 1972.

This program was refined over the years and a formalized control rod management pro. yam was issued in April 1990. The procedure for implementing the program was drafted prior to refueling. The program includes evaluation of flux gradient and empirical data; measurements of control rod bow, length, and end-of-cycle drop times; use of measured data to create a more accurate model; and orienting Group D rods with the bow away from the center of the core.

Following the event of June 23, the licensee has modified the program from-the original 13 rods to all 24 rods.

Based on the results of tests and measurements, the licensee replaced four control rods - at positions 9, 17, 18 and 21. The five Group D rods not replaced (19, 20, AUGUST 10, 1990 ENCLOSURE N

q 22, 23 and 24) were re-oriented with the rod bow away_from the center of core. The measurements taken were used to confirm the bow model and orien-tation concept. The licensee found that the measured rod growth data was conservatively bounded by the linear model, except for two rods; a quad-ratic model provided a better fit of the data. The predicted bow using the quadratic equation was, in general, found to be more conservative than the measured bow of four of five Cycle 20 rods. Predicted bows for Cycle 21 are found to be within the acceptable limit of 0.25 inches.

The licensee will perform control rod drop time tests prior to restart and has committed to do them whenever the reactor is in mode 3.

The licensee has also committed to further validate the bow model with data obtained from domestic BWRs by mid-Cycle 21, and to have an independent peer review done of the bow model by the end of 1990.

NRC Region I will continue follow-up of this issue.

In addition, the Operational Safety Team Inspection which was initiated at the Vogtle site on August 6, 1990 continues. This inspection is being conducted to ascertain the utility's commitment to safe )lant operations and to look into allegations which have been received ay the Region.

To date, no new issues of safety import

Zion Nuclear Power Station Emergency preparedness representatives from the Zion station and the Commonwealth Edison corporate office met with Region 111 emergency preparedness management in the Region 111 office to discuss licensee performance problems with the onsite response organization during a recent exercice and licensee corrective actions which include a followup i

partial exercise scheduled September 10, 1990.

A. Gibson, Team Leader for th. Zion Diagnostic Evaluation Team (DET) inspection, briefed Mr. James raylor. Executive Director for Operation, Dr. Thomas E. Murley, Director, Nuclear Reactor Regulation, Mr. A. Bert Davis, Regional Administrator, Region Ill, and other senior NRC management on the findings of the DET inspection. The briefing included the team's assessment of the root causes for Zion Station's current performance level.

Dresden Nuclear-Station At 1:05 a.m. (CDT) on August 2, 1990, safety relief valve 2A opened j

unexpectedly and remained open.

The reactor was at 87 percent power. The Target Rock safety relief valve which opened and four Electromatic relief valves constitute the automatic depressurization system.

Reactor operators followed abnormal operating procedures, but were unable to close the valve.

The reactor was scrammed manually at 1:16 a.m.

The licensee declared an Unusual Event under its emergency plan.

Steam was discharged through the valve into the pressure suppression pool (torus). The licensee believes the valve' closed at about 3:50 a.m. as indicated by torus temperature and reactor pressure changes.

The Resident Inspectors responded to the site and a specialist was sent from Region 71! to inspect the valve and an electrical junction box found separated from its mount.

Region til issued a-Confirmatory Action Letter documenting the licensee's commitments to certain actions following this event on August 3, 1990. The licensee replaced the' valve, remounted the junction box, and analyzed ~the effects'of the reactor cooldown and discharge to the torus. Testing of the removed valve indicates that there was a mechanical failure of the valve.

The unit was restarted August 8, 1990.

Regional followup on tH s matter is continuing.

Dresden Nuclear Station On August 10, 1990, Dr. Carl J. Paperiello, Deputy Regional Administrator, and other members of the regional staff, conducted an Enforcement Conference with Commonwealth Edison Company representatives to discuss violations associated with the licensee's Environmental Qualification Program at Dresden. The Region is considering escalated enforcement action for these violations.

Clinton Nuclear Power Station Steve Perry, Vice President - Nuclear Illinois Power Company, and members-of his staff met with A. Bert Davis, Regional Administrator, members of his staff, and representatives ~from NRR on August 9, 1990. Current plant status, progress being~made on the Clinton Improvement Plan, and plans and preparation for the refueling outage which begins in October were the primary points of discussion.

Clinton appears to be making slow progress in improving its performance. Major items of concern which remain are the quality of engineering support, plant operations during non-routine evolutions, and the number of events that continue to occur at Clinton.

This use was caused by high-load demand (high temperatures) and the loss of a major intertie supplying power to SMUD.
